What Are the Symptoms of a Blocked Artery in Your Neck

A blocked artery in your neck often produces no symptoms at all until it triggers a serious event like a mini-stroke or a full stroke. The carotid arteries, one on each side of your neck, supply blood to your brain. When fatty plaque narrows these arteries, the first warning sign is frequently a transient ischemic attack (TIA), a brief episode where blood flow to part of the brain is temporarily cut off.

Why Most Blockages Have No Early Symptoms

The frustrating reality of carotid artery disease is that plaque can build up for years without causing anything you’d notice. A blockage can reach 50%, 60%, or even 70% of the artery’s width before you feel a thing. Many people only discover it when a doctor hears an unusual sound through a stethoscope during a routine checkup, or when imaging is done for an unrelated reason.

That sound, called a carotid bruit, resembles a whooshing noise similar to water rushing through a fast-moving river. It’s the sound of blood flowing in a turbulent, choppy pattern instead of smoothly through the artery. To check for it, your doctor presses a stethoscope against several spots on your neck and may ask you to hold your breath for a few seconds while they listen. Not everyone with a blockage has a bruit, and not every bruit means a dangerous blockage, but it’s often the clue that leads to further testing.

Mini-Strokes: The Most Common Warning

A TIA is the body’s clearest alarm that a carotid artery is significantly blocked. It starts suddenly and produces stroke-like symptoms that last anywhere from 2 to 30 minutes before resolving completely. The symptoms include:

  • Sudden numbness or weakness in the face, arm, or leg, typically on one side of the body
  • Sudden confusion or trouble speaking
  • Sudden vision problems in one or both eyes
  • Sudden difficulty walking, dizziness, or loss of coordination
  • Sudden severe headache with no obvious cause

Because these symptoms disappear quickly, many people dismiss them or convince themselves nothing serious happened. That’s a dangerous mistake. A TIA is essentially a stroke that resolved on its own, and it signals that a full, permanent stroke could follow. Roughly 45% of ischemic strokes (the kind caused by a blocked blood vessel) are directly attributable to vascular disease, and carotid blockages are a major contributor.

Temporary Vision Loss in One Eye

One symptom that’s particularly tied to carotid artery blockages is a sudden, temporary loss of vision in one eye. People who experience this often describe it as a gray or black shade coming down over their field of vision, almost like a curtain dropping. The episode typically lasts a few seconds to several minutes, and then vision returns to normal.

This happens when a small piece of plaque or a tiny blood clot breaks loose from the carotid artery and temporarily blocks blood flow to the retina. It affects only the eye on the same side as the blocked artery. Because vision comes back, it’s easy to shrug off, but it carries the same urgency as any other TIA symptom.

Facial Drooping and One-Sided Weakness

When a carotid blockage reduces blood flow enough to affect the brain’s frontal lobe, which controls voluntary muscle movement, the result is often facial drooping or weakness on one side of the body. If the left carotid artery is involved, the right side of the face may droop or twist. If the right carotid is blocked, the left side is affected. The person may have trouble smiling or notice that one side of their mouth sags.

This same pattern applies to the arms and legs. Weakness or numbness on one side of the body, appearing suddenly and without explanation, is the hallmark of disrupted blood flow from a carotid blockage. The FAST acronym (Face drooping, Arm weakness, Speech difficulty, Time to call emergency services) was designed around exactly these symptoms.

Risk Factors That Increase Your Chances

Certain factors make carotid blockages more likely to develop and more likely to cause a stroke once they do. A large study tracking over 1,100 patients with asymptomatic carotid blockages found several characteristics tied to higher stroke risk: a smoking history of 10 or more pack-years increased risk by 65%, a history of blockage or stroke symptoms on the opposite side tripled the risk, and each 10-point rise in systolic blood pressure added about 11% more risk. Older age and impaired kidney function also played a role.

Plaque composition matters too. Blockages with soft, fatty cores are more dangerous than hard, calcified plaques. Imaging studies have shown that these softer plaques more than double the risk of stroke compared to firmer ones, because they’re more likely to rupture and send fragments into the brain’s blood vessels.

How Blockages Are Diagnosed

If a bruit is heard or symptoms suggest carotid disease, the first test is almost always a carotid duplex ultrasound. It’s painless, involves no radiation, and remains the most commonly used technique for evaluating these blockages. The ultrasound uses sound waves to create images of the artery and measure how much it has narrowed.

CT angiography is sometimes used as a follow-up. It provides detailed cross-sectional images of the arteries and is fast and efficient, with less radiation than older catheter-based imaging methods. In some cases, doctors may also use transcranial Doppler ultrasound to detect tiny emboli (microparticles) traveling toward the brain. In one study of 467 patients with blockages of 60% or greater, about 16.5% showed these microembolic signals, and their presence was associated with a sixfold increase in stroke risk over two years.

How Quickly Blockages Get Worse

Carotid plaque doesn’t grow at the same rate in everyone. In one study following patients over three years with standard risk factor treatment, about 18% showed meaningful plaque progression. Over a longer five-year window, that number climbed to 63%. The rate depends heavily on how well risk factors like blood pressure, cholesterol, and smoking are controlled.

An eight-year tracking study found striking differences based on plaque trajectory. Patients whose blockages actually regressed (shrank) had a 0% stroke rate over that period. Those whose blockages stayed stable had a 9% stroke rate, and those whose blockages progressed had a 16% rate. This underscores that the direction of change matters as much as the current degree of narrowing.

When Treatment Becomes Necessary

For people who have experienced symptoms like a TIA or minor stroke, surgical guidelines recommend considering intervention when the artery is more than 50% blocked. The strongest evidence supports surgery, typically a procedure to physically remove the plaque, when the blockage reaches 70% to 99% in a symptomatic patient.

For people with no symptoms, the decision is more complex and depends on plaque characteristics, overall health, and individual risk factors. Many asymptomatic patients are managed with medications to lower cholesterol, control blood pressure, and prevent clot formation, with regular imaging to monitor whether the blockage is progressing or stable.
