Cirrhosis often has no symptoms until liver damage is already serious. Many people learn they have it through routine blood tests or a checkup for something else entirely. As scar tissue gradually replaces healthy liver tissue, the organ loses its ability to filter toxins, produce proteins, and regulate hormones. The symptoms that eventually appear depend on how much function remains.
Why Early Cirrhosis Often Feels Like Nothing
In what doctors call “compensated” cirrhosis, the liver is scarred but still working well enough to handle its basic jobs. At this stage, you may feel completely fine or notice only vague problems: mild fatigue, a general sense of low energy, or a dull ache in the upper right side of your abdomen. These are easy to brush off or blame on stress, poor sleep, or aging.
Some people do develop subtle early signs. Unexplained weight loss, reduced appetite, and nausea can appear before anything more dramatic. But because these symptoms overlap with dozens of other conditions, cirrhosis at this stage is almost always caught by lab work showing abnormal liver enzymes or low platelet counts, not by symptoms alone.
Skin Changes That Signal Liver Trouble
The liver processes a yellow pigment called bilirubin, a byproduct of old red blood cells breaking down. When the liver can’t clear bilirubin efficiently, it accumulates in the blood. Once levels rise above roughly 3 mg/dL, the buildup becomes visible: your skin and the whites of your eyes turn yellow. This is jaundice, and it’s one of the most recognizable signs that the liver is struggling.
Two other skin findings are closely linked to cirrhosis. Spider angiomas are tiny red spots, each with a visible central dot surrounded by thin radiating blood vessels that look like spider legs. They’re usually a few millimeters across and blanch when you press on them, then quickly refill with blood. While about 10 to 15 percent of healthy adults have one or two, multiple spider angiomas clustered on the face, neck, chest, or arms suggest liver dysfunction. The more you have, the more severe the underlying liver disease tends to be.
Palmar erythema, a persistent reddening of the palms, is the other hallmark. Both spider angiomas and red palms are driven partly by the same problem: the damaged liver can no longer properly break down estrogen, so levels rise and dilate small blood vessels near the skin’s surface.
Fluid Buildup in the Abdomen and Legs
As cirrhosis progresses, pressure builds inside the portal vein, the large blood vessel that carries blood from your digestive organs to the liver. When that pressure climbs above a certain threshold, fluid begins leaking out of blood vessels and collecting in the abdominal cavity. This is called ascites, and it can range from a barely detectable amount to liters of fluid that make your belly visibly swollen and tight.
The mechanism involves more than just high pressure. The scarred liver also produces less albumin, a protein that normally keeps fluid inside blood vessels. Without enough albumin, fluid seeps out more easily. At the same time, blood vessels in the gut dilate, and the body’s lymphatic drainage system becomes overwhelmed. Abdominal lymph production can increase by as much as 30-fold in cirrhosis.
You might notice your pants feeling tighter, unexpected weight gain over days or weeks, or difficulty breathing when lying flat because the fluid presses upward against your diaphragm. Swelling in the ankles and legs (edema) often accompanies ascites for the same reasons: too much pressure, too little protein.
Itching That Disrupts Sleep
Persistent, intense itching is one of the more miserable symptoms of cirrhosis. It’s caused by bile salts and other compounds accumulating in the bloodstream when the liver can’t process them normally. The itching tends to concentrate on the palms of the hands and soles of the feet, though it can spread everywhere. It’s typically worst at night, which disrupts sleep and compounds the fatigue that cirrhosis already causes.
This isn’t the kind of itch that comes with a rash or dry skin. The skin often looks completely normal, which can make it frustrating to explain. Scratching doesn’t relieve it in any lasting way because the irritation originates from chemicals circulating in the blood, not from anything happening on the skin’s surface.
Confusion and Personality Changes
One of the liver’s critical jobs is filtering ammonia, a toxic waste product of protein digestion, out of blood coming from the gut. When the liver fails at this task, ammonia and other toxins build up in the bloodstream and eventually cross into the brain. The result is hepatic encephalopathy, a spectrum of cognitive problems that ranges from barely noticeable to life-threatening.
At the mildest end, you or your family might notice small lapses in short-term memory, slower reaction times, or trouble concentrating. These changes can be so subtle that only people who know you well would pick up on them. As it worsens, the signs become harder to miss: mood swings, anxiety or unusual euphoria, difficulty with simple math, trouble writing or handling small objects, and a flipped sleep schedule where you’re drowsy during the day and wide awake at night.
More advanced stages bring clear personality changes and inappropriate behavior that’s out of character. Eventually, severe confusion, delirium, involuntary tremors, and a distinctive hand-flapping tremor can develop. At its worst, hepatic encephalopathy progresses to total loss of consciousness.
Signs of Internal Bleeding
The same portal vein pressure that causes ascites also forces blood to find alternate routes around the liver. Fragile, swollen veins called varices develop in places that weren’t designed to handle high blood flow, particularly the esophagus and stomach. These varices can rupture without warning.
The two key warning signs are vomiting large amounts of blood (which may look bright red or like dark coffee grounds) and stools that are black, tarry, or visibly bloody. Black stool gets its color from blood that has been digested as it passes through the intestines. Either of these is a medical emergency. Variceal bleeding is one of the most dangerous complications of cirrhosis.
Separately, you might notice that you bruise more easily than usual or that small cuts bleed longer than they should. The liver produces clotting proteins, and as cirrhosis advances, it can’t make enough of them. Low platelet counts, which are common in cirrhosis because the spleen enlarges and traps platelets, add to the problem.
Hormonal and Body Composition Changes
Because the damaged liver can’t properly metabolize hormones, estrogen levels rise in men while testosterone drops. This can cause gynecomastia (breast tissue growth), shrinking of the testicles, erectile dysfunction, and loss of body hair. Women with cirrhosis may experience irregular or absent periods.
Muscle wasting is another common change. The liver plays a central role in protein metabolism, and as it fails, the body struggles to maintain muscle mass. You might notice your arms and legs getting thinner even as your abdomen grows from fluid retention. Changes in how the body stores fat can also alter your overall appearance.
Other Visible Signs
A handful of additional physical changes can develop as cirrhosis progresses. Finger clubbing, where the fingertips widen and the nails curve downward, occasionally appears. Some people develop white discoloration of the nails. Enlarged veins radiating outward from the belly button, sometimes called caput medusae, can become visible on the abdominal wall as blood seeks detour routes around the scarred liver. An enlarged spleen, detectable as fullness or discomfort in the upper left abdomen, is common because the backed-up portal blood flow causes the spleen to swell.
Compensated vs. Decompensated Cirrhosis
The distinction matters because it changes both the outlook and the urgency. In compensated cirrhosis, the liver is scarred but managing. You may have few or no symptoms, and the focus is on protecting remaining liver function and treating the underlying cause, whether that’s alcohol, a viral infection, or fatty liver disease.
Decompensated cirrhosis is defined by the arrival of major complications: jaundice, ascites, variceal bleeding, hepatic encephalopathy, or liver cancer. Once any of these develops, the disease has entered a more serious phase. Doctors use scoring systems that combine lab results like bilirubin, albumin, and clotting time with clinical findings like ascites and encephalopathy to gauge severity and guide decisions about treatment, including whether a liver transplant evaluation is appropriate.
The transition from compensated to decompensated cirrhosis doesn’t always happen gradually. Some people remain stable for years, then develop ascites or bleeding after an infection, a medication change, or continued exposure to whatever caused the liver damage in the first place. That unpredictability is part of why monitoring matters even when you feel fine.
