Mercury poisoning causes a range of symptoms depending on the type of mercury involved and whether exposure happened all at once or gradually over time. The most common signs include tremors in the fingers and hands, mood changes like irritability and anxiety, numbness or tingling in the extremities, and difficulty with coordination. Because early symptoms are often vague, mercury poisoning is frequently misdiagnosed as other conditions.
Why the Type of Mercury Matters
Mercury exists in three forms, and each one enters and affects the body differently. Elemental mercury is the liquid silver metal found in older thermometers and some industrial settings. It becomes dangerous when it evaporates into a vapor you breathe in. Organic mercury, primarily methylmercury, builds up in fish and shellfish and enters your body through your diet. Inorganic mercury salts are found in some industrial chemicals and older consumer products. The form of mercury determines which organs take the biggest hit: elemental mercury vapor targets the lungs and brain, methylmercury concentrates in the nervous system, and inorganic mercury salts damage the digestive tract and kidneys.
Symptoms of Breathing in Mercury Vapor
Elemental mercury vapor is the most common source of acute poisoning, typically from broken mercury-containing devices or occupational exposure. After a high-level exposure, you may feel fine for the first one to four hours. Symptoms then start abruptly: fever, chills, nausea, chest tightness, shortness of breath, and a dry, persistent cough. In severe cases, fluid can accumulate in the lungs within days.
Chronic, lower-level exposure to mercury vapor produces a very different picture. The nervous system bears the brunt, and the symptoms creep in so gradually that they’re easy to dismiss. Fine tremors in the fingers, eyelids, and lips are among the earliest signs. Over time, hand tremors can worsen enough to interfere with everyday tasks like writing. Alongside the physical symptoms, psychological changes develop: depression, irritability, emotional instability, excessive shyness, insomnia, and an exaggerated startle response. The CDC has noted that the vagueness of these early signs often leads to misdiagnosis. Inflammation of the throat and upper airway can also develop with repeated exposure, and in rare cases, long-term lung scarring has been reported.
Symptoms From Methylmercury in Food
Methylmercury is the form most people encounter, primarily through eating fish with higher mercury levels like swordfish, shark, king mackerel, and tilefish. It crosses from the bloodstream into the brain more readily than other forms, which is why its effects are overwhelmingly neurological.
The hallmark symptom is numbness and tingling, usually starting in the fingers and toes and sometimes spreading to the area around the mouth. This happens because methylmercury selectively destroys sensory nerves while largely sparing motor nerves. Coordination problems follow as the cerebellum, the brain region responsible for balance and movement, sustains damage to a specific layer of cells. People with significant methylmercury poisoning may develop slurred speech, narrowed visual fields, difficulty hearing, and a stumbling, unsteady gait. These were the symptoms documented extensively in Minamata, Japan, where industrial mercury contamination of seafood caused widespread poisoning in the 1950s.
For most people, dietary methylmercury exposure stays well below toxic levels. The risk increases with frequent consumption of large predatory fish over months or years. Pregnant and breastfeeding women face the greatest concern because methylmercury readily crosses the placenta and can affect fetal brain development at levels that might not cause obvious symptoms in the mother.
Symptoms From Inorganic Mercury Salts
Inorganic mercury compounds are corrosive on contact. Swallowing them causes immediate, severe burning pain in the mouth, throat, and stomach, often followed by bloody diarrhea and vomiting. The kidneys are particularly vulnerable because they filter mercury from the blood, and kidney damage can develop rapidly after ingestion. Skin and eye contact with inorganic mercury salts causes burns and irritation.
How Symptoms Differ in Children
Children exposed to mercury, particularly inorganic forms, can develop a condition called acrodynia, sometimes known as pink disease. The hands and feet become pink, swollen, painful, and tender, with skin peeling and damaged fingernails. Children with acrodynia often also experience excessive sweating, a rapid heartbeat, and high blood pressure. Beyond these visible signs, mercury-exposed children may show developmental regression, losing skills they had previously gained, along with failure to thrive. Because young children can’t always describe what they’re feeling, the physical signs of acrodynia can be an important clue that mercury exposure has occurred.
Acute vs. Chronic: How Timing Changes the Picture
A single high-dose exposure and months of low-level exposure produce strikingly different symptom patterns. Acute poisoning hits fast and hard, primarily affecting the lungs and gut depending on whether the mercury was inhaled or swallowed. Respiratory distress, chest pain, nausea, and fever dominate the picture.
Chronic poisoning is subtler and more insidious. Symptoms build gradually as mercury accumulates in the body, and they worsen as a function of how much mercury you’re exposed to, how long the exposure lasts, and your individual sensitivity. The nervous system is always the primary target in chronic cases. Tremors, mood disturbances, cognitive difficulties like forgetfulness and trouble concentrating, fatigue, and loss of appetite develop over weeks to months. Many people don’t connect these symptoms to mercury exposure because each one, taken individually, has dozens of other possible explanations.
How Mercury Poisoning Is Detected
If mercury poisoning is suspected, the most common test measures mercury levels in a 24-hour urine collection. For adults, the normal range is below 2 micrograms per 24 hours. Toxicity generally becomes apparent at levels above 50 micrograms per 24 hours, a threshold established by the World Health Organization. Blood tests can also measure mercury levels, though urine testing is more widely used for assessing ongoing exposure. Reference ranges for children under 18 have not been formally established, which can complicate diagnosis in younger patients.
How Mercury Poisoning Is Treated
Treatment centers on removing the source of exposure and, when mercury levels are high enough, using medications called chelators that bind to mercury in the body so it can be excreted in urine. For acute poisoning from swallowing inorganic mercury salts, treatment is given by injection and works even if the kidneys are already damaged, since the mercury complex can be removed through dialysis. For chronic or milder cases, an oral chelator is preferred because it has fewer side effects and is easier to tolerate.
The nervous system damage from mercury can be permanent if exposure continues long enough. Tremors, memory problems, and mood changes sometimes persist even after mercury levels return to normal, particularly with methylmercury poisoning where sensory nerve damage may be irreversible. Early identification and removal of the mercury source gives the best chance of full recovery.
