What Are the Worst Side Effects of Letrozole?

The worst side effects of letrozole are joint and muscle pain, bone loss, cardiovascular risks, and cognitive changes. While many people tolerate the drug, nearly half of long-term users develop musculoskeletal symptoms significant enough to affect daily life, and roughly 1 in 20 stop treatment entirely because of side effects. Here’s what the most serious side effects look like and what you can do about them.

Joint and Muscle Pain

Joint pain is the single most reported complaint with letrozole. In clinical studies, about 30% of users report joint pain specifically, while muscle or joint stiffness affects another 8%. But the full picture is worse than individual numbers suggest: a large meta-analysis covering more than 13,000 participants found that musculoskeletal symptoms overall affect 46% of people on aromatase inhibitors like letrozole, with individual studies reporting rates anywhere from 20% to 74%.

The pain tends to affect the hands, wrists, knees, and feet. It often feels like stiffness that’s worst in the morning and improves with movement, though for some people it remains constant. This isn’t minor discomfort for many users. In one trial, when patients who quit letrozole due to musculoskeletal pain were switched to a different aromatase inhibitor, 41% of them discontinued the second drug for the same reason. The pain appears to be a class-wide effect driven by estrogen suppression, not something unique to one formulation.

Bone Thinning and Fracture Risk

Letrozole works by blocking estrogen production, and estrogen plays a key role in maintaining bone density. Without it, bones lose mineral content faster than they rebuild. Over years of treatment, this can progress from mild thinning (osteopenia) to osteoporosis, raising the risk of fractures in the spine, hip, and wrist. This is particularly concerning because letrozole is often prescribed for five years or longer.

Bone density monitoring is recommended throughout treatment. Weight-bearing exercise, calcium, and vitamin D supplementation are standard strategies to slow bone loss, and in some cases, bone-strengthening medications are added to the regimen.

Hot Flashes and Fatigue

Hot flashes affect about 25% of users, making them the second most common side effect after joint pain. They range from mild warmth to drenching episodes that disrupt sleep. New or worsening fatigue hits roughly 16% of users and can compound the effects of poor sleep caused by hot flashes and insomnia.

Both side effects stem from the same mechanism: a sudden, deep drop in estrogen. For women who haven’t yet gone through menopause naturally, or who transitioned recently, these symptoms can feel especially intense because the hormonal shift is more dramatic.

Cardiovascular Risks

Letrozole carries real cardiovascular concerns that don’t get as much attention as joint pain. A population-based study published in Circulation tracked aromatase inhibitor users and found the following incidence rates per 1,000 person-years of use:

  • Heart attack: 3.9 cases
  • Ischemic stroke: 5.6 cases
  • Heart failure: 5.4 cases
  • Cardiovascular death: 9.5 cases

Some clinical trials have also linked aromatase inhibitors to rising cholesterol levels, though results are mixed. Estrogen has a protective effect on blood vessels and lipid balance, so removing it from the equation may shift cardiovascular risk. Cholesterol monitoring during treatment is recommended for this reason.

Cognitive and Mood Changes

Many people on letrozole describe what’s commonly called “brain fog,” and research supports that this isn’t imagined. Studies have found reduced activity in the hippocampus, the brain region central to memory, in women taking letrozole. Some research documents measurable memory deficits, though not all studies agree on the severity.

Depression and anxiety are also associated with aromatase inhibitor use. Estrogen influences neurotransmitter systems that regulate mood, and suppressing it can push some people toward symptoms they’ve never experienced before. Insomnia compounds both cognitive and mood effects, creating a cycle where poor sleep worsens thinking and emotional well-being, which in turn makes sleep harder.

Rare but Serious Events

Beyond the common side effects, letrozole carries rare risks that require immediate attention if they occur. These include blood clots (particularly in the legs or lungs), heart attack, and stroke. Warning signs include sudden chest pain, leg pain or swelling in the calves, unexplained shortness of breath, sudden severe headache, slurred speech, sudden weakness or numbness on one side of the body, and vision changes. These events are uncommon but can be life-threatening.

Short-Term Use for Fertility Is Different

If you’re taking letrozole for ovulation induction rather than breast cancer, your experience will likely be very different. Fertility protocols typically involve just five days of treatment per menstrual cycle, compared to years of daily use in oncology settings. At that short duration, the most common complaints are mild hot flashes, headaches, and occasionally bloating. The serious long-term risks like bone loss, cardiovascular changes, and persistent joint pain are associated with months and years of continuous estrogen suppression, not brief courses.

Why People Stop Treatment

In the MA.17 trial, which followed over 5,000 women on letrozole after prior breast cancer treatment, 4.5% discontinued the drug due to toxic side effects. That number may sound small, but it represents people whose symptoms were severe enough to outweigh the cancer-prevention benefit in their judgment. The real rate of people struggling is much higher: studies consistently show that quality-of-life impacts from musculoskeletal pain, fatigue, and mood changes affect daily functioning for a large proportion of users, even among those who stay on the medication.

Switching between different aromatase inhibitors sometimes helps, but not always. In one study, 41% of patients who tried a second aromatase inhibitor after quitting the first one ended up stopping that one too, for the same musculoskeletal reasons. For some people, the side effects are a class-wide problem rather than something specific to letrozole.
