The most common blood test for inflammation is C-reactive protein, usually called CRP. A healthy CRP level is 1.0 mg/dL or lower, and anything above that suggests inflammation somewhere in the body. But CRP isn’t the only option. Doctors often order it alongside other markers, and the combination of results paints a much clearer picture than any single test.
CRP: The Most Common Inflammation Test
CRP is a protein your liver produces in response to inflammation. It’s the go-to test because it reacts quickly. When inflammation flares, CRP levels rise within hours and drop relatively fast once the inflammation resolves. That speed makes it useful both for detecting new problems and for tracking whether treatment is working.
A standard CRP test measures general inflammation. A more sensitive version, called high-sensitivity CRP (hs-CRP), detects much lower levels and is used specifically to assess cardiovascular risk. An hs-CRP reading above 3 mg/L signals increased inflammatory risk for heart disease, even in people who feel fine. The American College of Cardiology considers this threshold clinically meaningful for identifying patients who may benefit from closer monitoring.
One important caveat: CRP runs higher in people with obesity and related metabolic conditions. A single elevated reading doesn’t automatically mean something is wrong. Your doctor will typically look at CRP alongside your symptoms, medical history, and other lab results.
ESR: A Slower but Useful Companion Test
The erythrocyte sedimentation rate, or ESR, takes a different approach. A lab technician places your blood sample in a tall, thin tube and measures how fast your red blood cells sink to the bottom. Normally, red blood cells settle slowly. But when you have inflammation, proteins released into the blood cause red blood cells to clump together. These clumps are heavier, so they sink faster. A higher sedimentation rate means more inflammation.
ESR is less precise than CRP because many things affect it beyond inflammation. Normal values shift with age (higher after 50), sex (higher in women), and other factors including pregnancy, obesity, alcohol use, and exercise. Medications matter too. Statins and anti-inflammatory drugs like ibuprofen can push ESR readings lower, potentially masking real inflammation. Low red blood cell counts can falsely raise the number.
Despite these limitations, ESR remains widely used because it’s inexpensive and helpful for monitoring chronic inflammatory conditions like rheumatoid arthritis or lupus over time. Doctors frequently order CRP and ESR together since each captures slightly different information.
White Blood Cell Count and Differential
A complete blood count, or CBC, is one of the most commonly ordered blood panels, and the white blood cell breakdown it provides offers indirect clues about inflammation. Different types of white blood cells respond to different kinds of inflammation:
- Neutrophils spike during acute inflammation, such as from a severe burn, heart attack, or sudden infection.
- Monocytes tend to rise with chronic, ongoing inflammation.
- Basophils can increase in chronic inflammatory conditions like lupus, Crohn’s disease, and allergies.
- Eosinophils climb in allergic reactions and certain inflammatory conditions, including parasitic infections.
A CBC won’t confirm inflammation on its own, but an abnormal white cell pattern often prompts doctors to order more specific tests like CRP or ESR.
Ferritin: Not Just an Iron Test
Most people know ferritin as a measure of iron stores, but it also rises during inflammation. Your body produces extra ferritin as part of its inflammatory response, which means an elevated ferritin level doesn’t always mean you have too much iron. Conditions like rheumatoid arthritis, liver disease, and overactive thyroid can all push ferritin up through inflammation alone.
Normal ferritin ranges are 24 to 336 micrograms per liter for men and 11 to 307 for women. When ferritin comes back high and iron overload has been ruled out, doctors often interpret it as evidence of an inflammatory process.
IL-6: A More Specialized Marker
Interleukin-6, or IL-6, is a signaling molecule that acts as an early alarm during infection and inflammation. Normal levels are below 6.4 pg/mL. During inflammatory conditions, IL-6 can rise severalfold. It’s elevated in infections, sepsis, autoimmune diseases like rheumatoid arthritis and lupus, inflammatory bowel disease, and was notably high in severe COVID-19 cases.
IL-6 isn’t part of routine screening. It’s typically ordered when doctors need to gauge the intensity of an inflammatory response, particularly in hospitalized patients or those with complex autoimmune conditions. Like CRP, it’s nonspecific: a high level tells you inflammation is happening but not where or why. And a normal IL-6 doesn’t completely rule out inflammation.
Procalcitonin: Separating Infection From Inflammation
When the question shifts from “is there inflammation?” to “is this a bacterial infection?”, procalcitonin becomes relevant. This marker rises much more dramatically with bacterial infections than with other types of inflammation. Typical bacterial pneumonia, for example, pushes procalcitonin to around 2.5 ng/mL, while atypical bacterial infections produce a much smaller bump of about 0.2 ng/mL.
A procalcitonin level below 0.25 ng/mL, combined with clinical judgment, can support a decision to stop or avoid antibiotics. That said, procalcitonin can also rise after major trauma, surgery, cardiac arrest, and pancreatitis, so it’s not a perfect separator of infection from inflammation.
What Can Affect Your Results
Several everyday factors can shift inflammation markers up or down, sometimes enough to change clinical decisions. Statins and NSAIDs (like ibuprofen or naproxen) lower ESR readings, which could hide real inflammation. Obesity independently raises CRP. Vigorous exercise and chronic alcohol use can both affect ESR. Even your menstrual cycle can cause fluctuations.
If you’re having blood drawn for inflammation markers, mention all medications and supplements you’re taking. Your doctor may also want to repeat the test after a few weeks to confirm the pattern, since a single snapshot can be misleading. Two or three consistent readings carry more diagnostic weight than one surprising result.
Why Doctors Often Order Multiple Tests
No single blood test tells the full inflammation story. CRP reacts fast and reflects what’s happening right now, while ESR moves more slowly and captures broader trends. Ferritin adds context when iron-related conditions are on the table. IL-6 and procalcitonin help in more acute or complex scenarios. A CBC with differential shows which immune cells are activated and whether the pattern looks acute or chronic.
The combination your doctor orders depends on what they’re looking for. Routine screening for cardiovascular risk might involve only hs-CRP. Evaluating joint pain and fatigue might call for CRP, ESR, and ferritin together. Sorting out a possible infection in a hospitalized patient could add procalcitonin and IL-6 to the panel. Each marker fills in a different piece of the puzzle, and together they help narrow down what’s driving the inflammation and how aggressively it needs to be addressed.
