Elevated blood pressure has dozens of potential causes, ranging from everyday habits to underlying medical conditions you may not know about. In 85% to 95% of cases, no single identifiable cause exists. The remaining cases trace back to a specific condition, medication, or other trigger. Understanding what pushes your numbers up is the first step toward bringing them down.
For reference, normal blood pressure is below 120/80 mmHg. Readings of 120 to 129 systolic (the top number) with a bottom number still under 80 are classified as elevated. Stage 1 hypertension starts at 130/80, and stage 2 begins at 140/90 or higher.
Why Most Cases Have No Single Cause
Doctors divide high blood pressure into two categories. Primary (or essential) hypertension accounts for the vast majority of cases and develops gradually over years without a clear trigger. It results from a combination of genetics, aging, body composition, and lifestyle factors that interact in complex ways. You won’t get a blood test that points to one broken mechanism.
Secondary hypertension, by contrast, is caused by an identifiable condition. It tends to appear more suddenly, produce higher readings, and respond less well to standard treatment. Common culprits include narrowing of the arteries that supply the kidneys, hormone-producing adrenal tumors, thyroid disorders, and certain rare genetic mutations. When blood pressure spikes unexpectedly or resists multiple medications, doctors typically investigate these secondary causes.
Excess Weight and How It Raises Pressure
Carrying extra weight, particularly around the midsection, is one of the strongest modifiable risk factors for high blood pressure. The connection goes far deeper than simply having a larger body that requires more blood flow.
Fat stored around and inside the kidneys physically compresses them, which disrupts their ability to regulate fluid balance. At the same time, excess insulin (common in people with insulin resistance) ramps up your body’s fight-or-flight nervous system, keeping blood vessels in a tighter, more constricted state. Fat tissue surrounding blood vessels also interferes with a key molecule that normally helps arteries relax, while producing inflammatory signals and stress chemicals that stiffen vessel walls over time. These overlapping pathways explain why even modest weight loss often produces a noticeable drop in blood pressure.
Chronic Stress and Cortisol
A stressful moment raises your blood pressure temporarily. That’s normal. But when stress becomes chronic, the hormonal machinery behind those spikes starts doing lasting damage.
A large study tracked by the National Heart, Lung, and Blood Institute found that people with high levels of stress hormones in their urine were significantly more likely to develop high blood pressure over the following six to seven years. Cortisol levels were especially concerning: over an 11-year follow-up, each doubling of urinary cortisol was associated with a 90% increased risk of cardiovascular events like heart attacks and strokes. Chronic stress doesn’t just feel bad. It reshapes your cardiovascular risk profile over time.
Genetics and Family History
Your genes play a meaningful role in determining your blood pressure. Twin studies estimate that roughly 40% to 60% of the variation in systolic blood pressure between individuals is heritable. Diastolic pressure has a somewhat lower heritability, around 30% to 40%. No single gene controls blood pressure on its own. Instead, hundreds of small genetic variants each nudge your baseline risk up or down.
What this means in practice: if both your parents had high blood pressure, your own risk is substantially higher than average, even if you maintain a healthy lifestyle. That doesn’t make prevention pointless. It means you benefit more from the controllable factors, not less.
Medications That Can Raise Blood Pressure
Some of the most common over-the-counter and prescription drugs can push blood pressure higher, sometimes without you realizing it.
- Pain relievers (NSAIDs): Ibuprofen (Advil) and naproxen (Aleve) cause the body to retain sodium and fluid, which raises pressure. This effect is more pronounced with regular use.
- Decongestants: Pseudoephedrine and phenylephrine, found in many cold and sinus medications, work by narrowing blood vessels in your nasal passages. They narrow blood vessels elsewhere too.
- Hormonal birth control: Pills and patches containing hormones can raise blood pressure in some people. Most carry a warning about this side effect.
- Antidepressants: Several classes of antidepressants can elevate blood pressure, including older types like tricyclics and MAO inhibitors, as well as some newer SSRIs.
- ADHD stimulants: Medications like methylphenidate (Ritalin, Concerta) increase heart rate and blood pressure as part of their stimulant effect.
- Caffeine: Found in coffee, energy drinks, and caffeine pills, it can cause short-term spikes, especially if you don’t consume it regularly.
Herbal supplements can also be a hidden source. Licorice root, ginseng, guarana, and ephedra (ma-huang) have all been linked to blood pressure increases. If you take supplements and your readings are creeping up, it’s worth checking the ingredient list.
Salt, Alcohol, and Other Dietary Factors
Sodium is the most well-established dietary driver of blood pressure. When you consume more salt than your kidneys can efficiently excrete, your body retains extra fluid to dilute it. That extra fluid increases the volume of blood your heart has to pump, raising pressure against artery walls. Most people consume far more sodium than the recommended limit of about 2,300 mg per day, largely from processed and restaurant foods rather than the salt shaker.
Alcohol raises blood pressure through multiple pathways, including stimulating your stress hormones and reducing the effectiveness of your blood vessels’ relaxation signals. The effect is dose-dependent: the more you drink, the greater the impact. Heavy drinking over time can cause sustained elevation that persists even between drinks. Cutting back typically produces measurable improvements within weeks.
A diet low in potassium also contributes. Potassium helps your kidneys flush out sodium. When intake is low (common in diets lacking fruits, vegetables, and beans), sodium’s blood-pressure-raising effect is amplified.
Physical Inactivity and Aging
Regular physical activity keeps arteries flexible and helps your heart pump blood more efficiently at lower pressures. A sedentary lifestyle does the opposite. Over months and years, inactivity allows arteries to stiffen, the heart to work harder at rest, and weight to accumulate, all of which push readings upward.
Aging itself is a major factor. Arteries naturally lose elasticity over decades as their walls accumulate collagen and calcium. This stiffening means the heart has to push blood against greater resistance, which is why systolic blood pressure tends to rise steadily after age 40 even in otherwise healthy people. You can’t stop aging, but exercise, diet, and maintaining a healthy weight can slow the rate at which your arteries stiffen.
Sleep Apnea
Obstructive sleep apnea, where the airway repeatedly collapses during sleep, is one of the most underdiagnosed causes of resistant high blood pressure. Each time breathing stops, oxygen levels drop and your body mounts a stress response that spikes blood pressure and heart rate. Over time, these repeated surges remodel blood vessels and keep daytime pressure elevated. If you snore heavily, wake up feeling unrested, or have blood pressure that doesn’t respond well to medication, sleep apnea is worth investigating. Treating it with a continuous positive airway pressure (CPAP) device often brings blood pressure down noticeably.
Kidney Disease
The kidneys and blood pressure exist in a feedback loop. Your kidneys regulate how much sodium and fluid your body retains, which directly controls blood volume and pressure. When kidney function declines, this system breaks down. The kidneys hold on to too much sodium and fluid, and they also release signals that constrict blood vessels. High blood pressure, in turn, damages the kidneys further, creating a cycle that accelerates both conditions. Chronic kidney disease is both a cause and a consequence of hypertension, which is why routine blood work that checks kidney function is part of any high blood pressure evaluation.
Recreational Drugs and Nicotine
Cocaine, amphetamines, and ecstasy (MDMA) all cause sharp, sometimes dangerous blood pressure spikes by flooding the body with stimulating signals that constrict blood vessels and accelerate heart rate. These spikes can be severe enough to trigger strokes or heart attacks, even in young people.
Nicotine, whether from cigarettes, vapes, or chewing tobacco, raises blood pressure acutely every time you use it by tightening blood vessels and triggering adrenaline release. While the spike from a single cigarette is temporary, repeated use throughout the day means your blood pressure is elevated for most of your waking hours. Over time, nicotine also accelerates artery stiffening, compounding the problem.
