Swollen feet happen when fluid builds up in the tissues of your lower extremities, a condition called edema. The causes range from something as simple as sitting too long to serious conditions involving your heart, kidneys, or liver. Understanding the difference helps you figure out whether your swelling is a minor inconvenience or something that needs medical attention.
Prolonged Sitting, Standing, and Diet
The most common and least worrisome causes of foot swelling are lifestyle-related. Gravity constantly pulls fluid downward, and your leg muscles act as pumps to push it back up toward your heart. When you sit or stand in the same position for hours, those pumps aren’t working, and fluid pools in your feet and ankles. Long flights, desk jobs, and road trips are classic triggers.
Eating too much salt is another frequent culprit. Sodium causes your body to hold onto water, and that extra fluid tends to settle in your lower legs. The American Heart Association recommends no more than 2,300 milligrams of sodium per day, with an ideal target of 1,500 milligrams for most adults. For context, a single fast-food meal can easily exceed 2,000 milligrams. If your feet swell after a salty dinner or a weekend of eating out, the connection is likely direct.
Hot weather also plays a role. Heat causes blood vessels to expand, which allows more fluid to leak into surrounding tissues. Combine a hot day with prolonged standing, and the swelling can be significant even in otherwise healthy people.
Heart, Kidney, and Liver Disease
When foot swelling is persistent or worsening over time, it can signal that a major organ isn’t working properly. Heart failure, kidney failure, and liver failure are three of the most common systemic causes.
In heart failure, the heart can’t pump blood efficiently, so pressure builds in the veins of your legs and fluid leaks into the tissues. The swelling is typically worse at the end of the day and may improve overnight when you’re lying flat. You might also notice shortness of breath, fatigue, or a need to prop yourself up on pillows to sleep comfortably.
Kidney disease causes swelling through a different mechanism. Your kidneys filter excess fluid and sodium from your blood. When they can’t keep up, that fluid has to go somewhere, and it often ends up in your feet, ankles, and around your eyes. Liver disease, particularly cirrhosis, reduces production of a protein called albumin that helps keep fluid inside your blood vessels. Without enough of it, fluid seeps into surrounding tissues and collects in the lower body.
Medications That Cause Swelling
Foot swelling is a recognized side effect of several common medications. If your swelling started or worsened after beginning a new prescription, the drug itself may be responsible.
Calcium channel blockers, a widely prescribed class of blood pressure medication, are among the most frequent offenders. Amlodipine is a common example. Nearly half the people who take calcium channel blockers experience some degree of foot and ankle swelling. Other blood pressure drugs, including beta blockers, clonidine, and hydralazine, can do the same.
Beyond blood pressure medications, the list includes:
- Hormone therapies: corticosteroids, estrogen, progesterone, and testosterone
- Nerve pain and seizure drugs: gabapentin and pregabalin
- NSAIDs: common over-the-counter pain relievers like ibuprofen and naproxen
- The diabetes drug pioglitazone
- Certain antidepressants (MAOIs)
- Chemotherapy agents
If you suspect a medication is causing your swelling, don’t stop taking it on your own. Your prescriber can often adjust the dose or switch to an alternative.
Vein and Lymphatic Problems
Your circulatory system has two drainage networks in your legs: veins that carry blood back to the heart, and lymphatic vessels that circulate immune fluid through your tissues. Problems with either system can cause persistent swelling.
Chronic venous insufficiency happens when the one-way valves inside your leg veins weaken or fail. Blood that should be flowing upward instead pools in the lower legs. Over time, this leads to swelling that worsens throughout the day, along with skin discoloration, a feeling of heaviness, and sometimes slow-healing wounds on the lower legs. Varicose veins are a visible sign that venous valves aren’t working well.
Lymphedema occurs when lymphatic fluid can’t drain properly and accumulates in the tissues, most often in the arms or legs. It can develop after surgery or radiation therapy that damages lymph nodes, or it can appear without a clear trigger. Both conditions share symptoms in their later stages, including limb heaviness, skin changes, and blisters or wounds that won’t heal. The key difference is in the type of fluid involved and the treatment approach, so getting the right diagnosis matters.
Pregnancy-Related Swelling
Some degree of foot and ankle swelling is normal during pregnancy, especially in the third trimester. Your body produces roughly 50% more blood and fluid to support the baby, and the growing uterus puts pressure on the veins that return blood from your legs. Most pregnant women notice swelling that comes and goes, tends to be worse in the evening, and improves with rest and elevation.
The warning signs that something more serious is happening include swelling that gets worse suddenly, unexpected swelling in your face or hands, or swelling paired with headaches or vision changes. These can indicate pre-eclampsia, a condition where blood pressure rises to dangerous levels during pregnancy. A sudden increase in swelling during pregnancy warrants a prompt call to your healthcare team.
Blood Clots and Other Emergencies
Swelling that appears suddenly in one leg, especially when accompanied by pain, warmth, or redness, can indicate a deep vein thrombosis (DVT), a blood clot in one of the deep veins of your leg. DVT is a medical emergency because the clot can break free and travel to your lungs.
Seek emergency care if leg swelling comes with chest pain, difficulty breathing, coughing up blood, fainting, or dizziness. These may be signs of a clot that has already reached the lungs. Also get immediate attention if your swelling happens suddenly for no clear reason, follows a physical injury like a fall or car accident, or appears in just one leg with pain or pale, cool skin.
How Pitting Edema Is Assessed
One way doctors evaluate foot swelling is the pitting test. They press a finger into the swollen area for five to 15 seconds and then release. If an indentation stays behind, you have pitting edema. The depth of the dent and how long it takes to fill back in tell them how severe it is, on a scale from grade 1 (a shallow 2-millimeter pit that bounces right back) to grade 4 (an 8-millimeter pit that takes two to three minutes to flatten out).
Not all swelling pits. Lymphedema, for instance, often produces non-pitting swelling where the skin feels firm rather than spongy. This distinction helps narrow down the cause.
Practical Ways to Reduce Swelling
For mild, lifestyle-related swelling, a few simple changes can make a noticeable difference. Elevating your feet above heart level for 15 to 20 minutes several times a day helps gravity work in your favor. Moving around regularly, even just flexing your ankles or taking short walks, activates the muscle pumps in your calves that push fluid back toward your heart. Cutting back on sodium is one of the most effective dietary changes you can make.
Compression stockings provide graduated pressure that supports your veins and reduces fluid accumulation. Low-pressure stockings (under 20 mmHg) are available over the counter and work well for mild swelling. Stockings rated at 20 mmHg or higher require a prescription and are typically used for chronic venous insufficiency or more significant edema. Getting the right fit matters, so it’s worth being measured rather than guessing your size.
When swelling is caused by an underlying condition like heart failure or kidney disease, managing that condition is the primary treatment. Reducing salt intake and wearing compression garments still help, but they work alongside the medical treatment rather than replacing it.
