Blood pressure can drop for dozens of reasons, ranging from something as simple as standing up too fast to something as serious as a severe infection. Clinically, low blood pressure is defined as a reading below 90/60 mmHg. Some people run naturally low without any symptoms, but when a drop causes dizziness, fainting, or confusion, it signals that your organs aren’t getting enough blood flow.
Dehydration
One of the most common and overlooked causes of a blood pressure drop is not drinking enough fluids. Your blood is mostly water, so when you lose fluid through sweating, vomiting, diarrhea, or simply not drinking enough, your total blood volume shrinks. As one Cleveland Clinic cardiologist put it, “you’re just not filling up the pipes enough for what your vascular system needs.” That reduced volume means less pressure pushing blood through your arteries, and your organs may not get the oxygen they need to function properly.
Hot weather, intense exercise, illness with fever, and alcohol consumption all accelerate fluid loss. If you’re feeling lightheaded and can’t remember the last time you had water, dehydration is a likely culprit.
Standing Up Too Quickly
When you go from lying down or sitting to standing, gravity pulls blood toward your legs. Normally your body compensates within seconds by tightening blood vessels and slightly increasing heart rate. When that reflex is sluggish or impaired, blood pressure drops sharply. This is called orthostatic hypotension, and it’s diagnosed when your systolic pressure (the top number) falls by 20 mmHg or more within two to five minutes of standing, or your diastolic pressure (the bottom number) drops by 10 mmHg or more.
Older adults, people who are dehydrated, and those on blood pressure medications are especially prone. Moving slowly from lying to sitting, pausing for a moment, and then standing can help your body adjust. Drinking water before long periods of standing also reduces the risk.
Medications
A long list of medications can lower blood pressure, sometimes more than intended. The most obvious are drugs prescribed specifically for high blood pressure: diuretics (water pills), beta blockers, and alpha blockers. If your dose is too high or you’ve recently started a new prescription, your pressure may dip below a comfortable range.
But blood pressure medications aren’t the only offenders. Tricyclic antidepressants, Parkinson’s disease drugs, and erectile dysfunction medications can all cause drops. Erectile dysfunction pills are particularly risky when combined with nitroglycerin, a common heart medication, because both widen blood vessels and the combined effect can send pressure dangerously low. If you notice lightheadedness, fatigue, or near-fainting after starting or adjusting any medication, the timing is probably not a coincidence.
Eating a Meal
After you eat, your body redirects extra blood to your digestive system. To compensate, your heart rate normally ticks up and blood vessels elsewhere in the body tighten. When that compensatory response is weak, blood pressure falls. This is called postprandial hypotension, and it’s surprisingly common in older adults. Research reviews estimate that about 40% of people between ages 65 and 86 experience it.
The risk is higher if you have diabetes, Parkinson’s disease, heart failure, or kidney disease. Large, carbohydrate-heavy meals tend to trigger bigger drops. Eating smaller, more frequent meals and drinking water before eating can blunt the effect.
Heart Problems
Your heart is the pump that generates blood pressure, so anything that weakens the pump can lower the pressure it produces. An abnormally slow heart rate (bradycardia) means fewer beats pushing blood out per minute. Heart valve problems can allow blood to leak backward instead of flowing forward efficiently. Heart failure, where the heart muscle is too weak to pump adequately, directly reduces the force behind each beat. A heart attack can cause a sudden, dramatic drop in pressure if enough muscle is damaged.
Hormonal and Nutritional Causes
Your endocrine system plays a quieter but important role in blood pressure regulation. The adrenal glands produce hormones that help control how tightly blood vessels constrict and how much salt and water your kidneys retain. When these glands underperform, as in Addison’s disease, blood pressure can fall chronically. Thyroid disorders, both overactive and underactive, can also disrupt normal pressure regulation.
Nutritional deficiencies contribute too. When you lack vitamin B12 or folate, your body produces red blood cells that are oversized and inefficient at carrying oxygen. The resulting anemia means your tissues are oxygen-starved even when blood flow seems adequate. Symptoms overlap heavily with low blood pressure: tiredness, dizziness, and shortness of breath. Iron deficiency anemia works through a similar pathway.
Severe Infections and Allergic Reactions
Some of the most dangerous blood pressure drops happen during sepsis or anaphylaxis. In both cases, the underlying problem is the same: blood vessels throughout the body suddenly relax and widen, and the capillaries (the smallest vessels) become leaky, letting fluid seep out of the bloodstream. The result is a catastrophic loss of the pressure needed to drive blood to the brain, heart, and kidneys.
In septic shock, a bacterial infection triggers a massive inflammatory response that causes this widespread vessel dilation. In anaphylactic shock, a severe allergic reaction (to foods, insect stings, or medications) does essentially the same thing within minutes. Both are medical emergencies. Symptoms that distinguish a dangerous pressure drop from a benign one include confusion, cold or clammy skin, rapid shallow breathing, a weak and fast pulse, and blurred vision. These signs mean organs are being deprived of blood flow and the situation is urgent.
Other Common Triggers
Several everyday situations cause temporary dips in blood pressure that are usually harmless but can catch you off guard:
- Prolonged bed rest. Days or weeks of lying flat decondition the reflexes that maintain blood pressure when upright, making lightheadedness almost guaranteed when you finally stand.
- Heat exposure. Hot baths, saunas, and high ambient temperatures widen blood vessels near the skin to release heat, which lowers overall pressure.
- Pregnancy. In the first and second trimesters, the circulatory system expands rapidly. Blood volume increases, but it takes time for the body to produce enough blood to fill the larger network, so pressure often runs low.
- Blood loss. Whether from an injury, surgery, or heavy menstrual bleeding, losing blood directly reduces the volume in your circulatory system.
A single episode of lightheadedness after standing up on a hot day is rarely concerning. But if you’re experiencing frequent dizziness, repeated near-fainting, or persistent fatigue, those patterns point to something worth investigating. Tracking when your symptoms happen, whether after meals, after standing, or after taking medication, gives you and your doctor useful information about what’s driving the drops.
