A stuffy nose usually isn’t caused by too much mucus. The real culprit is swollen blood vessels inside your nasal passages. When those vessels dilate, the surrounding tissue puffs up and blocks airflow. Knowing this helps explain why some remedies work and others don’t, and why the best approach depends on what’s causing the swelling in the first place.
Why Your Nose Feels Blocked
The lining of your nose is packed with tiny blood vessels. When you’re fighting a cold, dealing with allergies, or exposed to irritants like dry air or strong odors, those vessels expand. The increased blood flow causes the tissue to swell, narrowing your nasal passages. Mucus production often ramps up at the same time, but the swelling itself is what makes breathing feel so difficult.
This is why blowing your nose over and over doesn’t fix the problem. You can clear mucus temporarily, but the underlying swelling remains. Effective remedies target that swelling directly, either by constricting the blood vessels or reducing the inflammation driving them to expand.
Saline Rinses and Nasal Irrigation
Flushing your nasal passages with salt water is one of the safest and most effective ways to relieve congestion. A neti pot, squeeze bottle, or bulb syringe physically washes out mucus, allergens, and irritants while moisturizing swollen tissue. Unlike medications, you can use saline rinses as often as you need them without side effects.
One critical rule: never use plain tap water. Tap water can contain microscopic organisms that are harmless when swallowed (stomach acid kills them) but dangerous when introduced into nasal passages, where they can cause serious and occasionally fatal infections. Use distilled or sterile water from the store, water that’s been boiled for 3 to 5 minutes and cooled to lukewarm, or water filtered through a device specifically designed to trap infectious organisms. Boiled water should be used within 24 hours. After each use, wash the device and let it air dry or dry the inside with a paper towel.
Decongestant Sprays: Effective but Risky
Medicated nasal sprays containing ingredients like oxymetazoline work by shrinking the blood vessels in your nose. Less blood flow means less swelling, and air moves through more freely. The relief is fast and noticeable.
The catch is that you can’t use them for more than three days. After that, they can actually make congestion worse through a process called rebound congestion. When the spray repeatedly cuts off blood flow to nasal tissue, the tissue becomes deprived of nutrients it needs. This triggers new inflammation, bringing back the exact stuffiness you were trying to fix, often worse than before. People who keep spraying to counteract the rebound can end up stuck in a cycle that’s difficult to break. Save these sprays for short stretches when congestion is at its worst, like the first few nights of a bad cold.
Oral Decongestants: Check the Label
If you’re reaching for a pill instead of a spray, what’s inside matters more than you might think. Many popular cold and allergy products on store shelves contain oral phenylephrine as their decongestant. In 2023, an FDA advisory panel unanimously concluded that oral phenylephrine is no more effective than a placebo for relieving nasal congestion. The FDA has since proposed removing it as an approved over-the-counter decongestant ingredient, though products containing it remain on shelves for now.
Pseudoephedrine, on the other hand, does work. It’s kept behind the pharmacy counter (not because it’s a prescription drug, but because it can be used to manufacture methamphetamine), so you’ll need to ask a pharmacist and show ID. If you’ve been buying a decongestant off the shelf and wondering why it isn’t helping, this is likely the reason.
Nasal Steroid Sprays for Ongoing Congestion
For congestion driven by allergies or chronic inflammation, over-the-counter nasal steroid sprays are a strong option. These work differently from decongestant sprays. Rather than constricting blood vessels, they reduce the inflammatory response that causes swelling in the first place. They don’t carry a risk of rebound congestion and are safe for daily use over weeks or months.
The tradeoff is patience. In clinical studies, one common nasal steroid showed meaningful improvement within about 36 hours of the first dose, while another took closer to 60 hours. Neither showed a significant difference from placebo within the first 12 hours. So if you start using one and feel nothing after a single day, give it at least two to three days before judging whether it’s working. These sprays are best suited for allergy-related stuffiness or congestion that lingers for weeks rather than the acute stuffiness of a two-day cold.
Humidity and Steam
Dry air irritates nasal tissue and can make swelling worse. Keeping your indoor humidity between 30% and 50% helps keep nasal passages moist and comfortable. A humidifier in your bedroom can make a noticeable difference overnight, particularly in winter when heating systems dry the air out.
For children with stuffy noses, cool-mist humidifiers appear more helpful than warm-mist models. Heated humidified air hasn’t shown the same benefit in studies. A hot shower can also provide temporary relief, since the warm, moist air loosens mucus and soothes irritated tissue. The effect fades after you leave the steam, but it can be a useful reset before bed.
Does Menthol Actually Work?
Menthol, the cooling compound in products like vapor rubs, cough drops, and mentholated balms, creates a strong sensation of open airways. But research shows it doesn’t actually reduce nasal resistance or change airflow at all. What it does is activate sensory nerve endings in the nose that make you perceive more airflow. Your passages are just as swollen, but your brain registers a feeling of easier breathing.
That’s not worthless. If congestion is keeping you awake, the subjective sensation of clearer breathing can be enough to help you fall asleep. Just don’t rely on menthol as your only strategy if you need real decongestion.
Sleeping With a Stuffy Nose
Congestion almost always feels worse at night. When you lie flat, gravity can no longer help drain fluid away from your nasal passages, and blood pools in the vessels of your nasal lining, increasing swelling. Propping your head up with an extra pillow helps counteract this by encouraging mucus to drain downward and reducing the pressure that builds when you’re flat. Sleeping on your back with your head elevated tends to work better than lying on your side, where one nostril often clogs completely.
Combining an elevated head position with a humidifier running nearby and a saline rinse before bed can make a real difference in how well you sleep through a cold or allergy flare.
When Congestion Needs Attention
Most stuffy noses resolve on their own within a week or so. But certain signs suggest something more is going on. Congestion lasting more than 10 days, a high fever, facial pain, or nasal discharge that turns yellow or green alongside fever or sinus pain may point to a bacterial infection that needs treatment. Bloody discharge or a nose that keeps running after a head injury also warrants a visit to a healthcare provider. For infants, congestion that interferes with nursing or breathing should be evaluated promptly.
