Persistently high blood sugar damages nearly every system in your body, from your smallest blood vessels to your brain, teeth, and reproductive organs. The harm builds gradually, often over years, which is why many people don’t connect their symptoms to blood sugar until significant damage has already occurred. Here’s what elevated glucose actually does inside your body and what that looks like in everyday life.
Nerve Damage and Loss of Sensation
One of the most common consequences of prolonged high blood sugar is nerve damage, known as diabetic neuropathy. Excess glucose triggers a cascade of harmful chemical reactions inside nerve cells, including the buildup of sugar alcohols through what’s called the polyol pathway and the formation of advanced glycation end-products, which are essentially sugar-damaged proteins that accumulate and interfere with normal cell function. Oxidative stress compounds the problem, gradually degrading the nerve fibers themselves.
The result usually starts in your feet and hands: tingling, numbness, burning pain, or a strange sensation that feels like wearing gloves or socks when you’re not. Over time, you may lose the ability to feel temperature changes or small injuries altogether. This is why foot ulcers are so common in people with uncontrolled blood sugar. You step on something sharp or develop a blister and simply don’t feel it. That small wound can then spiral into a serious infection.
Nerve damage also affects internal organs. It can slow digestion (a condition called gastroparesis), cause bladder problems, and disrupt the signals that regulate your heart rate and blood pressure.
Blood Vessel Damage Throughout the Body
High glucose is corrosive to the lining of your blood vessels, both the tiny capillaries and larger arteries. In small vessels, the damage shows up in three classic ways: in the eyes, kidneys, and nerves.
In your eyes, damaged capillaries in the retina can leak fluid or bleed, distorting your vision. Left unchecked, this progresses to diabetic retinopathy, a leading cause of blindness in adults. In the kidneys, similar small-vessel injury gradually destroys the filtering units that clean your blood. Early on, this causes protein to spill into your urine. Over years, it can lead to kidney failure requiring dialysis.
In larger vessels, high blood sugar accelerates atherosclerosis, the buildup of fatty plaques in artery walls. This raises the risk of heart attack and stroke substantially. Cardiovascular disease is the leading cause of death among people with diabetes, and elevated blood sugar is a major driver even in people who haven’t been formally diagnosed.
Slow Wound Healing and Weakened Immunity
If you’ve noticed that cuts, scrapes, or surgical wounds take much longer to heal than they used to, high blood sugar may be the reason. Healing depends on your immune system sending the right cells to an injury site in the right order. Research from the National Institutes of Health has shown that in people with diabetic foot ulcers, the recruitment of key immune cells called neutrophils and macrophages to the wound is significantly suppressed. Genes responsible for promoting tissue repair, particularly those involved in triggering healthy inflammation, are far less active in diabetic wounds compared to normal healing skin.
High glucose also feeds bacteria and fungi directly, creating a more hospitable environment for infection. This means not only do wounds heal slowly, but they’re more likely to become infected in the first place. Urinary tract infections, skin infections, and yeast infections are all more frequent when blood sugar runs high.
Brain Health and Cognitive Decline
Your brain is one of the most blood-hungry organs in your body, and it suffers when high blood sugar damages the vessels feeding it. Over time, reduced blood flow to the brain causes brain cells to die, leading to problems with memory, concentration, and thinking speed. According to the CDC, this process can ultimately result in vascular dementia. There is also a well-documented link between chronic high blood sugar and an increased risk of Alzheimer’s disease, though the exact mechanisms are still being studied.
The cognitive effects aren’t limited to older adults. Even in middle age, poorly controlled blood sugar is associated with measurable declines in mental sharpness. Some people notice brain fog, difficulty finding words, or trouble with tasks that used to feel automatic long before any formal diagnosis of dementia.
Gum Disease and Dental Problems
High blood sugar doesn’t just circulate through your bloodstream. It also increases glucose levels in your saliva. That extra sugar feeds the harmful bacteria in your mouth, accelerating the formation of plaque and raising your risk of cavities. But tooth decay is only part of the picture.
Gum disease is the most common and serious oral complication. It starts as gingivitis, mild inflammation that makes your gums red, swollen, or prone to bleeding when you brush. If blood sugar stays elevated, gingivitis often progresses to periodontitis, a deeper infection that destroys the bone holding your teeth in place and can eventually lead to tooth loss. People with high blood sugar also face higher rates of dry mouth, oral thrush (a painful fungal infection that causes white patches), burning mouth syndrome, and changes in the way food tastes.
Sexual and Reproductive Health
The combination of nerve damage and blood vessel injury takes a direct toll on sexual function in both men and women. In men, the connection is widely recognized: damaged blood vessels and nerves reduce blood flow to the penis and impair the signaling needed for erections. Erectile dysfunction is one of the earlier complications many men experience.
In women, the effects are equally real but less commonly discussed. Sexual arousal depends on increased blood flow to the clitoris and vaginal tissues, a process regulated by the same chemical messenger (nitric oxide) that’s disrupted by chronic high glucose. When blood vessel linings are damaged, that blood flow response is significantly impaired. Hyperglycemia also decreases hydration of vaginal membranes, reducing lubrication and often causing pain during intercourse. Nerve damage can further alter how sexual stimuli are perceived, contributing to reduced sensation or arousal difficulties. Some researchers have even proposed that unexplained vulvar pain could be an early sign of sensory nerve damage from elevated blood sugar.
Diabetic Ketoacidosis and Acute Emergencies
Not all consequences of high blood sugar develop over years. When blood sugar climbs extremely high, usually above 250 mg/dL, your body may begin breaking down fat for fuel at an accelerated rate, producing acidic byproducts called ketones. If ketones build up faster than your body can clear them, the result is diabetic ketoacidosis (DKA), a life-threatening emergency. Symptoms include nausea, vomiting, abdominal pain, fruity-smelling breath, confusion, and rapid breathing. DKA is most common in type 1 diabetes but can occur in type 2 as well, particularly during illness or infection.
Another acute danger is hyperosmolar hyperglycemic state, where blood sugar soars above 600 mg/dL. The blood becomes so concentrated that it pulls water from organs, causing severe dehydration, confusion, and potentially coma. This occurs more often in older adults with type 2 diabetes and carries a high mortality rate without prompt treatment.
The Compounding Effect
What makes high blood sugar particularly destructive is that these complications don’t exist in isolation. They feed each other. Nerve damage in the feet leads to unnoticed injuries. Poor wound healing turns those injuries into chronic ulcers. Blood vessel damage limits the oxygen and immune cells reaching the wound. Infection sets in. In severe cases, this chain of events ends in amputation, not because of a single dramatic failure but because of years of quiet, overlapping damage.
The same compounding effect plays out in the kidneys, where high blood sugar forces the organs to work harder filtering excess glucose, which damages their filtering units, which raises blood pressure, which damages them further. Each system weakened by high blood sugar makes the others more vulnerable, which is why early and consistent blood sugar management has such an outsized effect on long-term health outcomes.
