Your liver handles over 500 functions, from filtering toxins to processing nutrients, and it has a remarkable ability to repair itself when you give it the right conditions. The most impactful things you can do are maintain a healthy weight, limit alcohol, stay active, and be strategic about what you eat and drink. Here’s how each of those works in practice.
Lose Even a Small Amount of Weight
If you’re carrying extra weight, your liver is likely storing excess fat. Non-alcoholic fatty liver disease affects roughly one in four adults worldwide, and weight loss is the single most effective treatment. Losing just 3 to 5 percent of your body weight is enough for fat to start disappearing from liver cells. For someone who weighs 200 pounds, that’s only 6 to 10 pounds.
Reducing inflammation and scarring in the liver requires more effort: about 10 percent of body weight lost. That’s a meaningful goal, but you don’t need to reach it overnight. Gradual, sustained weight loss is safer for the liver than crash dieting, which can actually worsen liver inflammation by flooding the organ with fatty acids all at once.
Drink Coffee
Coffee is one of the most consistently supported liver-protective habits in nutrition research. Drinking two cups a day cuts the odds of cirrhosis by 44 percent, and four cups a day lowers them by 65 percent. Regular coffee consumption also slows the progression of fibrosis, hepatitis B and C, and non-alcohol-related fatty liver disease.
These benefits appear to come from the coffee itself, not just the caffeine. Both caffeinated and decaffeinated coffee show protective effects, though caffeinated tends to perform better in studies. Black coffee or coffee with minimal added sugar gives you the benefit without extra calories working against your liver.
Exercise, Even Without Losing Weight
Physical activity reduces liver fat independently of weight loss. In one study, participants who did resistance training three times a week for eight weeks saw a 13 percent reduction in liver fat, even without dropping pounds on the scale. Each session lasted 45 to 60 minutes and included a warm-up on a stationary bike followed by circuit-style strength exercises.
You don’t need to follow that exact routine. Both aerobic exercise (walking, cycling, swimming) and resistance training (weights, bodyweight exercises, resistance bands) reduce liver fat. The key is consistency: aim for at least 150 minutes of moderate activity per week, spread across several days. If you’re starting from zero, even brisk daily walks make a measurable difference.
Limit Alcohol or Cut It Out
Alcohol is processed almost entirely by the liver, and heavy drinking is one of the fastest routes to liver damage. The CDC defines moderate drinking as two drinks or fewer per day for men and one drink or fewer per day for women. Staying within these limits significantly reduces your risk of alcoholic liver disease.
If you already have any form of liver disease, the recommendation is to avoid alcohol entirely. Even moderate drinking can accelerate damage in a liver that’s already inflamed or scarred. If you’re unsure about your liver’s current condition, a simple blood test can check your enzyme levels before you decide what role alcohol plays in your life.
Watch Your Medications
Acetaminophen (the active ingredient in Tylenol) is the most common medication-related cause of liver injury. It’s safe at normal doses, but the margin between a therapeutic dose and a harmful one is narrower than most people realize. The maximum safe dose for adults is 4,000 milligrams per day across all sources combined.
That “all sources combined” part matters because over 600 medications contain acetaminophen, including many cold medicines, sleep aids, and prescription painkillers that combine it with other ingredients. It’s easy to double up without knowing it. Always check the active ingredients on every medication you take, and be especially careful about combining acetaminophen with alcohol, which dramatically increases its liver toxicity.
Reduce Exposure to Environmental Toxins
A class of synthetic chemicals called PFAS, sometimes called “forever chemicals,” accumulates in human tissues including the liver. NIH-funded research found that three of the most common PFAS chemicals were associated with elevated liver enzymes and linked to the early stages of fatty liver disease in both human and animal studies.
PFAS show up in places you might not expect: grease-resistant paper, fast food wrappers, microwave popcorn bags, pizza boxes, candy wrappers, some nonstick cookware, and certain cleaning products. You can’t eliminate all exposure, but you can reduce it. Use stainless steel or cast iron cookware instead of nonstick, avoid microwaving food in its packaging, and choose uncoated paper products when possible. When working with chemical cleaning products, aerosol paints, or pesticides, ventilate the area well and wear gloves to limit what your liver has to process.
Eat to Support Your Liver
No single food fixes liver damage, but your overall dietary pattern matters enormously. A Mediterranean-style diet, rich in vegetables, fruits, whole grains, fish, nuts, and olive oil, consistently shows up in research as protective for liver health. This pattern works because it reduces insulin resistance and inflammation, both of which drive fatty liver disease.
A few specific adjustments help:
- Cut back on added sugars, especially fructose from sweetened beverages. The liver processes fructose directly, and excess intake promotes fat buildup in liver cells.
- Choose whole grains over refined carbohydrates. White bread, pastries, and sugary cereals spike blood sugar and contribute to insulin resistance.
- Eat fiber-rich foods. Vegetables, legumes, and whole grains help regulate how your body absorbs and processes fats and sugars.
- Limit saturated fat. High intake of saturated fat from red meat, butter, and fried foods contributes to fat accumulation in the liver.
What About Milk Thistle?
Milk thistle is the most popular liver supplement, and the research on it is genuinely mixed. One large observational study of over 2,600 patients with chronic liver disease found that eight weeks of supplementation reduced several liver enzyme markers. But controlled clinical trials, the gold standard, have produced conflicting results. A major trial involving 154 patients with chronic hepatitis C found that milk thistle, even at higher-than-usual doses, failed to significantly reduce liver enzyme levels compared to placebo.
Side effects are minimal. The most commonly reported issues are mild digestive upset and a slight laxative effect. Allergic reactions are rare and mainly seen at very high doses above 1,500 milligrams per day. Milk thistle is unlikely to harm you, but the evidence doesn’t strongly support it as a reliable treatment for liver disease. It shouldn’t replace the lifestyle changes that have far more robust data behind them.
Get Vaccinated Against Hepatitis
Hepatitis B is a viral infection that directly attacks liver cells and can lead to chronic liver disease, cirrhosis, and liver cancer. A vaccine exists and is highly effective. The CDC recommends hepatitis B vaccination for all adults aged 19 to 59, and for adults 60 and older who have risk factors. If you were never vaccinated as a child or aren’t sure, it’s worth checking with your doctor.
Hepatitis A, which spreads through contaminated food and water, also targets the liver. While it doesn’t usually cause chronic disease, a severe infection can take months to recover from. Vaccination is available and recommended for travelers, people with existing liver conditions, and several other groups.
Know Your Baseline Numbers
A standard blood panel can measure your liver enzyme levels and flag problems before you feel any symptoms. The two most commonly checked enzymes are ALT (normal range: 7 to 55 units per liter) and AST (normal range: 8 to 48 units per liter). Elevated levels don’t diagnose a specific condition, but they signal that liver cells are being damaged and prompt further investigation.
Liver disease is often called a silent condition because it can progress for years without obvious symptoms. By the time you notice fatigue, yellowing skin, or abdominal swelling, the damage may be advanced. A routine blood test is the simplest way to catch problems early, when lifestyle changes are most effective at reversing them.
