Vitamin D: Daily Low Doses Work Best

Vitamin D plays a central role in activating your immune cells, and being deficient makes you noticeably more vulnerable to respiratory infections. The key finding from pooled clinical trials is that small daily doses (300 to 2,000 IU per day) cut infection risk roughly in half compared to placebo, while large monthly mega-doses did almost nothing. Once your blood levels are in a healthy range, taking more doesn’t add extra protection.

In five trials involving marathon runners, skiers, and soldiers training in subarctic conditions, vitamin C cut cold risk by 52%. If you train hard, race, or regularly push your body to exhaustion, a daily supplement (typically 200 mg to 1,000 mg) is worth considering. If your lifestyle is more sedentary, eating fruits and vegetables will cover you just fine.

Zinc: Effective but Easy to Overdo

Zinc lozenges are one of the better-studied cold interventions. Cochrane reviewed 34 trials and found that zinc gluconate lozenges, the most commonly tested form, can reduce cold duration and may help with prevention when taken regularly. Doses in prevention and treatment studies ranged from 45 to 276 mg per day, but the safe upper limit for adults is 40 mg per day for ongoing use. Taking 50 mg or more for weeks at a time can backfire: it blocks copper absorption, weakens the very immune function you’re trying to support, and lowers healthy cholesterol levels.

For everyday prevention, a modest zinc supplement (15 to 30 mg daily) or zinc-rich foods like meat, shellfish, legumes, and pumpkin seeds is a safer long-term strategy than high-dose lozenges, which are better reserved for the first sign of a cold.

Probiotics: Gut Bacteria That Protect Your Lungs

Your gut immune system and your respiratory tract talk to each other constantly. Several probiotic strains have been shown to reduce the frequency and duration of upper respiratory infections in clinical trials. The strains with the most evidence behind them include Lactobacillus casei, Lactobacillus gasseri, Lactobacillus reuteri, and Bifidobacterium animalis subsp. lactis. Probiotics aren’t a quick fix. They work best when taken consistently over weeks, giving the beneficial bacteria time to establish themselves in your gut. Look for products that list specific strain names (not just the species) and contain at least 1 billion colony-forming units.

Garlic: Surprisingly Strong Evidence

Garlic often gets dismissed as folk medicine, but one well-designed trial of 146 people found striking results. Participants who took a daily garlic capsule containing allicin (the compound released when you crush fresh garlic) for 12 weeks had 24 colds compared to 65 in the placebo group. The catch: this is based on a single trial, and Cochrane notes the evidence base is still thin. Still, garlic is inexpensive, safe for most people, and easy to add to your diet. Allicin is most potent in raw or lightly cooked garlic, or in supplements specifically standardized for allicin content.

Elderberry and Echinacea: Better for Treatment Than Prevention

Elderberry extract has gained popularity as an immune booster, but the clinical evidence points more toward treatment than prevention. A review of five randomized trials found that elderberry likely does not reduce the risk of catching a cold. Where it does seem to help is after you’re already sick: it can shorten the duration and severity of cold and flu symptoms. Echinacea has a more interesting prevention story. One controlled trial during the COVID-19 pandemic found that people taking an echinacea preparation had a 25% lower risk of any respiratory virus infection and a 63% lower risk of SARS-CoV-2 specifically. However, the study was open-label (participants knew what they were taking), which weakens the findings. Echinacea also interacts with certain liver enzymes involved in processing antipsychotic and antidepressant medications, so if you take those, talk to your pharmacist before adding it.
