What Can You Catch From Kissing? Diseases & Risks

Kissing can transmit a surprisingly wide range of infections, from common colds to herpes, mono, and even some sexually transmitted infections. Most of these spread through saliva, though a few require direct contact with a sore or broken skin. Here’s what you can actually catch and how concerned you should need to be.

Oral Herpes (HSV-1)

Oral herpes is the most common infection spread by kissing. An estimated 3.8 billion people under age 50, about 64% of the global population, carry herpes simplex virus type 1 (HSV-1). It spreads through contact with saliva, and the tricky part is that many carriers never develop visible cold sores. People with herpes can spread the virus even when they have no symptoms at all, a process called asymptomatic shedding. This is why oral herpes is so widespread: most people who transmit it don’t know they’re doing so.

When symptoms do appear, they typically show up as cold sores or blisters around the lips and mouth. These outbreaks come and go, and the virus stays in the body permanently. The risk of transmission is highest during an active outbreak, but it never drops to zero.

Mono (The “Kissing Disease”)

Infectious mononucleosis, commonly called mono, is caused by the Epstein-Barr virus (EBV). It spreads primarily through saliva, which is exactly why it earned its nickname. Symptoms typically appear four to six weeks after infection, so by the time someone feels sick, they may have been unknowingly passing the virus along for over a month.

Mono causes intense fatigue, sore throat, fever, and swollen lymph nodes. Most people recover within two to four weeks, though the fatigue can linger for months. Like HSV-1, EBV is extremely common. Most adults have been exposed at some point, though not everyone develops noticeable symptoms.

Strep Throat and Respiratory Infections

The bacteria behind strep throat, group A streptococcus, normally travels through respiratory droplets from coughing and sneezing. But sharing saliva through kissing, or even sharing a glass or utensils, is another route. The bacteria can also spread simply through close conversation with an infected person. If kissing can transmit it, so can any prolonged face-to-face contact.

Cold and flu viruses follow a similar pattern. Any virus that lives in the throat or nasal passages can hitch a ride through a kiss. This is one reason couples tend to get sick around the same time. The risk is straightforward: if someone is fighting a respiratory illness, kissing them puts you in direct contact with the same germs.

Meningococcal Disease

The bacteria that cause meningococcal disease (which can lead to meningitis) spread through respiratory and throat secretions. The CDC specifically lists kissing as an example of the kind of close contact that transmits this bacteria. It generally takes prolonged or repeated exposure, so a quick peck is far less risky than an extended makeout session with someone carrying the bacteria.

Meningococcal disease is rare but serious. Most carriers never get sick themselves, which means they can pass the bacteria without knowing it. Vaccination, which is routine for adolescents in many countries, is the most effective protection.

Syphilis

This one surprises most people. Syphilis spreads through direct contact with a syphilis sore, and those sores can develop on or around the lips and inside the mouth. Kissing someone with an oral syphilis sore can transmit the infection. These sores are sometimes hard to spot, especially when they’re inside the mouth, and they may not be painful.

Syphilis transmission through kissing requires contact with an active sore, so it’s not something that spreads through saliva alone. But because the sores can be subtle or hidden, it’s possible to encounter one without realizing it.

HPV and Oral Cancer Risk

Human papillomavirus can be transmitted through deep kissing. Research from Moffitt Cancer Center found that people with 10 or more deep-kissing partners were more than twice as likely to develop an HPV-related cancer of the throat. About 70% of oropharyngeal cancers (cancers in the back of the throat, base of the tongue, and tonsils) are caused by HPV.

The risk from any single kiss is very low, and most HPV infections clear on their own without ever causing problems. But over a lifetime, oral HPV exposure is worth knowing about, particularly because HPV vaccination protects against the strains most likely to cause cancer.

What About HIV?

HIV is not transmitted through saliva. Closed-mouth kissing carries zero risk. Deep, open-mouth kissing has an extremely small theoretical risk only if both people have open sores or bleeding gums, allowing blood-to-blood contact. The CDC describes this scenario as very rare. In practical terms, you cannot get HIV from kissing.

Cavity-Causing Bacteria

This one matters most for parents and caregivers. The bacteria responsible for tooth decay aren’t present in a baby’s mouth at birth. They colonize the mouth later, often transferred from an adult’s saliva. Kissing a baby on the lips, sharing spoons to taste their food, or cleaning a pacifier with your mouth can introduce decay-causing bacteria to a child who didn’t have them before.

For adults kissing other adults, this is less of a concern since your mouth already hosts a full ecosystem of bacteria. But it’s a practical reason to maintain good oral hygiene: brushing twice a day and flossing reduces the bacterial load in your saliva.

Lowering Your Risk

You can’t eliminate every risk that comes with kissing, but a few things meaningfully reduce your exposure. Avoid kissing anyone with visible cold sores, mouth sores, or signs of illness. Open wounds, cracked lips, and bleeding gums all create entry points for bacteria and viruses, so good oral health acts as a natural barrier. Staying current on vaccinations for HPV and meningococcal disease removes two of the more serious risks from the equation entirely.

Keep in mind that many of the infections on this list are carried by people who look and feel perfectly healthy. Herpes, EBV, HPV, and meningococcal bacteria all spread from asymptomatic carriers. That’s not a reason to avoid kissing altogether, but it helps explain why these infections are so common.
