A Doctor of Pharmacy (PharmD) degree opens far more career paths than most people realize. While the majority of graduates work in community or hospital pharmacies, the degree also qualifies you for roles in the pharmaceutical industry, managed care organizations, government agencies, academia, and technology. The median annual salary for pharmacists was around $131,000 to $153,000 in 2024, depending on the setting, with ambulatory healthcare services paying the highest at $152,980.
Community Pharmacy
This is the most visible career path and where most PharmD graduates start. Community pharmacists dispense medications, but the role has expanded well beyond counting pills. In many states, pharmacists now assess patients, provide point-of-care testing, manage chronic conditions like diabetes and high blood pressure, and even prescribe certain medications independently, including statins and contraceptives. Medication reviews, vaccine administration, and pharmaceutical counseling are daily responsibilities.
Some community pharmacists specialize further, running anticoagulation clinics or monitoring patients with atrial fibrillation. These pharmacist-led services have demonstrated strong clinical outcomes, including reduced stroke risk and lower costs. The median annual wage for pharmacists in retail and drug store settings was $131,640 in 2024, the lowest among major pharmacy work settings but still a six-figure salary.
Hospital and Clinical Pharmacy
Clinical pharmacists work alongside physicians, nurses, and other providers inside hospitals and health systems. They round with medical teams, recommend drug therapies, adjust dosing for critically ill patients, and monitor for drug interactions and adverse effects. Unlike community pharmacy, these roles are deeply embedded in direct patient care for complex conditions.
Hospital pharmacists earned a median of $149,240 in 2024. Many clinical positions, especially specialist roles, expect you to complete at least one year of post-graduate residency training, and competitive subspecialty positions typically require two years. Clinical pharmacists often focus on a specific patient population or disease area: oncology, infectious diseases, critical care, pediatrics, or psychiatry, among others.
Board Certification and Specialization
The Board of Pharmacy Specialties (BPS) currently recognizes sixteen specialty certifications, with more than 63,400 pharmacist certifications worldwide. The full list includes ambulatory care, cardiology, compounded sterile preparations, critical care, emergency medicine, geriatrics, infectious diseases, nuclear pharmacy, nutrition support, oncology, pain management, pediatrics, pharmacotherapy, pharmacy informatics, psychiatric pharmacy, and solid organ transplantation.
Board certification isn’t legally required for most positions, but it signals advanced competence and is increasingly expected for clinical specialist and leadership roles. Each certification requires passing a rigorous exam, and most require documented practice experience in that specialty area.
Pharmaceutical Industry
PharmD graduates are well suited for several functional areas within drug companies, and these roles often pay more than traditional pharmacy positions. The main tracks include:
- Clinical development: You design and manage clinical trials, develop study protocols, select investigators and trial sites, oversee data collection, report adverse events, and publish results. Senior roles involve chairing international trial team meetings and managing external contractors.
- Regulatory affairs: You develop regulatory strategy, compile submissions to the FDA and global health authorities (including new drug applications), and review advertising and promotional materials for compliance.
- Medical affairs: This covers medical communications, education, and the Medical Science Liaison (MSL) role. MSLs are field-based specialists who serve as the scientific bridge between a pharmaceutical company and the medical community. They communicate clinical information to physicians, sales teams, and clinical trial investigators. Medical communications pharmacists evaluate evidence-based literature, plan continuing education programs, and collaborate with key opinion leaders on publications.
Many PharmD graduates enter industry through one- or two-year fellowship programs, such as those offered through Rutgers, which provide structured training across these functional areas.
Managed Care and Insurance
Health insurance companies and pharmacy benefit managers employ pharmacists to shape which medications millions of people have access to. This is a behind-the-scenes role with enormous influence on drug spending and patient outcomes.
Managed care pharmacists build and maintain drug formularies, which are the lists of medications covered by a given health plan. The goal is balancing access to effective therapy with sustainable resource use. They also run drug utilization review programs, structured analyses of prescribing patterns that flag drug interactions, duplicate therapies, and contraindications. Prior authorization review is another core function: evaluating whether specific prescriptions meet clinical criteria before the plan covers them. If you enjoy data analysis, policy work, and population-level thinking more than face-to-face patient care, managed care is worth exploring.
Government and Public Health
Federal agencies hire pharmacists in roles that look nothing like traditional pharmacy practice. At the FDA, pharmacists evaluate drug proposals submitted by manufacturers, conduct post-market surveillance of approved drugs for safety and efficacy, advise on drug therapy, and research new compounding techniques for investigational drugs. The CDC, Department of Veterans Affairs, Indian Health Service, and military branches all employ pharmacists in clinical, administrative, and policy roles.
Public health pharmacists may also work at the state or local level, coordinating immunization campaigns, managing drug supply chains during emergencies, or developing opioid harm-reduction programs.
Pharmacy Informatics
As healthcare becomes more technology-driven, informatics pharmacists fill a growing niche. They build and maintain clinical content within electronic health record systems, develop clinical decision support tools (the alerts that warn providers about dangerous drug interactions), manage automated dispensing systems, and create medication safety policies tied to health information technology. They also develop reports specific to medication management and maintain billing integrity within EHR systems.
This role combines clinical pharmacy knowledge with IT skills. Pharmacy informatics is one of the sixteen BPS-recognized specialties, and demand has grown as hospitals invest in medication safety technology.
Pharmacogenomics
One of the newer applications of the PharmD degree is using patients’ genetic data to personalize drug therapy. Pharmacogenomics pharmacists review genetic test results, interpret how a patient’s DNA affects their response to specific medications, and recommend dosing adjustments or alternative drugs to the prescribing physician.
Several major health systems have built pharmacist-led pharmacogenomics programs. Mayo Clinic trained over 300 pharmacists to assess and provide recommendations for pharmacogenomic consultations. At St. Jude Children’s Research Hospital, pharmacists operate under collaborative practice agreements with the authority to order genetic tests and provide written consults with therapy recommendations. The University of Colorado Hospital created a model where patients are referred to a pharmacist for a dedicated visit and genetic testing, after which the pharmacist develops a therapeutic plan in coordination with the physician. These programs are expanding, particularly in oncology, where genetic variations can dramatically affect how patients metabolize chemotherapy drugs.
Academia and Research
PharmD graduates who enjoy teaching can pursue faculty positions at colleges of pharmacy. Most tenure-track academic roles require completing a residency or fellowship, and many faculty members also hold a PhD or conduct funded research. Academic pharmacists teach courses in pharmacology, therapeutics, and clinical skills while often maintaining a clinical practice site. Non-tenure positions focused purely on teaching are also available and may require less research output.
Post-Graduate Training Pathways
Many of the careers above, especially clinical and industry roles, benefit from or require additional training after your PharmD. The two main options are residencies and fellowships.
A PGY1 (first-year) pharmacy residency builds broad clinical competence across four areas: patient care, advancing practice, leadership and management, and teaching. Completing a PGY1 qualifies you for staff clinical pharmacist positions and is a prerequisite for further specialization. A PGY2 (second-year) residency narrows your focus to a specific area like oncology, critical care, or pediatrics. Institutions filling clinical specialist positions generally prefer candidates who have completed a PGY2. If you finish only a PGY1, your broader training gives you more flexibility across different job types and patient populations.
Industry fellowships, typically one to two years, train you specifically for pharmaceutical company roles in clinical development, regulatory affairs, medical affairs, or outcomes research. These are competitive but provide a direct pipeline into industry careers that are difficult to access otherwise.

Industry, managed care, and informatics roles often fall outside these BLS categories and can exceed $160,000, particularly for MSLs, regulatory specialists, and those with several years of experience. Geographic location also matters significantly, with pharmacists in urban and high-cost-of-living areas typically earning more.
