What works best depends on the type of arthritis you have, which joints are affected, and how severe your symptoms are. Here’s a practical breakdown of what’s available.

Over-the-Counter Anti-Inflammatories

NSAIDs (nonsteroidal anti-inflammatory drugs) are the most effective oral medications for osteoarthritis pain. The common ones you’ll find at any pharmacy include ibuprofen (Advil, Motrin), naproxen (Aleve), and diclofenac. They all work by blocking enzymes that cause pain and swelling in your joints.

Naproxen lasts longer per dose than ibuprofen, so many people with chronic joint pain prefer it for all-day coverage. Ibuprofen acts faster, which can make it a better choice for flare-ups. The catch is that it needs to be applied several times a day to be effective, and it tends to work better alongside other treatments, like an oral NSAID, rather than on its own. Expect a burning sensation at first that fades with regular use over a week or two.

Other topical products, like menthol-based counterirritants and salicylate creams, are widely available but perform only slightly better than placebos in clinical studies.

Corticosteroid Injections and Oral Steroids

For flare-ups that don’t respond well to over-the-counter options, corticosteroid injections directly into the affected joint are a strongly recommended treatment for knee and hip osteoarthritis. They reduce inflammation quickly and can provide weeks to months of relief. Your doctor may use imaging guidance for hip injections to ensure accuracy.

Oral corticosteroids like prednisone are sometimes used short-term to control severe flares, particularly in inflammatory types of arthritis like rheumatoid arthritis. Typical doses for rheumatoid arthritis range from 5 to 10 mg daily. These aren’t meant for long-term use because the side effects accumulate: bone thinning, weight gain, elevated blood sugar, high blood pressure, cataracts, and sleep disruption. If you’ve taken prednisone regularly for more than two weeks, stopping suddenly can cause a dangerous drop in your body’s natural cortisol production, so you’ll need to taper off gradually.

Prescription Drugs That Slow Joint Damage

If you have rheumatoid arthritis or another autoimmune form, pain relief alone isn’t enough. Disease-modifying drugs (DMARDs) calm down an overactive immune system to prevent it from attacking your joints. Starting them early is critical because they can reduce or prevent bone and joint damage and preserve how well your joints function over time.
