Here’s a practical breakdown of what works, what the evidence actually supports, and what to expect from each option.
Over-the-Counter Pain Relievers
The two main categories are acetaminophen (Tylenol) and NSAIDs like ibuprofen (Advil, Motrin) and naproxen (Aleve). For joint pain specifically, NSAIDs tend to work better because they reduce inflammation in addition to blocking pain signals. Acetaminophen relieves pain but does nothing for swelling, which makes it a weaker choice when inflammation is driving your symptoms.

Diclofenac gel (sold over the counter as Voltaren) is a topical NSAID that delivers anti-inflammatory medication directly through the skin. For best results, you apply it four times a day. The amount varies by location: a roughly 2.25-inch strip of gel for upper body joints like hands, wrists, or elbows, and a 4.5-inch strip for lower body joints like knees, ankles, or feet. Capsaicin cream, derived from hot peppers, works differently. It gradually depletes the nerve chemicals that transmit pain signals from the joint. The catch is that it causes a burning sensation when you first start using it, and that sensation only fades with consistent, daily use. Reducing how often you apply it won’t help the burning and will reduce the pain relief. Most people need one to two weeks of regular application before they notice meaningful improvement.

Curcumin
