A charley horse is a sudden, involuntary muscle contraction, most often in the calf, that can last anywhere from a few seconds to several minutes. The most common causes are muscle fatigue, dehydration, and low levels of key minerals like potassium, calcium, or magnesium. But the full picture is more nuanced, and understanding the specific trigger behind your cramps can help you prevent them.
Muscle Fatigue and Nerve Misfiring
The leading theory behind exercise-related charley horses centers on what happens inside your nervous system when a muscle gets tired. Normally, your body has a built-in feedback loop: sensors in your tendons monitor how much tension a muscle is under and send signals to the spinal cord to dial back the contraction when needed. When a muscle is fatigued, this system breaks down. The sensors that detect stretch become overexcited, while the sensors that should pump the brakes become less active. The result is a runaway signal that locks the muscle into a sustained, painful contraction.
This is why charley horses so often strike during or after intense exercise, especially when you push a muscle beyond what it’s conditioned for. It also explains why the cramp typically hits muscles that cross two joints (like the calf, which spans the knee and ankle), since those muscles are more vulnerable to being shortened and fatigued simultaneously.
Dehydration and Fluid Shifts
Heavy sweating without adequate fluid replacement changes the concentration of your body’s extracellular fluid. As that fluid becomes more concentrated, water migrates out of the spaces between cells (the interstitial space) to try to balance things out. This shift increases pressure on the nerve endings that run through muscle tissue, physically deforming them and triggering involuntary contractions.
This is why charley horses are more common in hot weather, during long endurance events, or anytime you’re sweating heavily without drinking enough. The combination of fluid loss and mineral depletion through sweat creates a double hit on the muscles’ ability to contract and relax normally.
Electrolyte Imbalances
Your muscles depend on a precise balance of potassium, sodium, calcium, and magnesium to contract and relax properly. At the cellular level, tiny pumps on muscle fibers constantly shuttle sodium out and potassium in, maintaining the electrical charge that allows a muscle to fire on command and then release. When potassium levels drop or sodium balance is off, those pumps can’t keep up with the demands of an active muscle. The electrical signal that tells the muscle to relax essentially stalls.
Calcium plays a similarly critical role. It’s the mineral that directly triggers muscle fibers to shorten. When calcium regulation goes wrong, whether from dietary deficiency or a condition that throws off your body’s calcium balance, muscles become prone to contracting when they shouldn’t. Magnesium supports this entire system by helping regulate both calcium and potassium channels, which is why it’s often cited as a cramp remedy.
That said, the evidence for magnesium supplements as a cramp fix is weaker than most people assume. A Cochrane review, one of the most rigorous forms of medical evidence analysis, found that magnesium supplements did not meaningfully reduce cramp frequency, intensity, or duration in older adults with nighttime leg cramps. Across five studies involving over 300 participants, the difference between magnesium and placebo amounted to fewer than 0.2 cramps per week. The supplements did, however, cause digestive side effects like diarrhea in up to 37% of people taking them.
Medications That Trigger Cramps
Several common medications increase cramp risk, often by disrupting electrolyte balance. Diuretics (water pills) prescribed for blood pressure are a well-known culprit. They work by flushing extra fluid from your body, but they take potassium, magnesium, and sodium along with it. Research has documented reduced concentrations of all three minerals, plus reduced activity of the sodium-potassium pumps in muscle tissue, during diuretic treatment.
Statins, used to lower cholesterol, are another frequent cause. Mild muscle pain is a common side effect, and in rare cases, statins can trigger a more serious condition involving muscle breakdown that causes severe cramping and soreness. The risk increases when statins are combined with certain other medications.
Why Cramps Strike at Night
Nighttime charley horses are extremely common, particularly in older adults, and often have no single identifiable cause. The best explanation is a combination of tired muscles from the day’s activity and the way your body’s nerve signaling changes during sleep. When you’re lying still for hours, your feet naturally point downward, which shortens the calf muscle. A muscle held in a shortened position is more susceptible to spontaneous cramping because the tension sensors in the tendon are less active in that position, removing the natural brake on contraction.
Poor circulation while lying down and the fact that you’re not drinking water for hours may also contribute. For many people, nighttime cramps come and go without ever being tied to a specific medical issue.
Pregnancy-Related Cramps
Leg cramps are a familiar complaint during pregnancy, particularly in the second and third trimesters. The exact mechanism isn’t fully understood, but some research points to lower circulating calcium levels during pregnancy as a contributing factor. The added weight and changes in circulation that come with carrying a baby also put extra strain on leg muscles, making them more fatigue-prone. For pregnant women, the evidence on whether magnesium supplements help is mixed, unlike in older adults where it’s clearly ineffective.
Underlying Medical Conditions
Occasional charley horses are normal. Frequent or severe cramps, especially if they don’t respond to stretching and hydration, can signal something else going on. Conditions linked to recurring cramps include:
- Reduced blood flow. Narrowed arteries in the legs can cause cramping pain during exercise that eases with rest. This is more common in older adults and smokers.
- Nerve compression. Pressure on nerves in the spine, from a herniated disc or spinal stenosis, can irritate the nerves that control leg muscles and trigger spasms. This type of cramping typically worsens with walking.
- Diabetes. Nerve damage from diabetes can disrupt normal muscle signaling and make cramps more frequent.
- Liver and thyroid disease. Both conditions alter the body’s mineral balance and nerve function in ways that predispose muscles to cramping.
Charley Horse vs. Blood Clot
A charley horse can feel alarming, but it’s a brief event. The muscle seizes, you stretch it out, and the pain fades within minutes. A deep vein thrombosis (blood clot in the leg) can initially feel like a cramp, but the two are very different.
With a blood clot, the pain is constant and worsens over time. Stretching or walking it off won’t help. The affected leg, usually just one, will often be visibly swollen, warm to the touch, and may appear reddish or discolored. If you have calf pain that persists, especially with swelling and warmth in one leg, that warrants prompt medical attention since a clot can become dangerous if it travels to the lungs.
Practical Ways to Prevent Cramps
Since most charley horses come down to some combination of fatigue, dehydration, and mineral depletion, prevention follows logically. Stay hydrated before, during, and after exercise, particularly in heat. Make sure your diet includes potassium-rich foods like bananas, potatoes, and leafy greens, along with adequate calcium from dairy or fortified alternatives.
Stretching the calves before bed can help if you’re prone to nighttime cramps. A simple wall stretch, where you lean forward with your hands on a wall and one leg extended behind you, held for 30 seconds per side, keeps the calf lengthened and may reduce the chance of a spontaneous contraction during sleep. Gradually increasing exercise intensity rather than jumping into unfamiliar activity also lowers the risk, since conditioned muscles are far less likely to experience the nerve misfiring that sets off a cramp.
