What Causes a Sore Throat When You’re Sick?

A sore throat during illness is caused by your immune system’s inflammatory response to infection, not by the virus or bacteria directly destroying tissue. When a pathogen invades the lining of your throat, your body releases chemical signals that increase blood flow, cause swelling, and activate pain nerve endings in the area. That inflammation is what you feel as soreness, and several overlapping mechanisms make it worse.

How Infection Triggers Throat Pain

When a virus or bacterium lands on the mucous membrane of your throat, your immune cells detect it and launch a defensive response. Part of that defense involves releasing inflammatory chemicals, including prostaglandins and bradykinin, into the surrounding tissue. These chemicals do two things: they dilate blood vessels to rush more immune cells to the area (causing the redness and swelling you’d see if you looked in a mirror), and they directly stimulate pain nerve endings embedded in the throat lining.

Bradykinin is a particularly potent pain trigger. It’s generated in the nasal passages and throat during rhinovirus infections (the common cold), and it sensitizes nerve endings so thoroughly that even mild stimulation, like swallowing saliva, registers as pain. This is why your throat can hurt constantly during a cold, not just when you eat or drink.

The nerve responsible for carrying those pain signals from the back of your throat to your brain is the glossopharyngeal nerve, one of 12 cranial nerves. It connects to your tonsils, the base of your tongue, your voice box, and the area around your ears and jaw. That wide network of connections explains why a throat infection can make your ears ache, why pain sometimes radiates to your jaw, and why swallowing feels like it hurts “everywhere” rather than in one precise spot.

Viruses Cause Most Sore Throats

Viruses account for 50% to 80% of all sore throats during illness. Rhinoviruses (common colds), influenza, adenoviruses, and respiratory syncytial virus are among the most frequent culprits. These infections tend to come with other symptoms like a runny nose, cough, sneezing, and mild body aches, which help distinguish them from bacterial causes.

Bacterial infections, primarily Group A Streptococcus (strep throat), cause 5% to 15% of sore throats in adults and 20% to 30% in children. The distinction matters because strep requires antibiotics while viral infections do not. Strep throat tends to come on suddenly, often with a fever, swollen lymph nodes in the neck, and white patches or pus on the tonsils, but without the cough and runny nose typical of a cold. Clinicians use a scoring tool based on age, fever, swollen lymph nodes, tonsillar coating, and presence or absence of cough to decide whether testing for strep is warranted.

Postnasal Drip and Mouth Breathing Make It Worse

The infection itself isn’t the only thing irritating your throat. When you’re sick, your nose produces excess mucus, and much of it drains down the back of your throat. This postnasal drip bathes the tissue in a steady stream of mucus that irritates and swells the tonsils and surrounding tissue. The soreness from postnasal drip often feels worse in the morning because mucus pools while you sleep.

Nasal congestion also forces you to breathe through your mouth, especially at night. Mouth breathing bypasses the nose’s natural ability to warm and humidify incoming air, so dry air flows directly over an already inflamed throat. The result is a dried-out mucous membrane that’s even more sensitive to pain. This is a major reason many people wake up with the worst sore throat of the day, then notice it improves somewhat after drinking fluids and moving around.

What a Viral Sore Throat Feels Like Over Time

A typical viral sore throat follows a predictable arc. It usually starts as a scratchy, dry sensation on day one or two of illness, peaks in intensity around days two through four, and then gradually fades over about a week. You might notice the pain shifts from a constant ache to something that only flares when you swallow, which is a sign the inflammation is resolving.

If your sore throat persists beyond two weeks, the cause may not be a simple infection. Chronic throat irritation can stem from acid reflux (stomach acid washing up into the throat, especially at night), ongoing postnasal drip from allergies or sinus problems, or habitual mouth breathing in dry environments. Rarely, persistent throat pain can signal something more serious that needs evaluation.

When a Sore Throat Signals Something Serious

Most sore throats during illness are uncomfortable but harmless. A few warning signs, however, point to complications that need urgent attention. Difficulty breathing, a high-pitched whistling sound when inhaling (called stridor), or an inability to swallow your own saliva are red flags. Excessive drooling in a child who seems anxious and is leaning forward to breathe can indicate epiglottitis, a swelling of the tissue that covers the windpipe. This is a medical emergency.

A sore throat that gets dramatically worse on one side, makes it hard to open your mouth, or causes a muffled “hot potato” voice could indicate a peritonsillar abscess, a pocket of pus forming near the tonsil. Strep throat that goes untreated can also lead to complications affecting the heart and kidneys, which is why testing and antibiotics matter when strep is suspected. For confirmed strep, a standard course of antibiotics typically allows a return to work or school within 12 to 24 hours of starting treatment, provided there’s no fever.

Why It Hurts More to Swallow

Swallowing requires the muscles of your throat to contract and squeeze together, pressing inflamed tissue against itself. When the lining is swollen and loaded with pain-sensitizing chemicals like bradykinin and prostaglandins, that mechanical pressure fires off pain signals far more intensely than when the throat is at rest. Cold liquids can temporarily dull this by slightly constricting blood vessels and slowing nerve signaling, while hot or acidic foods tend to amplify it.

The tonsils, sitting at the back of the throat where food and air pass, bear the brunt of this. They’re part of the immune system and often swell significantly during infection as they trap and fight pathogens. That swelling narrows the space available for swallowing, which means more tissue contact with every gulp, and more pain.
