A sudden drop in blood pressure happens when something disrupts your body’s ability to keep blood flowing steadily to your organs. The causes range from standing up too quickly to serious medical emergencies like severe infections or allergic reactions. Low blood pressure is generally defined as a reading below 90/60 mmHg, but what matters more than a single number is how fast the drop happens and whether your body can compensate.
Standing Up Too Fast
The most common cause of a sudden blood pressure drop is simply changing position, a condition called orthostatic hypotension. When you stand, gravity pulls more than 500 milliliters of blood downward into your legs and abdomen. Normally, your nervous system detects this shift within seconds and tightens your blood vessels while speeding up your heart rate to compensate. When that system is slow or impaired, your blood pressure drops before your body can catch up, and you feel dizzy or lightheaded.
Orthostatic hypotension is diagnosed when standing causes your systolic pressure (the top number) to fall by more than 20 points or your diastolic pressure (the bottom number) to fall by more than 10 points within three minutes. It becomes more common with age because the nervous system reflexes that regulate blood pressure get slower, and blood vessels become stiffer and less responsive.
The Vasovagal Response
Sometimes your nervous system overreacts to a trigger and causes both your heart rate and blood pressure to plummet at the same time. This is called vasovagal syncope, and it’s the reason people faint at the sight of blood, during a blood draw, or after standing in one place for too long. Heat exposure, fear of injury, and straining on the toilet are also common triggers.
During a vasovagal episode, your heart rate slows and the blood vessels in your legs widen simultaneously. Blood pools in your lower body, pressure drops, and your brain briefly loses adequate blood flow. The result is the classic feeling of tunnel vision, nausea, and warmth right before fainting. These episodes are usually harmless on their own, though falling can cause injury.
Medications That Lower Blood Pressure
Many common medications can cause sudden or unexpected drops in blood pressure, especially when you first start taking them, change your dose, or combine multiple drugs. The biggest culprits fall into several categories:
- Blood pressure medications: Alpha-blockers commonly cause severe drops when standing. Beta-blockers, calcium channel blockers, and ACE inhibitors can all lower pressure more than intended, particularly when combined.
- Diuretics (water pills): Both loop diuretics and thiazide diuretics work by removing fluid from your body. If you become even mildly dehydrated, your blood volume drops and so does your pressure. Low potassium from these drugs can also contribute to weakness and fainting.
- Heart medications: Nitroglycerin and similar drugs used for chest pain are a common cause of sudden fainting because they rapidly relax blood vessels.
- Psychiatric medications: Tricyclic antidepressants, antipsychotics, MAO inhibitors, and even some newer antidepressants like SNRIs can cause orthostatic hypotension. This is one reason falls are a significant concern for older adults on these medications.
- Parkinson’s disease drugs: Dopamine-based medications used for Parkinson’s frequently cause blood pressure drops when standing.
If you’ve recently started a new medication or had a dose change and you’re feeling dizzy when you stand, the timing is probably not a coincidence.
Dehydration and Blood Loss
Your blood pressure depends partly on having enough fluid in your circulatory system. Anything that reduces blood volume can cause a rapid drop. Obvious causes include heavy bleeding from an injury or surgery, but less obvious ones are just as common: prolonged vomiting, severe diarrhea, excessive sweating, or simply not drinking enough water on a hot day.
When blood volume drops significantly, the condition is called hypovolemic shock. Your heart tries to compensate by beating faster, but if too much volume is lost, it can’t maintain adequate pressure. The skin becomes cold and clammy, breathing speeds up, and confusion sets in. This is a medical emergency that requires immediate fluid replacement.
Severe Infections and Allergic Reactions
Two of the most dangerous causes of sudden blood pressure drops are sepsis and anaphylaxis. Both fall under a category called distributive shock, where blood vessels throughout the body suddenly relax and widen. This dramatically lowers the resistance that normally keeps blood pressure stable, and the small blood vessels become leaky, allowing fluid to escape from the circulation.
In sepsis, a bacterial infection triggers an overwhelming immune response that causes blood vessels to dilate uncontrollably. In anaphylaxis, a severe allergic reaction (to foods, insect stings, or medications, for example) does essentially the same thing through a different chemical pathway. Both conditions can cause blood pressure to crash within minutes, and both require emergency treatment.
Heart Problems
Your heart is the pump that generates blood pressure, so any condition that weakens the pump can cause pressure to drop. A very slow heart rate, called bradycardia, means the heart isn’t pushing blood out frequently enough to maintain steady pressure. This can result from aging-related damage to the heart’s electrical system, heart attacks, inflammation of the heart muscle, or conditions you’re born with.
Some people experience alternating fast and slow heart rhythms, where the heart’s natural pacemaker misfires. Heart valve problems can also prevent efficient blood flow. If your heart can’t fill properly or can’t eject blood with enough force, pressure falls and symptoms like dizziness, fatigue, and fainting follow.
Drops After Eating
Postprandial hypotension is a blood pressure drop that occurs within two hours of eating a meal. After you eat, your body diverts extra blood to your digestive system. In most people, blood vessels elsewhere tighten to compensate. But in older adults especially, stiffened arteries can’t adjust quickly enough, and blood pressure falls.
Large meals and meals high in carbohydrates tend to cause the biggest drops. Eating smaller, more frequent meals and avoiding heavy carbohydrate loads can reduce episodes. This condition is particularly common in people who already have high blood pressure or neurological conditions like Parkinson’s disease.
Hormonal Causes
Your adrenal glands produce hormones that play a direct role in keeping blood pressure stable. Cortisol helps control blood pressure, while aldosterone maintains the balance of sodium and potassium that regulates fluid levels in your blood. When the adrenal glands don’t produce enough of these hormones, a condition called adrenal insufficiency (or Addison’s disease), blood pressure can drop dangerously.
During periods of physical stress like illness, injury, or surgery, the body normally ramps up cortisol production. People with adrenal insufficiency can’t do this, which can trigger a life-threatening crisis with severely low blood pressure, low blood sugar, and dangerous shifts in sodium and potassium levels. Thyroid disorders can also contribute to blood pressure instability, though typically less dramatically.
Warning Signs That Need Immediate Attention
Mild dizziness when standing up quickly is common and usually not dangerous. But certain symptoms alongside a blood pressure drop signal that your organs aren’t getting enough blood. Cold, clammy skin means your body is diverting blood away from the surface to protect vital organs. Rapid, shallow breathing is the body’s attempt to increase oxygen delivery. Confusion or difficulty thinking clearly suggests your brain isn’t receiving adequate blood flow.
A weak, rapid pulse paired with pale skin and these other symptoms points toward shock, regardless of the underlying cause. The combination of a known trigger (recent infection, allergic exposure, heavy bleeding, or new medication) with these signs warrants emergency care, because blood pressure drops from these causes tend to worsen rapidly without treatment.
