What Causes Calluses on Feet and How to Treat Them

Calluses form on your feet when skin is exposed to repeated pressure or friction, triggering a protective response that thickens the outer layer of skin. The most common culprits are ill-fitting shoes, specific foot shapes, and activities that put sustained stress on certain areas of the foot. While calluses are rarely dangerous on their own, understanding what drives them helps you prevent them from coming back.

How Calluses Form

Your skin has a built-in defense mechanism. When the same spot on your foot gets pressed or rubbed over and over, the skin cells in that area start multiplying faster than normal and produce extra keratin, a tough structural protein. This accelerated process thickens the outermost layer of skin, called the stratum corneum, creating the firm, yellowish patch you recognize as a callus.

This thickening is technically called mechanical hyperkeratosis, and it’s your body’s attempt to build armor against ongoing damage. The process is entirely benign. It only becomes a problem when the thickened skin grows large enough to cause discomfort, cracks open, or develops over a bony prominence where it can press inward and cause pain.

Shoes Are the Biggest Factor

Footwear is the single most common cause of calluses, and it works in several ways. Tight shoes create excess pressure, especially across the toes and the ball of the foot. Shoes that are too loose let your foot slide around inside, generating friction with every step. Both extremes damage the skin enough to kick off that thickening response.

Specific shoe types carry higher risk. High heels shift your body weight forward onto the forefoot, concentrating pressure under the metatarsal heads (the bony bumps behind your toes). Pointed-toe shoes squeeze the toes together, creating friction between them and against the shoe’s interior. Thin-soled shoes and flats with no arch support spread impact unevenly, overloading areas that weren’t designed to bear that much force.

Material matters too. Non-breathable shoes trap sweat, which softens the skin and makes it more vulnerable to friction damage. Leather and mesh uppers allow airflow. Synthetic materials often don’t.

Foot Structure and Gait

Some people are simply more prone to calluses because of how their feet are shaped or how they walk. High arches concentrate pressure on the heel and ball of the foot, leaving less surface area to absorb impact. Flat feet can cause the foot to roll inward excessively, creating friction along the inner edge. Bunions, hammertoes, and other structural changes push bony areas into contact with shoes in ways a normally shaped foot wouldn’t experience.

Your walking pattern plays a role too. If you tend to strike heavily on one side of your foot or push off unevenly, the skin in those high-pressure zones thickens faster. Runners and people who spend long hours on their feet often develop calluses in predictable locations: under the ball of the foot, along the outer edge of the big toe, and on the heel.

Other Contributing Factors

Going barefoot regularly, especially on hard surfaces, exposes the soles to direct friction and pressure without any cushioning. Walking on concrete, tile, or hardwood floors without shoes is a reliable way to build up calluses on the heels and forefoot.

Age also plays a part. As you get older, the fat pads on the bottom of your feet thin out, removing a natural cushion that once absorbed shock. With less padding between bone and ground, the skin takes on more mechanical stress. Dry skin compounds the problem because it’s less elastic and more likely to crack or thicken in response to pressure.

How to Reduce and Prevent Calluses

Since pressure and friction are the root causes, prevention comes down to minimizing both. Start with shoes that fit properly: snug enough that your foot doesn’t slide, roomy enough that your toes aren’t compressed. A thumb’s width of space between your longest toe and the front of the shoe is a good benchmark. Look for cushioned soles and breathable materials.

Insoles and orthotic inserts can redistribute pressure away from problem areas. Custom orthotics designed around your foot’s pressure profile are particularly effective for people with recurring calluses under the metatarsal heads. Even over-the-counter metatarsal pads, which sit just behind the ball of the foot, can shift weight off high-pressure zones and provide meaningful relief.

Moisture-wicking socks reduce friction and keep skin from softening excessively. If you’re active or on your feet for work, changing socks midday can help. Applying a basic moisturizer to your feet at night keeps skin pliable and less prone to cracking, which can worsen callus buildup.

Treating Existing Calluses at Home

For most people, calluses respond well to simple home care. Soaking your feet in warm water for 10 to 15 minutes softens the thickened skin, making it easier to gently file down with a pumice stone or foot file. The key word is “gently.” Removing too much skin at once can cause soreness or expose raw tissue.

Over-the-counter creams containing urea at concentrations up to 40% work as keratolytic agents, meaning they chemically break down the excess keratin that makes calluses thick and hard. Products combining 40% urea with 2% salicylic acid are particularly effective because the salicylic acid loosens dead skin cells while the urea softens and hydrates the area. Apply these to the callused area at night, cover with socks, and let them work while you sleep. Results typically show within one to two weeks of consistent use.

Medicated callus pads containing salicylic acid are another option, though they need to be placed carefully to avoid irritating the healthy skin surrounding the callus.

When Calluses Become a Medical Concern

For most healthy adults, calluses are a cosmetic annoyance at worst. But for people with diabetes, peripheral neuropathy, or blood circulation problems, they can be genuinely dangerous. Nerve damage from diabetes distorts foot anatomy over time, producing claw toes, high arches, and prominent metatarsal heads that make callus formation almost inevitable. At the same time, reduced sensation means you may not feel the pain that would normally signal a problem.

The real danger is what happens underneath. Increased plantar pressure combined with callus buildup creates a mechanical environment highly susceptible to ulceration. Calluses can mask bleeding or tissue breakdown happening beneath the thickened skin, and by the time it becomes visible, a wound may already be infected. Diabetic foot ulcers are a leading cause of lower-limb amputation, so calluses in this population aren’t something to manage with a pumice stone alone.

If you have diabetes, peripheral neuropathy, poor circulation, or chronic kidney disease, routine foot care including callus removal is considered a medical procedure rather than a cosmetic one. A podiatrist can safely debride thickened skin, assess for underlying tissue damage, and set up a monitoring schedule that catches problems early.
