Gallbladder pain is most often caused by gallstones blocking the flow of bile, triggering intense cramping in the upper abdomen. About 6% of the global population has gallstones, and while many people carry them without symptoms, a stone lodged in the wrong spot can produce pain that’s hard to ignore. Beyond stones, several other conditions can inflame or irritate the gallbladder and produce similar discomfort.
How Gallstones Cause Pain
Your gallbladder is a small, pear-shaped organ tucked beneath your liver. Its job is to store bile, a digestive fluid, and squeeze it into your small intestine when you eat. Gallstones form when substances in bile, usually cholesterol, crystallize into solid pieces ranging from grain-of-sand size to golf-ball size.
The pain starts when a stone gets pushed into the narrow duct that connects the gallbladder to the rest of the digestive tract. The duct spasms around the obstruction, producing what’s called biliary colic: a steady, building ache in the upper right abdomen or just below the breastbone. Despite the word “colic,” the pain doesn’t come in sharp waves like intestinal cramps. It typically ramps up over minutes, holds at a peak for anywhere from 20 minutes to several hours, then gradually fades once the stone shifts.
Fatty meals are the classic trigger. When fats and proteins reach your small intestine, cells lining the gut release a hormone that tells the gallbladder to contract. That contraction forces a stone into the duct opening. This is why gallbladder attacks so often strike after a heavy dinner or a greasy meal rather than first thing in the morning.
When the Gallbladder Becomes Inflamed
If a stone stays stuck long enough, the gallbladder wall becomes swollen and inflamed, a condition called cholecystitis. The pain shifts from episodic to constant and is typically more severe. Fever and chills often accompany it, which is a key distinction from a simple stone episode. On ultrasound, the gallbladder wall thickens beyond 4 millimeters, and fluid may collect around the organ.
Inflammation can also happen without any stones at all. This is known as acalculous cholecystitis, and it tends to develop in people who are already seriously ill: those recovering from major surgery, dealing with severe burns, or spending extended time in intensive care. Prolonged fasting or receiving nutrition entirely through an IV deprives the gallbladder of its normal hormonal signal to contract. Bile sits stagnant, thickens, and irritates the gallbladder wall. Though less common than stone-related inflammation, acalculous cholecystitis can be more dangerous because it’s harder to catch early.
Gallbladder Pain Without Gallstones
Some people experience textbook gallbladder symptoms, repeated upper-abdominal pain after meals, nausea, bloating, yet imaging shows no stones at all. The problem may be in how the gallbladder contracts. In a condition called biliary dyskinesia, the gallbladder either squeezes too weakly or too forcefully.
To measure this, doctors use a specialized scan that tracks how much bile the gallbladder empties after being stimulated. A normal gallbladder ejects roughly 35% to 80% of its contents. Below 35% is considered sluggish, meaning bile pools and causes discomfort. Above 80% suggests the gallbladder is contracting too aggressively, which can also produce pain. Both patterns can feel identical to a gallstone attack, and many people with dyskinesia go through multiple rounds of testing before getting a diagnosis.
Stones in the Common Bile Duct
Sometimes a stone slips past the gallbladder and lodges in the common bile duct, the larger tube that carries bile to the intestine. This creates a different, often more serious problem. The blockage backs up bile into the liver, which can turn the skin and eyes yellow (jaundice) and cause dark urine and pale stools. Blood tests typically show a spike in liver enzymes and bilirubin levels, sometimes dramatically high, because the backed-up pressure damages liver cells. These enzyme levels drop quickly once the blockage is cleared, which helps doctors distinguish a duct stone from actual liver disease.
A bile duct stone also raises the risk of infection in the duct itself or inflammation of the pancreas, since the pancreatic duct shares the same opening into the intestine. Pain from a duct stone tends to be more persistent and central in the upper abdomen compared to a typical gallbladder attack.
Where Gallbladder Pain Shows Up
The classic location is the upper right side of the abdomen, just below the ribcage. But gallbladder pain is notorious for showing up in unexpected places. Many people feel it dead center in the upper abdomen, which is why it’s so often mistaken for heartburn or a stomach ulcer.
Perhaps the most confusing pattern is referred pain to the right shoulder or between the shoulder blades. This happens because the gallbladder shares nerve pathways with the diaphragm. The right phrenic nerve, which runs from the neck down to the diaphragm, carries sensory signals from the gallbladder. Your brain interprets those signals as coming from the shoulder area rather than the abdomen. Additional nerve fibers from the mid-spine region (around the level of the lower chest) contribute to pain that wraps around to the back.
Telling It Apart From Other Conditions
Gallbladder pain overlaps with several other conditions, and the timing and triggers offer the best clues. A peptic ulcer tends to cause a gnawing or burning sensation that improves with food or antacids. Gallbladder pain worsens after eating, particularly after fatty meals, and antacids do nothing for it. Ulcer pain can wake you at night on an empty stomach; gallbladder pain is more likely to hit an hour or two after dinner.
Duration matters too. A gallbladder episode typically lasts at least 20 minutes and can stretch to several hours before resolving. Acid reflux and indigestion tend to fluctuate more and respond to position changes. Fever and chills point toward gallbladder inflammation rather than an ulcer, where fever is uncommon. Pain radiating to the right shoulder is another strong indicator that the gallbladder is the source rather than the stomach.
Who Is Most at Risk
Gallstones affect women more than men. Globally, about 7.3% of women have gallstones compared to 5.3% of men, largely because of hormonal differences. Estrogen increases the amount of cholesterol the liver dumps into bile, making it more likely to crystallize. It also slows gallbladder contractions, so bile sits longer and has more opportunity to form stones. Progesterone further reduces gallbladder motility. This is why pregnancy is a well-known risk period: rising levels of both hormones create ideal conditions for stone formation.
Age is the other major factor. Prevalence roughly doubles after 50, climbing from about 3.4% in younger adults to nearly 9% in those over 50. Geography plays a role as well. Rates are highest in Latin America, where roughly 15% of the general population has stones, followed by North America at about 8%. Rates in South Asia and the Middle East hover around 4% to 5%.
Rapid weight loss, obesity, and diets high in refined carbohydrates also increase risk. Crash diets are a particularly overlooked trigger. When you lose weight very quickly, the liver secretes extra cholesterol into bile, and the gallbladder, stimulated less often due to reduced food intake, doesn’t empty frequently enough to flush it out.
