High blood pressure in cats is almost always caused by another underlying disease. Chronic kidney disease and an overactive thyroid gland are the two most common culprits, and the condition overwhelmingly affects older cats. In a smaller number of cases, no underlying cause can be identified, which veterinarians call idiopathic or primary hypertension. Understanding the root cause matters because treating the blood pressure alone, without addressing what’s driving it, rarely solves the problem long term.
Chronic Kidney Disease Is the Leading Cause
Chronic kidney disease (CKD) is the single most frequent condition linked to high blood pressure in cats. As the kidneys lose function, they struggle to regulate fluid balance and filter waste properly. The body responds by activating a hormonal cascade that tightens blood vessels and holds onto salt and water, both of which push blood pressure up. This system, sometimes called the renin-angiotensin-aldosterone system, is meant to be a short-term fix for low blood pressure. When it stays switched on due to kidney damage, the result is chronic hypertension.
The relationship runs in both directions. CKD causes high blood pressure, but high blood pressure also accelerates kidney damage by forcing excess protein through the kidneys’ filtering units and causing scarring in the blood vessels that supply them. A study of over 200 cats found increased scarring and vessel damage in cats with higher blood pressure. This vicious cycle is why early detection matters so much.
Hyperthyroidism and Other Hormonal Conditions
An overactive thyroid gland is extremely common in older cats, and it frequently raises blood pressure. The excess thyroid hormone speeds up the heart rate, increases the volume of blood the heart pumps, and makes blood vessels more reactive. Many cats are diagnosed with hypertension and hyperthyroidism at the same time, and in some cases, getting thyroid levels under control is enough to normalize blood pressure.
A less common but important hormonal cause is a condition where the adrenal glands produce too much aldosterone. Aldosterone tells the kidneys to retain sodium and water, which expands blood volume and raises pressure. Tumors or overactivity in the adrenal glands can cause this. Other hormonal conditions, including diabetes and disorders that lead to excess cortisol production, can also contribute to hypertension, though less frequently than kidney disease or thyroid problems.
Why Older Cats Are Most at Risk
Feline hypertension is primarily a disease of aging. Both CKD and hyperthyroidism become increasingly common as cats move into their senior years, typically after age 10. Since these two conditions are the main drivers of high blood pressure, older cats carry a much higher risk simply because they’re more likely to develop one or both underlying diseases. Cats younger than 7 are rarely diagnosed with hypertension unless they have an unusual condition affecting the kidneys, adrenal glands, or thyroid.
Idiopathic Hypertension: When No Cause Is Found
In a minority of cats, thorough testing reveals no kidney disease, no thyroid abnormality, and no other identifiable trigger. These cats are diagnosed with primary or idiopathic hypertension. It’s the feline equivalent of “essential hypertension” in humans. Some of these cats may have early kidney disease that hasn’t yet shown up on blood work, or subtle hormonal imbalances that current tests can’t detect. Regardless, the condition still needs treatment because the consequences of uncontrolled blood pressure are the same no matter the cause.
How High Blood Pressure Damages a Cat’s Body
What makes hypertension dangerous isn’t the elevated pressure itself but the damage it quietly inflicts on vulnerable organs. The eyes, brain, heart, and kidneys are the primary targets.
Eyes
The eyes are the organ most visibly affected. Roughly 50% of hypertensive cats develop eye changes, and retinal damage can begin at a systolic blood pressure of around 160 mmHg. The tiny blood vessels in the retina are especially fragile. Sustained high pressure can cause bleeding inside the eye, fluid buildup beneath the retina, or full retinal detachment. Many cats with severe damage present with sudden blindness and dilated pupils that don’t respond to light. If treated very quickly, some vision loss can be reversed, but in most cases the blindness is permanent.
Brain
Between 15% and 46% of hypertensive cats show neurological signs. When blood pressure stays high long enough, it overwhelms the brain’s ability to regulate its own blood flow, leading to swelling and dysfunction. You might notice your cat seeming disoriented, walking unsteadily, having seizures, or acting unusually depressed or confused. These signs tend to appear when blood pressure rises rapidly, and they often improve once treatment brings the pressure down.
Heart
The heart has to work harder to push blood through vessels under increased pressure. Over time, this extra workload causes the heart walls to thicken, a change seen in up to 60% of hypertensive cats. A veterinarian might detect a heart murmur, gallop rhythm, or irregular heartbeat on a physical exam. In rare cases, the strain can progress to heart failure.
Kidneys
As mentioned, hypertension and kidney disease fuel each other. High blood pressure damages the delicate filtering structures inside the kidneys and increases protein leakage into the urine. This happens in cats with and without pre-existing kidney disease, meaning hypertension from any cause can start kidney problems even if the kidneys were initially healthy.
How Blood Pressure Is Measured in Cats
Measuring blood pressure in cats comes with a unique challenge: the “white coat effect.” Cats that are anxious, stressed, or uncooperative at the vet can have artificially elevated readings that don’t reflect their true resting blood pressure. Veterinarians typically use a small inflatable cuff on the leg or tail connected to a device that detects blood flow.
The Doppler method, which uses an ultrasound probe to listen for blood flow, tends to be faster and more practical for cats. Studies have shown it takes under five minutes to get reliable readings with a Doppler device in most cats, compared to the oscillometric method, which is slower and less cooperative-cat-friendly. To minimize stress, many clinics allow the cat to acclimate to the exam room for several minutes before taking readings, and some will take measurements with the cat in its carrier or on the owner’s lap. A systolic reading consistently at or above 160 mmHg is considered elevated, and readings above 180 mmHg indicate severe hypertension with a high risk of organ damage.
How Feline Hypertension Is Managed
Treatment has two goals: lower the blood pressure to reduce the risk of organ damage, and address whatever underlying disease is driving it. If hyperthyroidism is the cause, managing the thyroid condition through medication, dietary therapy, or other approaches may bring blood pressure back to normal on its own. If CKD is the underlying problem, blood pressure medication is typically needed because the kidney damage is usually irreversible.
The most commonly used blood pressure medication for cats is a calcium channel blocker given as a small oral tablet, usually once or twice daily. It works by relaxing the walls of blood vessels, allowing blood to flow more easily and reducing the pressure. Most cats tolerate it well, and blood pressure often drops noticeably within the first week or two. One consideration is that this type of medication can trigger the body’s salt-and-water-retention hormones as a compensatory response. In cats that also have kidney disease, veterinarians sometimes add a second medication to counteract that hormonal activation and protect the kidneys from further damage.
Once treatment starts, regular blood pressure monitoring becomes part of your cat’s routine care. Most vets recommend rechecking within one to two weeks of starting medication, then every three to four months once the pressure is stable. Eye exams are also important at follow-up visits because retinal changes can develop silently before any outward signs appear.
