What Causes High Cholesterol in Men: Key Factors

High cholesterol in men results from a combination of diet, body composition, genetics, hormonal shifts, and lifestyle habits like smoking and drinking. Men tend to develop unfavorable cholesterol levels earlier than women, partly because they lack the protective effects of estrogen on blood vessel walls and partly because male-pattern fat storage around the abdomen directly affects how the liver processes cholesterol. A healthy total cholesterol level for men 20 and older is less than 200 mg/dL, with LDL (“bad”) cholesterol below 100 mg/dL and HDL (“good”) cholesterol at 60 mg/dL or above.

How Belly Fat Drives Cholesterol Up

The type of fat matters more than total body weight. Men are more likely to store fat around the organs in the abdomen, known as visceral fat, rather than under the skin on the hips and thighs. This visceral fat isn’t just a storage depot. It acts as an active organ that releases inflammatory compounds, including interleukin-6 and other signaling molecules, at higher rates than fat stored elsewhere in the body. These substances flow directly to the liver through the portal vein and interfere with how the liver manages cholesterol and triglycerides.

The result is that the liver ramps up production of triglyceride-rich particles, which eventually get remodeled into LDL cholesterol in the bloodstream. This is why a man with a relatively normal weight but a large waist circumference can still have high cholesterol. If your waist measures over 40 inches, your liver is likely dealing with the metabolic consequences of excess visceral fat, even if your BMI looks fine on paper.

What Saturated Fat Does Inside the Body

Diets high in saturated fat, found in red meat, full-fat dairy, butter, and many processed foods, raise LDL cholesterol through a specific mechanism. Saturated fats suppress the activity of LDL receptors on liver cells. These receptors are responsible for pulling LDL particles out of the bloodstream, so when they’re less active, LDL accumulates. Saturated fats also interfere with cholesterol transport inside liver cells, reducing the cell’s ability to process and clear cholesterol efficiently.

Trans fats, still present in some fried and commercially baked foods, do the same thing but worse: they raise LDL while simultaneously lowering HDL. Replacing even a portion of saturated fat with unsaturated fats from sources like olive oil, nuts, and fatty fish can meaningfully shift the balance.

Testosterone’s Role in Cholesterol

Testosterone has a complicated relationship with cholesterol in men. During puberty, rising testosterone is actually associated with increasing triglycerides and falling HDL. But in adult men, the relationship flips. Data from the Tromsø study found that higher total testosterone levels were independently associated with higher HDL cholesterol, and men with an unfavorable lipid profile (low HDL combined with high triglycerides) had significantly lower testosterone levels, even after adjusting for age and BMI.

This becomes especially relevant as men age. Testosterone levels naturally decline by about 1 to 2 percent per year after age 30. As testosterone drops, HDL tends to drop with it, and triglycerides often rise. This partly explains why cholesterol problems tend to worsen in middle-aged and older men. It also means that conditions causing abnormally low testosterone, such as hypogonadism or certain chronic illnesses, can contribute to an unfavorable cholesterol profile independent of diet or exercise habits.

Genetic Causes

Some men have high cholesterol despite eating well and exercising regularly. The most common genetic explanation is familial hypercholesterolemia, which affects roughly 1 in 200 to 1 in 250 people worldwide. It’s caused by mutations in genes that control how the body clears LDL from the blood. The most frequent culprit is a mutation in the LDLR gene, which provides instructions for building LDL receptors on liver cells. When these receptors are faulty or reduced in number, LDL particles stay in the bloodstream much longer than they should.

Less commonly, mutations in the APOB or PCSK9 genes cause the same result through different pathways. APOB mutations alter the structure of LDL particles so they can’t bind properly to receptors, while PCSK9 mutations cause the body to break down LDL receptors faster than normal. People with familial hypercholesterolemia often have total cholesterol levels above 300 mg/dL from a young age and may develop heart disease decades earlier than expected. If your cholesterol has been high since your 20s or you have a family history of early heart attacks, a genetic cause is worth investigating.

Smoking and HDL Cholesterol

Smoking lowers HDL cholesterol by an average of about 4 mg/dL in men, based on findings from the Framingham Heart Study. That may sound small, but HDL is already the hardest number to move, and every point matters. HDL particles help remove cholesterol from artery walls and transport it back to the liver for disposal. When HDL drops below 40 mg/dL, a man’s cardiovascular risk rises sharply.

Chemicals in cigarette smoke also damage the lining of blood vessels, making it easier for LDL cholesterol to penetrate artery walls and form plaques. Quitting smoking can restore HDL levels within weeks to months, and the cardiovascular benefits continue accumulating for years afterward.

Alcohol and Triglycerides

Even moderate alcohol consumption raises triglyceride levels. The liver prioritizes metabolizing alcohol over its normal tasks, including processing fats, so triglycerides build up when you drink regularly. For men, moderate drinking is defined as up to two standard drinks per day. Heavy drinking, five or more drinks per day, can cause dramatic triglyceride spikes. High triglycerides don’t just show up on a lab report. They promote the formation of small, dense LDL particles, which are particularly good at burrowing into artery walls and causing damage.

Medications That Raise Cholesterol

Several commonly prescribed medications can worsen cholesterol levels as a side effect. Anabolic steroids, sometimes used for bodybuilding or prescribed for low testosterone, can cause a dramatic increase in LDL while simultaneously lowering HDL. Prednisone and other corticosteroids, often used for inflammatory conditions, can quickly and significantly raise LDL. Beta-blockers, prescribed for high blood pressure and heart conditions, may also nudge LDL up or HDL down. Cyclosporine, an immune-suppressing drug, and amiodarone, a heart rhythm medication, are additional culprits.

If you’ve noticed your cholesterol worsen after starting a new medication, that connection is worth raising with your doctor. In many cases, the benefit of the medication outweighs the cholesterol impact, but adjustments or additional treatment may be possible.

Why It Matters Earlier Than You Think

High cholesterol causes no symptoms on its own, which is why screening matters. The CDC recommends that most healthy adults have their cholesterol checked every four to six years, with initial screening ideally happening between ages 17 and 21. Men with risk factors like a family history of early heart disease, obesity, or diabetes should be screened more frequently.

One consequence of high cholesterol that’s specific to men is its link to erectile dysfunction. Because the arteries supplying the penis are smaller than those supplying the heart, plaque buildup from high LDL often affects sexual function years before it causes chest pain or other heart symptoms. Erectile dysfunction in a man under 50 with no obvious psychological cause can be an early warning sign that cholesterol-driven artery damage is already underway.
