Internal bleeding in dogs most often results from trauma, ruptured tumors, poisoning, or disorders that prevent blood from clotting properly. Unlike an external wound you can see and apply pressure to, internal hemorrhage happens inside the chest, abdomen, or digestive tract, making it invisible and dangerous. Recognizing the cause matters because some forms develop over days while others become life-threatening within hours.
Ruptured Tumors, Especially on the Spleen
Splenic tumors are one of the most common causes of sudden internal bleeding in middle-aged and older dogs. The spleen is a blood-rich organ tucked behind the stomach, and masses that grow on it can rupture without warning, flooding the abdomen with blood. In a study of 182 dogs that had their spleens surgically removed, 32.4% were diagnosed with hemangiosarcoma, an aggressive cancer of blood vessel walls. Malignant masses tend to have disorganized internal structure, which makes them more prone to spontaneous rupture than benign growths.
Breeds like German Shepherds, Golden Retrievers, and Labrador Retrievers face higher risk. A dog with a ruptured splenic tumor often collapses suddenly, breathes rapidly, and has a visibly distended belly. Some dogs experience smaller “warning bleeds” that resolve on their own before a catastrophic rupture occurs, so unexplained episodes of weakness or collapse in an older dog should always be investigated.
Rat Poison and Other Toxins
Anticoagulant rodenticides (rat and mouse poisons) are a leading toxin-related cause of internal bleeding in dogs. These products work by blocking the body’s ability to recycle vitamin K, a nutrient essential for producing four key clotting factors. Without those factors, blood loses its ability to form stable clots.
The dangerous part is the delay. Clotting tests don’t become abnormal until 2 to 5 days after ingestion, and visible signs of bleeding typically appear 3 to 7 days later. By that point, a dog may be bleeding into the chest cavity, abdomen, urinary tract, or under the skin. If you know or suspect your dog ate rodent bait, that window before symptoms appear is critical for treatment. Dogs treated early with vitamin K supplementation generally recover fully, while dogs that arrive already bleeding face a much harder road.
Medications That Damage the Gut Lining
Gastrointestinal hemorrhage is a major form of internal blood loss in dogs, and the most common trigger is ulceration of the stomach or intestinal lining. Nonsteroidal anti-inflammatory drugs (NSAIDs) and liver disease are the two most frequently reported risk factors for these ulcers.
NSAIDs reduce a protective layer of mucus in the stomach. When that barrier breaks down, stomach acid eats into the tissue and creates bleeding ulcers. This can happen with prescription veterinary NSAIDs given at the correct dose, but the risk climbs sharply when dogs accidentally swallow human medications like ibuprofen or naproxen. Corticosteroids (steroids used for allergies and inflammation) also increase ulcer risk, and the combination of an NSAID and a steroid together is particularly dangerous. Signs of GI bleeding include vomiting material that looks like coffee grounds, black tarry stool, or sudden lethargy and loss of appetite.
Trauma and Accidents
Being hit by a car, falling from a height, or suffering a severe blow to the abdomen can rupture organs, tear blood vessels, or fracture bones in ways that cause significant internal hemorrhage. The liver and spleen are especially vulnerable because they’re packed with blood vessels and sit in the abdomen without much bony protection. Rib fractures can puncture a lung and cause bleeding into the chest cavity.
Traumatic internal bleeding can be deceptive. A dog may walk away from an accident looking relatively normal, then deteriorate hours later as blood accumulates internally. Gums that shift from pink to pale or white are one of the most reliable early indicators that blood volume is dropping.
Immune System Attacks on Platelets
Immune-mediated thrombocytopenia (ITP) occurs when a dog’s immune system mistakenly destroys its own platelets, the tiny cell fragments responsible for forming clots. The body produces antibodies that tag platelets for destruction, and immune cells in the spleen and liver clear them from the bloodstream faster than the bone marrow can replace them. In some cases, the immune attack also targets the cells in bone marrow that produce platelets, further reducing supply.
A normal platelet count keeps bleeding in check after everyday bumps and micro-injuries. When the count drops below 100,000 per microliter, clotting becomes unreliable. Below 20,000, spontaneous bleeding can occur almost anywhere: into the skin (appearing as tiny red dots or bruises), the digestive tract, the urinary system, or the abdominal cavity. ITP can strike dogs of any breed but is more common in middle-aged females. Cocker Spaniels, Old English Sheepdogs, and Poodles appear to be overrepresented.
Inherited Clotting Disorders
Von Willebrand disease is the most common inherited bleeding disorder in dogs. It involves a deficiency in a protein that helps platelets stick together and form the initial plug at a wound site. There are three types, each varying in severity.
- Type 1 is the mildest and most widespread form, affecting Doberman Pinschers, German Shepherds, Golden Retrievers, Poodles, Shetland Sheepdogs, Pembroke Welsh Corgis, and many other breeds. Dogs with Type 1 have reduced levels of the clotting protein but still produce some.
- Type 2 involves a structurally abnormal version of the protein and is seen primarily in German Shorthaired Pointers and German Wirehaired Pointers.
- Type 3 is the most severe, with virtually no functional protein at all. Chesapeake Bay Retrievers, Scottish Terriers, and Shetland Sheepdogs are among the breeds with a familial pattern, though it also appears sporadically in Labrador Retrievers, Border Collies, and mixed breeds.
Dogs with mild forms may go undiagnosed until a surgery or injury triggers unexpectedly heavy bleeding. Dogs with Type 3 can experience spontaneous hemorrhage into joints, the GI tract, or body cavities.
Infections and Parasites
Several infectious diseases cause internal bleeding by damaging tissue directly or disrupting clotting. Parvovirus destroys the lining of the intestines in puppies, leading to severe bloody diarrhea and potentially fatal GI hemorrhage. Intestinal parasites like hookworms and whipworms latch onto the gut wall and feed on blood; heavy infestations, especially in young or debilitated dogs, can cause chronic blood loss significant enough to produce anemia.
Tick-borne infections such as ehrlichiosis and anaplasmosis can suppress platelet production or trigger immune-mediated platelet destruction, creating bleeding risks similar to ITP. Fungal infections, though less common, can invade the GI tract and cause ulceration and hemorrhage.
How to Recognize Internal Bleeding
Because the bleeding is hidden, you have to read indirect signs. Healthy dog gums are a steady shade of pink. Gums that turn pale, white, or grayish suggest blood loss or poor circulation and warrant emergency veterinary care. A belly that seems swollen or tense, especially if it develops quickly, may indicate blood pooling in the abdomen.
Other warning signs include sudden weakness or collapse, rapid breathing at rest, a racing heart rate you can feel by pressing gently on the chest, and cool extremities (ears, paws). Dogs bleeding into the GI tract may vomit blood or pass dark, tarry stool. Dogs bleeding into the chest may struggle to breathe or refuse to lie down.
Veterinarians can confirm internal bleeding quickly using a focused abdominal ultrasound that checks five specific areas of the belly for free fluid. This scan takes only minutes and can detect even small amounts of blood in the abdominal cavity. Blood work measuring clotting times, platelet counts, and markers of clot breakdown (like D-dimer levels) helps identify the underlying cause and guide treatment decisions.
Why Speed Matters
Internal bleeding that goes unchecked can spiral into a condition called disseminated intravascular coagulation, or DIC. In DIC, the body forms tiny clots throughout the bloodstream, using up clotting factors and platelets so rapidly that it paradoxically loses the ability to clot at all. The result is uncontrolled bleeding from multiple sites simultaneously. DIC is diagnosed through a panel of blood tests showing prolonged clotting times, low levels of clot-forming proteins, and elevated D-dimer (a fragment released when clots break down). Once DIC sets in, survival rates drop significantly, which is why identifying and treating the original source of bleeding early is so critical.
