Lower abdominal pain in males can stem from digestive problems, urinary issues, reproductive organ conditions, or muscle and tissue injuries. Some causes are minor and resolve on their own, while others require urgent medical attention. The location of the pain, how suddenly it started, and what makes it worse all help narrow down the source.
Appendicitis
Appendicitis is one of the most well-known causes of lower abdominal pain and one of the most time-sensitive. The classic pattern starts with vague pain around the belly button that migrates over several hours to the lower right side of the abdomen. The pain worsens with coughing, walking, or any jarring movement. Nausea, vomiting, loss of appetite, and fever often accompany it.
Not everyone follows the textbook pattern. Pain can start anywhere in the abdomen before settling in the lower right. If the appendix ruptures, the pain may briefly ease before spreading across the entire abdomen, which signals a more dangerous situation. Appendicitis is treated with surgery, and outcomes are excellent when caught early.
Kidney Stones
Kidney stones produce some of the most intense pain the body can generate. The pain typically starts in the lower back or side and radiates downward into the lower abdomen, sometimes extending into the groin or even the scrotum. It comes in waves, building to a peak and then easing before returning. This wave-like pattern is a hallmark of stones moving through the urinary tract.
Other signs include blood in the urine, pain during urination, frequent urges to urinate, nausea, and vomiting. Cloudy or foul-smelling urine and fever can indicate an infection alongside the stone. Smaller stones often pass on their own with hydration and pain management, while larger ones may need medical intervention to break them up or remove them.
Inguinal Hernia
Inguinal hernias are far more common in males than females. They occur when tissue, usually part of the intestine, pushes through a weak spot in the abdominal wall near the groin. The telltale sign is a visible or palpable bulge in the groin or scrotum, along with discomfort, heaviness, or a burning sensation in the area.
Symptoms get worse when you strain, lift something heavy, cough, or stand for extended periods. They tend to improve when you rest or lie down. A doctor can often diagnose an inguinal hernia during a physical exam by asking you to stand, cough, or strain while checking for the bulge. Ultrasound, CT, or MRI may be used when the diagnosis isn’t clear. Most inguinal hernias eventually require surgical repair, particularly if they’re growing or causing significant pain.
Sports Hernia
A sports hernia is different from a traditional hernia. It’s a strain or tear of soft tissue (muscle, tendon, or ligament) in the lower abdomen or groin, typically where the abdominal muscles and inner thigh muscles attach to the pubic bone. There’s usually no visible bulge, which is the key distinction from an inguinal hernia. The medical term is athletic pubalgia.
This injury is common in sports that involve sudden changes of direction, twisting, or kicking. Pain develops gradually with activity and eases with rest, then returns when you resume the activity. Over time, an untreated sports hernia can weaken the area enough to lead to a traditional inguinal hernia.
Prostatitis and Chronic Pelvic Pain
The prostate gland sits just below the bladder, so inflammation here can produce lower abdominal pain, pelvic discomfort, and urinary problems. There are two main types worth knowing about.
Acute bacterial prostatitis is an infection that comes on suddenly with fever, chills, painful urination, frequent urination, and difficulty urinating. It requires prompt antibiotic treatment.
Chronic pelvic pain syndrome is far more common, affecting roughly 1 in 3 men at some point. Despite the name, it’s not an infection. It causes persistent pain in the pelvis, the area between the scrotum and rectum, and the genitals. This pain can last months or even years, often fluctuating in intensity. Because no bacteria are involved, treatment focuses on managing symptoms through a combination of approaches rather than antibiotics.
Testicular Torsion
Testicular torsion happens when the spermatic cord twists, cutting off blood supply to the testicle. It causes sudden, severe pain on one side that can radiate into the lower abdomen. This is a surgical emergency because the testicle can be permanently damaged within hours.
Torsion is more common in adolescents and young men, and it typically occurs without any preceding infection or inflammation. The sudden onset is the critical clue. If you experience sharp, one-sided testicular pain that comes on abruptly, especially if there’s swelling or nausea, get to an emergency room immediately. Doctors prioritize ruling out torsion before considering less urgent diagnoses.
Epididymitis
Epididymitis, inflammation of the coiled tube behind the testicle, can mimic torsion but typically develops more gradually. It causes one-sided testicular pain and tenderness, swelling, and sometimes a fluid buildup around the testicle. The spermatic cord is usually tender and swollen. In sexually active younger men, it’s often linked to sexually transmitted infections. In older men, urinary tract infections are a more common trigger. The key difference from torsion is the slower onset, though any sudden testicular pain warrants urgent evaluation.
Diverticulitis
If the pain is concentrated in the lower left abdomen, diverticulitis is a leading suspect, especially in men over 40. Small pouches called diverticula form in the wall of the colon, most commonly in the lower left section. When these pouches become inflamed or infected, the result is diverticulitis.
The pain is usually steady and localized to the lower left side, often accompanied by fever, nausea, and changes in bowel habits. Mild cases can be managed with rest and sometimes antibiotics, while severe or complicated cases may require hospitalization. A high-fiber diet, adequate water intake, and regular physical activity can reduce the risk of developing diverticula in the first place.
Irritable Bowel Syndrome
IBS is a chronic condition that causes recurring abdominal pain tied to changes in bowel habits, whether that’s diarrhea, constipation, or alternating between the two. The pain often occurs in the lower abdomen and typically improves after a bowel movement. Bloating and gas are common companions.
Men and women experience IBS symptoms similarly, though research from UCLA suggests the brain processes IBS-related discomfort differently between sexes. In men, the brain’s sensory processing areas show more activation, meaning the pain itself tends to dominate the experience. IBS is managed through dietary changes, stress reduction, and sometimes medication to control specific symptoms.
Urinary Tract Infections
While less common in younger men than in women, urinary tract infections can cause lower abdominal pressure and pain, along with burning during urination, frequent urges to urinate, and cloudy or strong-smelling urine. UTIs in men become more common with age, particularly when prostate enlargement slows urine flow. Infections that travel up to the kidneys cause more severe symptoms, including back pain, fever, and chills.
When Pain Signals an Emergency
Certain features of lower abdominal pain indicate you need immediate medical care. Sudden, severe pain that doesn’t ease within 30 minutes is a red flag, as it can signal serious conditions like a ruptured organ, intestinal blockage, or compromised blood supply. Continuous severe pain paired with persistent vomiting, a rigid or extremely tender abdomen, fever, or bloody stool all point toward conditions that may require surgical intervention. Sudden one-sided testicular pain in particular should never be waited out, given the narrow window to save the testicle in torsion cases.
Pain that builds gradually over days and is accompanied by mild discomfort is less likely to be an emergency, but persistent or worsening symptoms still warrant evaluation. The combination of pain location, how it started, what makes it worse, and any accompanying symptoms gives doctors the information they need to identify the cause efficiently.
