What Causes Protein in Urine During Pregnancy?

Small amounts of protein in urine are normal during pregnancy, but levels above 300 mg in a 24-hour collection signal a problem that needs attention. The most common and most serious cause is preeclampsia, a condition unique to pregnancy that involves high blood pressure and kidney stress. Other causes include urinary tract infections, pre-existing kidney disease, and the normal physiological changes pregnancy puts on your body.

Why Pregnancy Increases Protein in Urine

During pregnancy, your blood volume increases by nearly 50%, and your kidneys filter significantly more blood than usual. This extra workload means slightly more protein slips through the kidneys’ filtering system than it would outside of pregnancy. A small amount of protein on a urine test, especially in a single sample, can be entirely normal.

The concern starts when protein levels cross certain thresholds or appear alongside other symptoms like high blood pressure. That combination points toward conditions where the kidneys are being damaged, not just working harder.

Preeclampsia: The Most Common Serious Cause

Preeclampsia is the condition most doctors are screening for when they check your urine at prenatal visits. It develops after the 20th week of pregnancy and involves high blood pressure along with signs of organ damage, most often to the kidneys. When the small blood vessels in your kidneys become inflamed and damaged, protein that normally stays in your blood leaks into your urine.

The diagnostic threshold is 300 mg of protein in a 24-hour urine collection, or a protein-to-creatinine ratio greater than 0.3 in a single urine sample. Preeclampsia affects roughly 5 to 8 percent of pregnancies and can progress quickly. Beyond protein in urine, warning signs include persistent headaches, vision changes (blurriness or seeing spots), upper abdominal pain especially on the right side, sudden swelling of the face or hands, and rapid weight gain from fluid retention.

In severe cases, preeclampsia can progress to HELLP syndrome, a dangerous complication involving breakdown of red blood cells, elevated liver enzymes, and low platelet counts. About 80% of HELLP cases involve high blood pressure or proteinuria, but the remaining 20% do not, which makes it unpredictable. HELLP results from widespread damage to the lining of small blood vessels, causing clotting problems and liver injury on top of the kidney issues seen in preeclampsia.

Pre-Existing Kidney Disease and Diabetes

If you had kidney disease, diabetes, or lupus before becoming pregnant, you may already have some degree of protein in your urine. The challenge for your care team is distinguishing a normal pregnancy-related increase from superimposed preeclampsia, which is preeclampsia developing on top of an existing condition.

A key marker is how much your protein levels change. Research tracking pregnancies in women with chronic kidney disease found that a doubling or greater increase in protein levels from the first trimester strongly predicted superimposed preeclampsia. In that study, 70% of pregnancies with a greater-than-doubling increase were clinically diagnosed with superimposed preeclampsia. Only 9% of pregnancies showed that kind of spike without preeclampsia being the cause. The exceptions were women with diabetic kidney disease or lupus-related kidney disease, where protein levels sometimes doubled on their own due to the underlying condition flaring during pregnancy.

This is why baseline protein measurements early in pregnancy matter so much if you have a known kidney condition. Without that first-trimester reference point, it becomes harder to tell what’s new and what was already there.

Urinary Tract Infections

UTIs are common during pregnancy and can cause protein to show up in urine tests. The infection triggers inflammation in the urinary tract, and the body’s immune response releases proteins that end up in the sample. UTIs during pregnancy also carry more risk than usual because untreated infections can spread to the kidneys (pyelonephritis), which is a serious complication. If a urine test shows protein along with bacteria, your provider will typically treat the infection first and then recheck protein levels to see if they return to normal.

What You Might Notice

Mild proteinuria usually causes no symptoms at all, which is why routine urine screening at prenatal visits is so important. You won’t feel protein leaking into your urine.

When protein loss becomes significant, the most recognizable sign is foamy or bubbly urine. This happens because protein acts like a surfactant, creating persistent bubbles when urine hits the toilet water. As protein levels in your blood drop (because too much is leaving through your kidneys), fluid shifts out of your blood vessels and into surrounding tissue. This causes visible swelling in your feet, ankles, face, and around your eyes, especially in the morning. You may also notice rapid weight gain over just a few days, which reflects fluid retention rather than actual body mass changes. Shortness of breath, fatigue, nausea, and nighttime muscle cramping can also develop when protein loss is substantial.

How Protein in Urine Is Measured

The first test you’ll encounter is a urine dipstick, the quick test done at nearly every prenatal appointment. A small strip is dipped in your urine sample and changes color based on protein concentration. It’s fast and convenient, but it has real accuracy limitations.

Research comparing dipstick results to 24-hour urine collections found that one in five women with a completely negative dipstick reading actually had significant proteinuria when measured more precisely. Among women with trace-level readings, one in three had significant proteinuria. On the other end, a high dipstick reading of 3+ or 4+ correctly predicted severe proteinuria only 38% of the time. In short, the dipstick is a screening tool, not a diagnosis.

When the dipstick raises concern, your provider will order a more accurate test. The two options are a 24-hour urine collection, where you save all urine produced over a full day for lab analysis, or a spot protein-to-creatinine ratio from a single urine sample. Both are considered acceptable for diagnosis. The 24-hour collection is more cumbersome but gives a complete picture. The spot ratio is faster and correlates well with the 24-hour result: a ratio above 0.3 corresponds to the 300 mg threshold used to diagnose preeclampsia.

What Happens After an Abnormal Result

A single elevated protein reading doesn’t automatically mean something is wrong. Dehydration, physical exertion, stress, and even the way you collected the sample can temporarily raise protein levels. Your provider will typically confirm the finding with a more precise test before drawing conclusions.

If proteinuria is confirmed, the next step depends on whether high blood pressure or other symptoms are present. Protein in urine combined with blood pressure at or above 140/90 after 20 weeks points strongly toward preeclampsia, and monitoring becomes much more intensive. You can expect more frequent prenatal visits, blood work to check liver and kidney function, and fetal monitoring to assess the baby’s well-being. In mild cases, careful observation and early delivery planning (often around 37 weeks) are the approach. Severe preeclampsia or HELLP syndrome may require earlier delivery, sometimes significantly earlier, because delivery is the only way to resolve the condition.

If protein appears without high blood pressure, your provider will look for other explanations like a UTI or a pre-existing condition, and will continue monitoring closely because isolated proteinuria sometimes precedes the development of preeclampsia by days or weeks.
