Strep throat is caused by a specific type of bacteria called Group A Streptococcus, which naturally lives in the nose and throat. Unlike most sore throats, which are viral, strep throat is a bacterial infection that spreads through respiratory droplets, direct contact with saliva or nasal secretions, and contact with open sores. It typically takes 2 to 5 days after exposure before symptoms appear.
The Bacteria Behind Strep Throat
Group A Streptococcus (sometimes called Group A strep or GAS) causes a wide range of infections, from mild conditions like strep throat to more serious invasive diseases. These bacteria are well-equipped to establish infection in your throat. They produce surface proteins that help them latch onto the cells lining your throat and tonsils, essentially anchoring themselves in place. Once attached, the bacteria multiply and trigger the immune response that produces the painful inflammation you feel.
What makes Group A strep particularly effective is its ability to dodge your immune system. The bacteria can break apart structures your immune cells deploy to trap them, and they produce proteins that block one of your body’s key defense mechanisms for killing bacteria. This is why your immune system often needs help from antibiotics to fully clear the infection.
How Strep Throat Spreads
The bacteria travel between people in three main ways: through tiny respiratory droplets released when an infected person coughs or sneezes, through direct contact with their saliva or nasal secretions (sharing drinks, kissing), and through contact with infected wounds. This is why strep throat spreads so easily in schools, households, and other close-contact settings.
Not everyone carrying the bacteria gets sick. During a school outbreak study published in The Lancet Microbe, researchers found that 10% to 27% of children carried the outbreak strain in their throats without showing any symptoms during a given week. Some of these asymptomatic carriers were still shedding bacteria when they coughed, meaning they could spread the infection without knowing they had it. In one strain, more than a third of symptom-free carriers were actively releasing bacteria into the air.
Why It Feels Different From a Viral Sore Throat
Most sore throats are caused by viruses, and telling the two apart matters because only bacterial strep throat benefits from antibiotics. Strep throat typically causes a red, swollen throat and tonsils, sometimes with white patches of pus. You may notice swollen lymph nodes at the front of your neck and tiny red spots on the roof of your mouth.
One of the most reliable clues is what’s absent. Strep throat usually does not come with a cough, runny nose, hoarseness, mouth ulcers, or pink eye. If you have those symptoms, a virus is the more likely culprit. When a sore throat arrives suddenly with fever, pain when swallowing, and swollen tonsils but no cold symptoms, strep is a strong possibility.
Even experienced clinicians cannot reliably distinguish viral from bacterial pharyngitis by examination alone. Confirmation requires either a rapid antigen detection test (the quick swab you get in the office, with results in minutes) or a throat culture, which takes a day or two but is more accurate.
Who Gets Strep Throat and When
Strep throat is most common in children between ages 5 and 15, though adults get it too. It peaks during late fall, winter, and early spring, when people spend more time indoors in close proximity. Households with school-age children see higher rates because kids bring the bacteria home, and the close contact of family life makes transmission easy.
Your risk goes up in any environment where people are in close quarters for extended periods: classrooms, dormitories, military barracks, and daycare centers. Having a recent exposure to someone with confirmed strep throat is one of the strongest predictors.
What Happens if Strep Throat Goes Untreated
Most people recover from strep throat even without treatment, but leaving it untreated carries real risks. The infection can trigger inflammatory complications that affect parts of the body far from the throat. Rheumatic fever is the most serious of these. It causes inflammation of the heart, joints, nervous system, and skin, and repeated episodes can permanently damage heart valves. This complication is the primary reason antibiotics are recommended for confirmed strep.
Other potential complications include kidney inflammation (which can cause dark or reduced urine and swelling), scarlet fever (marked by a widespread sandpaper-like rash), and reactive arthritis affecting the joints. These complications develop days to weeks after the initial throat infection when the immune system’s response to the bacteria inadvertently damages the body’s own tissues.
How Strep Throat Is Treated
The standard treatment is a course of antibiotics, and the good news is that Group A strep bacteria remain fully susceptible to the first-line options. Unlike many other bacteria, they have not developed resistance to the most commonly prescribed treatments for strep throat. This means treatment is straightforward and effective for the vast majority of people.
Resistance has emerged for some backup antibiotics. Roughly 1 in 3 invasive Group A strep infections now involve bacteria resistant to certain alternative antibiotics, but this primarily affects people with severe invasive infections rather than typical strep throat cases.
With antibiotics, most people start feeling better within a day or two. You’re generally no longer contagious after 12 to 24 hours of treatment, which is why many schools and workplaces use that as the benchmark for returning. Finishing the full course matters even after symptoms improve, because stopping early increases the chance of complications and contributes to antibiotic resistance over time.
