Swollen ankles result from fluid building up in the tissues around your ankle joint, and the causes range from sitting too long at your desk to serious conditions like heart failure or blood clots. The medical term is peripheral edema, and it affects a surprisingly large portion of adults. In most cases, gravity is the common thread: fluid naturally pools in the lowest parts of your body, and anything that disrupts your circulation or changes the pressure balance in your blood vessels makes that pooling worse.
How Fluid Ends Up in Your Ankles
Your blood vessels constantly exchange fluid with the surrounding tissues. Two opposing forces control this exchange: the pressure inside your capillaries pushing fluid out, and proteins in your blood (especially one called albumin) pulling fluid back in. When these forces fall out of balance, fluid leaks into the tissue faster than it can be reabsorbed or drained away by your lymphatic system.
This imbalance can happen in several ways. High blood pressure inside the veins of your legs forces more fluid out. Low protein levels in your blood reduce the pulling force that keeps fluid inside vessels. Damaged or leaky blood vessel walls let fluid and protein escape into surrounding tissue. And when your lymphatic drainage system is overwhelmed or blocked, fluid simply has nowhere to go. Every cause of swollen ankles traces back to one or more of these mechanisms.
Prolonged Sitting and Standing
The most common reason for ankle swelling is simply staying in one position too long. Your leg muscles act as pumps that squeeze blood back up toward your heart with every step. When you sit or stand without moving, those pumps shut off, and fluid accumulates in your lower legs. Research measuring fluid shifts in the legs found that uninterrupted sitting produced the greatest increase in lower leg swelling, with measurable fluid accumulation starting within just 10 to 20 minutes.
Standing still is also a problem, though slightly less so than sitting. The key factor is movement: people who transition between sitting and standing throughout the day accumulate significantly less fluid than those who stay in either position. Long flights, desk jobs, and extended driving are classic triggers. The good news is that this type of swelling resolves relatively quickly. Lying down with your legs elevated for about 10 minutes is typically enough to reset your fluid balance.
Chronic Venous Insufficiency
Your leg veins contain one-way valves that keep blood moving upward against gravity. When these valves become damaged, blood flows backward and pools in your lower legs, a condition called chronic venous insufficiency (CVI). An estimated 10% to 35% of adults in the United States have CVI, with women affected more often than men.
Over time, the increased pressure from pooling blood forces fluid out of your capillaries and into the surrounding tissue. Without treatment, this pressure can build high enough to burst your smallest blood vessels, leading to skin discoloration and eventually open sores. Severe, long-standing swelling can cause scar tissue to form in your lower leg tissues, which traps fluid and makes the swelling progressively harder to reverse. CVI tends to worsen gradually, so early swelling that goes away overnight can eventually become swelling that persists around the clock.
Heart, Kidney, and Liver Disease
Swollen ankles that appear in both legs symmetrically can signal a problem with one of your major organ systems.
In heart failure, the heart can’t pump blood efficiently, so blood backs up in the veins. This raises pressure in the leg veins and forces fluid into the tissues. Your kidneys respond to the sluggish circulation by retaining extra sodium and water, which compounds the problem. The swelling is typically worst in the evening and improves overnight when you’re lying flat.
Kidney disease causes ankle swelling through a different route. Damaged kidneys leak protein into the urine, depleting albumin from your blood. When albumin drops below a critical threshold (roughly 2 grams per deciliter), your blood loses enough pulling force that fluid seeps into tissues throughout your body, with gravity concentrating it at your ankles.
Liver cirrhosis hits from both directions. A scarred liver can’t produce enough albumin, so the pulling force drops. At the same time, blood flow through the liver is obstructed, raising pressure in the veins that drain the legs and abdomen. The combination of low protein and high venous pressure makes ankle and abdominal swelling a hallmark of advanced liver disease.
Medications That Cause Swelling
Several common drug classes cause ankle swelling as a side effect, and the mechanism varies by medication. Calcium channel blockers, widely prescribed for high blood pressure, are among the most frequent culprits. Drugs like amlodipine, nifedipine, and felodipine selectively relax the tiny arteries feeding your capillaries without relaxing the veins on the other side. This mismatch raises pressure inside the capillary bed and pushes fluid into the tissues. Importantly, this type of swelling is not caused by excess fluid in your bloodstream, which is why water pills (diuretics) don’t help much.
Other medications that commonly cause dose-related ankle swelling include:
- NSAIDs like ibuprofen and naproxen, which cause your kidneys to retain sodium and water
- Steroid hormones, including corticosteroids and estrogen-based therapies
- Diabetes medications in the thiazolidinedione class
- Blood pressure drugs such as hydralazine and minoxidil
If your ankles started swelling after beginning a new medication, that connection is worth raising with whoever prescribed it. The swelling is usually dose-dependent and reversible.
Pregnancy-Related Swelling
Some degree of ankle swelling is normal during pregnancy, especially in the third trimester. The growing uterus compresses pelvic veins, slowing blood return from the legs, while hormonal changes cause your body to retain extra fluid. This swelling tends to be gradual and affects both legs.
What is not normal is a sudden increase in swelling, particularly if it appears in your face and hands. Rapid onset of puffiness can be a warning sign of preeclampsia, a serious pregnancy complication involving dangerously high blood pressure. Preeclampsia requires prompt medical attention, so sudden swelling during pregnancy is always worth an immediate call to your provider.
Blood Clots and One-Sided Swelling
Swelling in just one ankle or leg is a different situation from bilateral swelling, and it warrants more urgency. A deep vein thrombosis (DVT), or blood clot in a leg vein, blocks blood from draining and causes rapid swelling, usually accompanied by warmth, redness, or a deep aching pain in the calf or thigh.
The danger with DVT isn’t just the leg swelling. A clot can break free and travel to the lungs, causing a pulmonary embolism. Warning signs that this has happened include sudden shortness of breath, chest tightness or pain, a racing heartbeat, coughing up blood, or feeling faint and clammy. These symptoms require emergency treatment.
Pitting vs. Non-Pitting Edema
You can learn something about the cause of your swelling with a simple test. Press your thumb firmly into the swollen area for a few seconds and release. If an indentation lingers, that’s pitting edema, which means the fluid in your tissues has a low protein content. Pitting edema points toward causes like heart failure, venous insufficiency, DVT, kidney disease, or medication side effects.
If the skin springs right back and doesn’t hold an indentation, that’s non-pitting edema. This pattern is characteristic of lymphedema, where the lymphatic drainage system itself is damaged or blocked, and the trapped fluid is protein-rich. Non-pitting edema can also occur with lipedema, a condition involving abnormal fat distribution in the legs that is often mistaken for simple weight gain.
Compression Stockings and Self-Management
For swelling caused by prolonged sitting or standing, venous insufficiency, or occupational demands, compression stockings are one of the most effective tools. They work by applying graduated pressure to your lower legs, helping push fluid back into circulation.
Light compression in the 10 to 15 mmHg range is effective for preventing everyday occupational swelling. For people who already have noticeable swelling, stockings in the 15 to 20 mmHg range produce significant improvement within a day or two. Higher pressure (20 to 30 mmHg) provides additional benefit, particularly for people who sit most of the day. Calf-length stockings that reach just below the knee are sufficient for most people. Beyond compression, the simplest interventions are also the most effective: taking walking breaks every 20 to 30 minutes, elevating your legs when resting, and reducing salt intake to limit fluid retention.
