Bacterial urinary tract infections in cats are less common than most owners expect. More than 95% of young cats showing urinary symptoms actually have sterile urine, meaning no bacteria are involved at all. True bacterial UTIs are most frequent in cats over 10 years old, particularly females, and are often tied to underlying health conditions that make the bladder more vulnerable to infection.
Most Urinary Problems in Cats Aren’t Infections
This is the single most important thing to understand about feline urinary issues. When a cat strains in the litter box, urinates in unusual places, or produces bloody urine, the instinct is to assume infection. In reality, roughly two-thirds of cats with lower urinary tract symptoms have a condition called feline idiopathic cystitis (FIC), a painful inflammation of the bladder with no bacterial cause. Large studies at the University of Minnesota and Ohio State University found FIC accounted for 54% to 79% of all feline lower urinary tract cases.
FIC is driven by stress rather than germs. Sensory neurons trigger bladder inflammation not because of infection or injury, but in response to the cat’s emotional state. This distinction matters because antibiotics won’t help a cat with FIC, and misdiagnosis can delay the right treatment. A urine culture, where a lab actually grows bacteria from a urine sample, is the only way to confirm a true bacterial infection.
Bacteria Behind True Feline UTIs
When cats do develop genuine bacterial infections, a handful of organisms are responsible for the vast majority of cases. E. coli, the same gut bacterium that causes UTIs in humans, is the leading culprit at 42.3% of all feline urinary isolates. It typically reaches the bladder by traveling up the urethra from the surrounding area.
The remaining cases are split among several bacterial groups: Streptococcus species (19.3%), Staphylococcus species (15.6%), Enterococcus species (6.6%), and Micrococcaceae (5.8%). These are all opportunistic bacteria, meaning they normally live harmlessly on the skin or in the gut but can cause infection when the cat’s natural defenses are weakened.
Age, Sex, and Anatomy
A cat’s age is one of the strongest predictors of whether urinary symptoms involve actual bacteria. Young and middle-aged cats almost never have bacterial UTIs. The risk climbs significantly after age 10, when the immune system becomes less efficient and chronic diseases begin to accumulate.
Female cats are more prone to bacterial UTIs than males, for the same anatomical reason as in humans: a shorter, wider urethra gives bacteria an easier path to the bladder. Male cats face a different urinary risk. Their longer, narrower urethra makes them more susceptible to urethral obstruction, a potentially life-threatening blockage, but they’re actually less likely to develop straightforward bacterial infections.
Underlying Diseases That Open the Door
Healthy cats have strong natural defenses against bladder infections. Concentrated urine, the bladder’s protective lining, and regular flushing during urination all help keep bacteria from gaining a foothold. Chronic diseases can erode these defenses.
Chronic kidney disease (CKD) is one of the biggest risk factors. Cats with CKD produce dilute urine that’s less hostile to bacteria, and between 15% and 30% of cats with the condition develop secondary UTIs. Older female cats with CKD are especially predisposed. Diabetes is another common contributor, since elevated sugar in the urine creates a more hospitable environment for bacterial growth. Hyperthyroidism, which increases thirst and urination volume, can also play a role by altering the bladder’s normal flushing patterns.
Urinary Crystals and Stones
Mineral crystals and stones in the urinary tract can both cause urinary symptoms on their own and set the stage for bacterial infection. Crystals irritate the bladder lining, creating micro-damage where bacteria can take hold. Stones can trap bacteria in crevices that antibiotics struggle to reach, leading to persistent or recurring infections.
The two most common stone types in cats form under different conditions. Struvite stones (made of magnesium, ammonium, and phosphate) develop when urine is alkaline and overly concentrated. Calcium oxalate stones form in acidic urine. Both types are more likely when a cat isn’t drinking enough water, because the minerals in their urine become more concentrated. This is why veterinarians often recommend high-moisture diets for cats with a history of urinary problems.
Stress and Environmental Triggers
While stress primarily drives non-infectious bladder inflammation (FIC) rather than bacterial UTIs, the two conditions can overlap. A stressed cat with a compromised bladder lining may be more vulnerable to secondary bacterial infection. The environmental factors that contribute to feline urinary distress are worth addressing regardless of the specific diagnosis.
Common stressors include conflict with other cats in the household, changes in routine, new people or animals in the home, and problems with litter box setup. Cats that feel intimidated by a housemate may avoid the litter box entirely, holding urine longer than they should. Prolonged urine retention gives bacteria more time to multiply and gives crystals more time to form.
Practical steps that reduce urinary stress include providing one more litter box than the number of cats in the household, placing boxes in quiet low-traffic areas, avoiding scented litter, and ensuring that no single cat can guard all the boxes. These measures won’t cure a bacterial infection, but they address conditions that make urinary problems more likely to develop or recur.
Diet and Water Intake
Cats evolved as desert animals that get most of their moisture from prey. Domestic cats on dry-food-only diets often don’t compensate by drinking enough water, leading to concentrated urine. That concentrated urine creates conditions favorable for both crystal formation and bacterial growth.
Wet food can make a meaningful difference. Its higher moisture content helps dilute the urine, which reduces mineral concentration and flushes bacteria from the bladder more frequently. For cats prone to urinary issues, targeting a more dilute urine through diet changes or encouraging water intake (with fountains, multiple water bowls, or adding water to food) is one of the most effective long-term strategies. Veterinarians may also recommend therapeutic diets that adjust urine pH to discourage specific stone types from forming.
Signs That Point to a Urinary Problem
Bacterial UTIs and non-infectious bladder inflammation look nearly identical from the outside. Common signs include frequent trips to the litter box with little urine produced, straining or crying during urination, blood-tinged urine, licking the genital area excessively, and urinating outside the litter box. A cat that suddenly stops using the litter box isn’t necessarily misbehaving. Pain during urination can create a negative association with the box itself, causing the cat to seek other locations.
One pattern worth noting: cats with kidney disease, diabetes, or hyperthyroidism often drink noticeably more water and produce larger volumes of urine. If your cat is suddenly draining the water bowl and flooding the litter box, that points toward a systemic illness rather than simple bladder inflammation. Behavioral spraying looks different from medical house soiling. Spraying cats stand and direct small amounts of urine onto vertical surfaces, while cats with urinary problems squat and produce puddles, often in unusual spots around the house.
How Feline UTIs Are Diagnosed and Treated
Because bacterial UTIs are uncommon in cats relative to other urinary conditions, current veterinary guidelines recommend confirming the diagnosis with a urine culture in every case. This involves collecting a urine sample directly from the bladder with a needle (a quick, routine procedure) and sending it to a lab to identify whether bacteria are present and which antibiotic will work against them.
For a first-time bacterial UTI, treatment is typically short: 3 to 5 days of antibiotics. Recurrent infections require more investigation. Your vet will look for an underlying cause, whether that’s kidney disease, stones, or anatomical abnormalities. Persistent infections where bacteria have invaded the bladder wall may need longer treatment courses of 7 to 14 days. In cats with recurring UTIs, identifying and managing the root cause matters more than repeatedly prescribing antibiotics.
