White spots on teeth have several common causes, but they almost always point to one thing: something disrupted the normal mineral content of your enamel. The most frequent culprits are too much fluoride during childhood, plaque buildup that eats away at enamel, and developmental disruptions that occurred while your teeth were still forming. The cause matters because it determines whether the spots can be reversed, need professional treatment, or are purely cosmetic.
Too Much Fluoride During Childhood
Dental fluorosis is one of the most common reasons for white spots, and it happens when developing teeth are exposed to more fluoride than they can handle. The U.S. Public Health Service recommends community water fluoridation at 0.7 mg per liter as the level that prevents cavities while minimizing risk. Visible enamel changes start appearing when water fluoride levels exceed 1.5 parts per million, roughly double the recommended concentration.
Fluorosis only affects teeth that haven’t yet erupted, so it’s a childhood issue. Kids who swallow fluoride toothpaste, drink water with naturally high fluoride levels, or take fluoride supplements on top of fluoridated water are at highest risk. The resulting spots range from faint white streaks in mild cases to brown discoloration and pitting in severe ones. Once your adult teeth are in, you can’t develop new fluorosis, but existing spots are permanent without treatment.
Plaque Buildup and Early Decay
White spot lesions from demineralization are a different story entirely. These spots are the earliest visible stage of tooth decay, and unlike fluorosis, they can appear at any age. Bacteria in dental plaque feed on sugars and produce acid. When that acid sits on a tooth surface long enough, it pulls calcium and phosphate out of the enamel, creating a chalky white patch that looks different from the glossy enamel around it.
This is extremely common with braces. A meta-analysis of 14 studies found that 45.8% of orthodontic patients developed new white spot lesions during treatment, and the overall prevalence of lesions in patients with braces was 68.4%. The brackets make it harder to brush thoroughly, and plaque accumulates in the gaps between the hardware and the tooth surface. But braces aren’t the only trigger. Anyone with poor brushing habits, a high-sugar diet, or chronic dry mouth can develop these spots.
The important distinction here is that demineralization spots are potentially reversible if caught early. Your saliva naturally deposits minerals back onto enamel, and fluoride accelerates that process. Once the damage progresses past the surface layer, though, it becomes a cavity that needs a filling.
Enamel Hypoplasia
Sometimes teeth simply don’t form correctly. Enamel hypoplasia means the enamel layer is thinner than normal in certain spots, leaving behind white or yellowish patches, pits, or grooves. This happens when something disrupts enamel development during childhood, typically between ages 2.5 and 5.5, when permanent teeth are actively forming beneath the gums.
The disruptions can be genetic, but they’re more often caused by external stressors: high fevers, malnutrition, certain infections, or trauma to the baby teeth that damages the developing adult teeth underneath. Premature birth and low birth weight also increase risk. Unlike demineralization, these spots can’t be reversed with remineralization because the enamel was never properly formed in the first place.
Celiac Disease and Other Medical Conditions
White spots sometimes signal an underlying health condition. Celiac disease is the best-documented example. According to the National Institute of Diabetes and Digestive and Kidney Diseases, dental enamel problems are fairly common in people with celiac disease, particularly children. The defects involve permanent teeth and include white, yellow, or brown spots along with poor enamel formation, pitting, and a translucent appearance. A hallmark clue is that the damage is symmetrical, often affecting the incisors and molars on both sides of the mouth.
If a dentist notices this pattern, especially in a child with digestive symptoms or unexplained nutritional deficiencies, celiac disease is worth investigating. The enamel damage happens because the autoimmune response interferes with normal tooth development, so early diagnosis and a gluten-free diet can protect teeth that haven’t fully formed yet.
How to Tell the Difference
The location and pattern of white spots give strong clues about their cause. Fluorosis tends to produce symmetrical streaks or speckling across multiple teeth, since all developing teeth were exposed to the same fluoride levels. Demineralization from plaque shows up near the gumline or around bracket sites, following the outline of where plaque sat the longest. Hypoplasia creates irregular patches or pits, often on just one or two teeth unless it was caused by a systemic illness like celiac disease, which affects teeth symmetrically.
Timing also helps. If the spots have been there since your adult teeth came in, fluorosis or a developmental defect is most likely. If they appeared gradually during or after braces, or after a period of poor oral hygiene, demineralization is the probable cause.
Treating White Spots at Home
For early demineralization spots, the first step is improving oral hygiene and using a remineralizing toothpaste. Fluoride toothpaste remains the standard recommendation. Toothpastes containing hydroxyapatite, a synthetic version of the mineral that makes up tooth enamel, are a newer option. A randomized clinical trial comparing the two over 90 days found that hydroxyapatite toothpaste was more effective at reducing tooth sensitivity associated with white spot lesions, though neither type showed a significant advantage over the other for repairing surface erosion in that timeframe.
Home care works best on spots that are still in the early demineralization stage, where the surface enamel is intact but porous. If the spot has been there for years or resulted from a developmental issue, toothpaste alone won’t make a visible difference.
Professional Treatment Options
For spots that don’t respond to home care, dentists have several approaches depending on how deep the damage goes.
Resin infiltration is a minimally invasive option where a low-viscosity resin is applied to the porous enamel. The resin fills microscopic gaps in the tooth surface through capillary action. Because the resin has a light-refracting property similar to healthy enamel, the white spot essentially disappears once the pores are filled. A four-year follow-up study found that the color and appearance remained stable over that entire period, with no progression of decay in the treated areas. No drilling is involved, and the procedure preserves the original tooth structure.
Microabrasion is another option, particularly for fluorosis. A dentist applies an acid-based paste and gently buffs away the outermost layer of stained enamel. This removes roughly 250 micrometers of enamel, about a quarter of a millimeter. Studies show it’s effective in over 90% of patients with moderate to severe fluorosis stains. It works best on superficial discoloration; deeper defects may need veneers or bonding instead.
For more severe cases where significant enamel is missing or deeply pitted, dental bonding or porcelain veneers provide a cosmetic cover. These are typically reserved for spots that don’t respond to less invasive treatments.
Preventing White Spots
Prevention depends on the cause. For fluorosis, supervising children’s toothpaste use is the most practical step. Kids under three should use a rice-grain-sized smear; ages three to six should use a pea-sized amount. Teaching them to spit rather than swallow makes a significant difference, since most excess fluoride exposure in developed countries comes from swallowed toothpaste rather than water.
For demineralization, consistent brushing and flossing are the foundation. If you or your child has braces, extra attention around brackets is critical given how common white spots are during orthodontic treatment. Reducing sugary snacks and acidic drinks limits the fuel that plaque bacteria use to produce enamel-damaging acid. Chewing sugar-free gum after meals stimulates saliva flow, which helps neutralize acid and redeposit minerals on enamel surfaces.
