What Causes Your Bones to Hurt: Injuries to Cancer

Bone pain feels different from muscle soreness or joint stiffness. It’s typically deep, penetrating, or dull, and it often persists whether you’re moving or resting. The causes range from something as straightforward as a hard impact to something as serious as cancer spreading to the skeleton. Understanding what’s behind bone pain starts with knowing how bones actually sense pain in the first place.

Why Bones Hurt at All

Bones aren’t the inert scaffolding most people imagine. They’re wrapped in a thin, nerve-rich tissue called the periosteum, and sensory nerve fibers also run through the interior cavity and the spongy inner layers of bone. These nerves have their cell bodies near the spinal cord, and when something damages or irritates bone tissue, they fire signals up through the spinal cord to pain-processing centers in the brain.

This wiring explains why bone pain feels so different from a pulled muscle. Muscle and tendon pain tends to flare with specific movements and ease with rest. Bone pain sits deeper. It can ache constantly, even when you’re lying still, and it’s often hard to pinpoint exactly where it’s coming from.

Injuries: Bruises, Fractures, and Stress Reactions

The most common reason for bone pain is physical trauma. A fall, collision, or direct blow can cause a bone bruise, which happens when the impact is hard enough to cause bleeding inside the bone but not hard enough to crack it. Most bone bruises heal within a few weeks, though more severe ones can take months.

Stress fractures are a step beyond bruising. These are tiny cracks that develop from repetitive force, like running on hard surfaces or a sudden increase in physical activity. They’re common in the shins, feet, and lower legs. The pain usually builds gradually, worsens with activity, and improves with rest. A full fracture, where the bone breaks through, causes immediate and often intense pain along with swelling and an inability to bear weight.

Vitamin D Deficiency and Soft Bones

Your body needs vitamin D to absorb calcium and phosphorus, the minerals that keep bones hard and strong. When vitamin D runs too low for too long, bones stop mineralizing properly and become soft and fragile. This condition is called osteomalacia in adults, and it causes a widespread, aching bone pain that people often mistake for general fatigue or “getting older.”

The pain tends to show up in the lower back, hips, pelvis, and legs. It can be worse with weight-bearing activity and sometimes comes with muscle weakness that makes climbing stairs or getting up from a chair noticeably harder. Osteomalacia is treatable once identified, usually by correcting the vitamin D deficit, but it often goes unrecognized for months because the symptoms are so nonspecific.

Bone Infections

Osteomyelitis is a bone infection most often caused by staphylococcus bacteria, the same type that normally lives on your skin and in your nose. These bacteria can reach bone through the bloodstream, through a nearby infection that spreads, or through a wound that exposes bone directly (like after surgery or an open fracture).

The hallmark symptoms are localized pain, swelling, warmth, and tenderness over the infected area. You may also develop a fever or feel generally unwell. Osteomyelitis can become serious quickly if untreated, since infection can destroy bone tissue and spread to surrounding structures. It’s more common in young children and older adults, and people with diabetes or weakened immune systems face higher risk.

Cancer That Starts in or Spreads to Bone

Cancer is one of the more serious causes of bone pain, and it can reach bone in two ways. Primary bone cancers like osteosarcoma start in the bone itself, but they’re relatively rare. Far more common is bone metastasis, where cancer from another organ spreads to the skeleton. Breast cancer, lung cancer, and prostate cancer are the most frequent sources, though other cancers can do this too.

When cancer cells settle in bone, they hijack the normal cycle of bone maintenance. Healthy bone constantly remodels itself: one type of cell builds new bone while another type breaks down old bone. Cancer disrupts this balance, typically accelerating bone breakdown. The cells that dissolve bone work faster than normal, releasing substances that create tiny holes in the skeleton. This weakens the bone and irritates the dense web of nerves running through it, producing pain that tends to be persistent, often worse at night, and gradually increasing over time.

Leukemia, a blood cancer that develops in bone marrow, can also cause bone pain. It tends to show up in bones with large amounts of marrow: the breastbone, ribs, hips, arms, and legs. The pain comes from the expanding mass of abnormal cells crowding the marrow space and pressing outward against the bone’s nerve-rich lining.

Paget’s Disease

Paget’s disease is a condition where the body breaks down and rebuilds bone much faster than normal. The rushed remodeling produces bone that’s disorganized and weaker than it should be, leading to pain, deformities, and an increased risk of fractures. The pelvis, skull, spine, and legs are the most commonly affected sites.

In some cases, the abnormal bone growth compresses nearby nerves. When this happens in the spine or skull, it can cause pain, weakness, tingling in the arms or legs, or even hearing loss. Many people with Paget’s disease have no symptoms at all and only discover it through imaging done for another reason.

Other Common Causes

Several other conditions can produce bone pain:

  • Osteoporosis. Bones become progressively thinner and more porous, especially after menopause. Osteoporosis itself doesn’t always hurt, but it dramatically increases the risk of fractures, including compression fractures in the spine that cause sudden, sharp back pain.
  • Sickle cell disease. Misshapen red blood cells can block blood flow to bone tissue, causing episodes of severe pain called vaso-occlusive crises. The pain often hits the long bones of the arms and legs, the chest, and the back.
  • Hyperparathyroidism. Overactive parathyroid glands pull too much calcium from the bones, weakening them and sometimes causing a deep, aching pain.
  • Certain medications. Some drugs used to treat cancer, osteoporosis, or hormonal conditions can cause bone pain as a side effect.

How to Tell Bone Pain From Muscle or Joint Pain

This distinction matters because the causes and treatments are very different. Bone pain is usually deep and dull, present even at rest, and doesn’t change much when you stretch or move the surrounding muscles. Tendon and ligament pain tends to be sharper, flares when you move the affected structure, and improves when you stop using it. Joint pain from arthritis typically worsens with movement but is also noticeable when the joint is still.

If your pain is deep, constant, doesn’t clearly respond to rest or movement, and has lasted more than a couple of weeks without an obvious injury, it’s worth investigating further.

Warning Signs That Need Prompt Attention

Most bone pain turns out to have a manageable cause. But certain patterns suggest something more serious is going on. Pain that wakes you up at night or doesn’t respond to typical pain relief is one red flag. Others include unexplained weight loss (particularly losing more than 10 kilograms in three months), fever or feeling generally unwell alongside the pain, pain that steadily gets worse rather than better, and pain with no history of injury.

A personal history of cancer raises the stakes significantly. Bone metastasis can appear years after the original cancer was treated, so persistent new bone pain in someone with a cancer history always warrants imaging. An unusual lump or swelling over a bone, especially one that’s growing, is another signal to act on quickly.

How Bone Pain Gets Diagnosed

Standard X-rays are often the first step, since they can reveal fractures, signs of infection, tumors, and bone thinning. But X-rays miss early problems. MRI is more sensitive for detecting bone marrow changes, stress reactions, and soft tissue involvement. For purely bone-related causes of pain, a specialized nuclear imaging scan can outperform MRI in pinpointing the exact source, with one study finding it had better specificity (60% vs. 31%) for identifying the pain generator in bone.

Blood tests often accompany imaging. Vitamin D levels, calcium, inflammatory markers, and blood counts can point toward nutritional deficiencies, infection, or blood cancers. The specific workup depends on the pattern of pain, your age, and your medical history.
