Swollen legs and feet happen when excess fluid gets trapped in the tissues beneath your skin, a condition called peripheral edema. The causes range from something as simple as sitting too long on a flight to serious conditions involving your heart, kidneys, or veins. Understanding why it happens starts with how your body normally moves fluid around, and which breakdowns lead to visible swelling.

How Fluid Ends Up Trapped in Your Tissues

Your body constantly pushes fluid out of tiny blood vessels (capillaries) and pulls it back in. Two forces drive this exchange: the pressure of blood pushing outward against vessel walls, and proteins in your blood that act like sponges, drawing fluid back in. When these forces fall out of balance, fluid leaks into the surrounding tissue faster than it can be reabsorbed.

Several things can tip that balance. Higher pressure inside the veins forces more fluid out. Lower protein levels in the blood reduce the “sponge effect” that pulls fluid back. Damaged or leaky vessel walls let fluid escape more easily. And if the lymphatic system, your body’s drainage network, gets sluggish or blocked, fluid has no way to return to circulation. Swelling in the legs and feet specifically is common because gravity pulls fluid downward throughout the day, making those tissues the first place excess fluid collects.

Venous Insufficiency: The Most Common Vascular Cause

Chronic venous insufficiency affects about 1 in 20 adults and is one of the leading reasons for persistent leg swelling. Your leg veins contain one-way valves that help push blood upward against gravity, back toward your heart. When those valves weaken or become damaged, blood flows backward and pools in the lower legs. This pooling raises pressure inside the veins, which forces fluid into surrounding tissue.

The swelling from venous insufficiency tends to worsen over the course of the day, especially if you stand or sit for long periods. Over time, you may also notice skin changes around your ankles: darkening, thickening, or a leathery texture. Varicose veins often accompany the condition. Compression stockings, leg elevation, and regular movement are the primary ways to manage it, though more advanced cases sometimes require procedures to close or remove damaged veins.

Heart, Kidney, and Liver Conditions

When swelling appears in both legs equally, it can signal a problem with a major organ system. These conditions cause fluid retention throughout the body, but gravity means the legs and feet show it first.

Heart Failure

A weakened heart pumps less blood forward with each beat. That creates a traffic jam in the veins, raising pressure and pushing fluid into tissues. At the same time, reduced blood flow to the kidneys triggers a backup response: the kidneys hold onto sodium and water in an attempt to boost blood volume, which only adds to the fluid overload. This cycle is a hallmark of congestive heart failure and explains why leg swelling often worsens alongside shortness of breath and fatigue.

Kidney Disease

Healthy kidneys filter excess fluid and sodium from your blood. When kidney function declines, that fluid stays in circulation and eventually seeps into tissues. Kidney disease can also cause protein to leak into the urine, which lowers the concentration of proteins in your blood. With fewer proteins acting as sponges, fluid moves out of blood vessels more easily.

Liver Disease

Your liver produces albumin, the main protein responsible for keeping fluid inside blood vessels. Advanced liver disease, particularly cirrhosis, reduces albumin production significantly. The result is widespread fluid accumulation, often visible as swelling in the legs and a distended abdomen.

Medications That Cause Swelling

A surprisingly long list of common medications can cause leg and foot swelling as a side effect. Calcium channel blockers, a widely prescribed class of blood pressure drugs, are among the most frequent culprits. They relax blood vessel walls, which can increase fluid leakage into tissues. Some people notice ankle swelling within weeks of starting these medications.

Other medications linked to fluid retention include corticosteroids (like prednisone), certain diabetes drugs, nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen, and some hormone therapies including estrogen and testosterone. If your swelling started shortly after beginning a new medication, that timing is worth noting. Stopping or switching the medication often resolves the swelling, but never adjust a prescription without talking to whoever prescribed it.

Pregnancy and Hormonal Shifts

Some degree of swelling in the feet and ankles is normal during pregnancy, particularly in the third trimester. The growing uterus puts pressure on veins that return blood from the legs, and hormonal changes cause the body to retain more fluid overall. Mild, gradual swelling that worsens with standing and improves with rest is typically expected.

What’s not normal is sudden, severe swelling, especially in the face and hands. This pattern after 20 weeks of pregnancy can be a warning sign of preeclampsia, a condition defined by high blood pressure and signs of organ damage, particularly to the kidneys. Preeclampsia requires prompt medical attention because it can progress quickly and become dangerous for both mother and baby.

Blood Clots: When One Leg Swells Suddenly

Deep vein thrombosis (DVT) occurs when a blood clot forms in a deep vein, usually in the leg. The classic presentation is swelling in one leg (not both), often accompanied by pain or cramping that starts in the calf, skin that looks red or purple, and a feeling of warmth in the affected area. However, DVT can also occur without noticeable symptoms, which makes it particularly dangerous.

The serious risk with DVT is that the clot can break free and travel to the lungs, causing a pulmonary embolism. Warning signs of that complication include sudden shortness of breath, chest pain that worsens when you breathe deeply or cough, a rapid pulse, dizziness, or coughing up blood. A pulmonary embolism is a medical emergency. If your leg swelling is sudden, one-sided, and painful, treat it urgently rather than waiting to see if it improves.

Lifestyle Factors and Prolonged Inactivity

Not all leg swelling points to a medical condition. Gravity alone can do it. Sitting in a chair for hours, whether at a desk or on a long flight, keeps your calf muscles inactive. Those muscles normally act as pumps, squeezing veins to push blood back toward the heart. Without that pumping action, fluid accumulates in your lower legs. Standing in one place for extended periods has the same effect.

Excess body weight increases the pressure on veins in the legs and pelvis, making fluid retention more likely. A high-sodium diet plays a role too. Sodium causes your body to hold onto water, expanding your blood volume and increasing the pressure that pushes fluid into tissues. The American Heart Association recommends staying under 1,500 mg of sodium per day, though many people consume double or triple that amount without realizing it, since most dietary sodium comes from processed and restaurant foods rather than the salt shaker.

How Swelling Is Evaluated

When you press a swollen area with your finger, pay attention to what happens. If the pressure leaves an indentation that takes a few seconds (or longer) to fill back in, that’s called pitting edema. Doctors grade it on a scale from 1 to 4 based on how deep the pit is and how long it takes to rebound. A grade 1 pit is about 2 millimeters deep and bounces back immediately. A grade 4 pit reaches 8 millimeters deep and can take two to three minutes to fill back in. Higher grades generally suggest more significant fluid overload.

The pattern of swelling gives important clues about the cause. Swelling in both legs that worsens over the day and improves overnight points toward venous insufficiency, heart failure, or fluid retention from medications or diet. Swelling in one leg, especially if it came on suddenly, raises concern for a blood clot. Swelling that doesn’t pit when pressed may involve the lymphatic system rather than fluid pressure from the veins.

Your doctor will likely start with blood work to check kidney function, liver function, and protein levels. An ultrasound of the leg veins can reveal clots or valve problems. If heart failure is suspected, an imaging study of the heart can show how well it’s pumping. The specific workup depends on whether the swelling is new or chronic, one-sided or both, and what other symptoms you have.

Reducing Swelling at Home

For mild, non-urgent swelling, a few strategies help. Elevating your legs above heart level for 20 to 30 minutes several times a day lets gravity work in your favor, draining fluid back toward your core. Compression stockings apply steady pressure that supports your veins and prevents fluid from pooling. Walking, even briefly, activates the calf muscle pump and improves circulation.

Cutting back on sodium makes a measurable difference. Aim for under 2,000 mg per day if you’re dealing with fluid retention. That means reading nutrition labels closely, cooking more meals at home, and being cautious with canned soups, deli meats, sauces, and fast food. Staying well hydrated sounds counterintuitive when you’re retaining fluid, but dehydration actually triggers your body to hold onto more water, not less.

These measures manage symptoms, but they don’t replace finding out why the swelling is happening in the first place. Persistent or worsening swelling, swelling accompanied by shortness of breath, or swelling that appears suddenly in one leg all warrant a medical evaluation to identify and treat the underlying cause.
