Stomach bloating happens when gas builds up in your digestive tract, when your intestines retain extra fluid, or when your body overreacts to normal amounts of gut contents. Sometimes your belly visibly swells. Other times you feel uncomfortably full and tight without any measurable change in size. These two experiences, the sensation and the visible expansion, can occur together or separately, and they have different underlying causes.
Too Much Gas From Fermentation
The most common reason for bloating is excess gas produced inside your intestines. Your gut bacteria feed on carbohydrates that weren’t fully digested higher up in the tract, and fermentation is the result. That process releases hydrogen, carbon dioxide, and methane, which inflate your intestines like a balloon. The two biggest drivers of this overproduction are bacterial overgrowth in the small intestine and food intolerances.
Certain carbohydrates are especially prone to fermentation. Known collectively as FODMAPs (fermentable sugars found in foods like onions, garlic, wheat, beans, and some fruits), these molecules can’t be broken down and absorbed in the small intestine. Instead, the small intestine draws in extra water to push them along to the colon, where bacteria consume them and release gas and fatty acids as byproducts. The combination of extra water and extra gas is what makes high-FODMAP meals feel so heavy.
Food Intolerances and Malabsorption
Lactose intolerance is one of the clearest examples of how malabsorption leads to bloating. When your body doesn’t produce enough of the enzyme that breaks down lactose, the sugar passes intact into the colon. There, bacteria ferment it and produce hydrogen, carbon dioxide, and methane. At the same time, the undigested lactose creates an osmotic pull, drawing water into the intestine. This double hit of gas and fluid is what makes dairy so uncomfortable for people who are intolerant.
Fructose, sorbitol, and other sugar alcohols work in a similar way. If your small intestine can’t absorb them efficiently, they travel downstream and become fuel for bacteria. The resulting gas production is often enough to cause visible distension, rumbling, and pain.
Your Gut May Overreact to Normal Gas
Not everyone who feels bloated is actually producing more gas than average. Many people have what’s called visceral hypersensitivity: their gut nerves are dialed up, so a normal volume of gas or food feels like too much. These people can be consciously aware of contents moving through their intestines in a way that others simply aren’t.
This heightened perception is amplified by signals traveling between the brain and the gut. Anxiety, depression, chronic stress, and hypervigilance all strengthen those neural pathways, making normal digestive sensations register as pressure, fullness, or pain. This is a real, physiological phenomenon, not something imagined. It helps explain why bloating often worsens during stressful periods even when your diet hasn’t changed.
When Your Muscles Push the Wrong Way
Your body has a reflex system that coordinates the diaphragm and abdominal wall muscles to clear gas from the intestines. In some people, this reflex misfires. Instead of the diaphragm lifting and the abdominal muscles staying firm, the diaphragm drops and the abdominal wall relaxes. The result is a belly that visibly protrudes, sometimes dramatically, even though the actual amount of gas inside is unremarkable. This abnormal reflex is one reason your stomach can look flat in the morning and distended by evening without any obvious dietary trigger.
Bacterial Overgrowth in the Small Intestine
Your small intestine normally has relatively few bacteria compared to the colon. When bacteria colonize the small intestine in larger numbers than they should, a condition called SIBO, they begin fermenting carbohydrates earlier in the digestive process. More bacteria means more gas and more byproducts, and because the small intestine is narrower and more sensitive than the colon, even modest gas volumes can cause significant bloating, nausea, and distension. SIBO often develops after food poisoning, abdominal surgery, or conditions that slow gut motility.
Slow Stomach Emptying
When the stomach takes too long to move food into the small intestine, a condition called gastroparesis, food sits and creates a persistent feeling of fullness and upper abdominal pressure. The vagus nerve controls the muscles that churn and empty the stomach, and if that nerve is damaged or dysfunctional, the whole process slows down or stalls. Diabetes, certain medications, and prior surgeries are common causes. People with gastroparesis often feel bloated after eating very small amounts of food.
Hormonal Shifts and the Menstrual Cycle
Estrogen and progesterone directly affect how quickly food moves through the digestive tract and how sensitive the gut is to pain and pressure. As these hormones fluctuate during the menstrual cycle, many women experience bloating in the days leading up to their period, when progesterone peaks and slows gut motility. The same mechanism explains why bloating is common during pregnancy, perimenopause, and menopause. Hormonal bloating also involves fluid retention, which adds to the sensation of abdominal fullness without necessarily involving extra gas.
Salt and Fluid Retention
A high-sodium diet contributes to bloating through a straightforward mechanism: salt causes your body to hold onto water. But the effect goes beyond simple fluid retention. Research from Johns Hopkins found that high-sodium diets increased the risk of bloating by about 27 percent compared to low-sodium versions of the same diet. Researchers suspect that sodium also changes the gut microbiome in ways that increase bacterial sulfide production, adding a gas component on top of the water retention.
Processed foods, restaurant meals, and canned soups are common culprits because their sodium content is often far higher than what you’d add from a salt shaker at home.
Fiber: Helpful but Tricky
Fiber is essential for healthy digestion, but increasing your intake too quickly is one of the most reliable ways to trigger bloating. The recommended daily intake ranges from about 22 to 34 grams depending on your age and sex. If you’re eating significantly less than that and suddenly jump to the target, your gut bacteria will produce a surge of gas as they adapt to the new fuel source.
The Academy of Nutrition and Dietetics recommends increasing fiber by no more than 5 grams per day until you reach your goal. This gives your microbiome time to adjust, and the bloating that comes with a fiber increase typically fades within a few weeks. Drinking extra water alongside fiber also helps keep things moving rather than fermenting.
Constipation and Backed-Up Stool
When stool moves slowly through the colon, it sits longer and gives bacteria more time to ferment whatever’s left. The gas produced has nowhere to go easily, and the physical bulk of backed-up stool adds pressure. Constipation is one of the most straightforward causes of bloating and one of the easiest to identify: if you’re not having regular bowel movements, the bloating is likely connected.
Swallowed Air
Every time you eat, drink, or swallow saliva, you take in small amounts of air. Certain habits increase this significantly: chewing gum, drinking through straws, eating quickly, talking while eating, and drinking carbonated beverages. The gas that enters this way is mostly nitrogen and oxygen rather than the hydrogen and methane produced by fermentation, but the physical effect is the same. Your stomach and intestines expand, and you feel pressure until the air is released through belching or passing gas.
