What Country Has the Lowest Cancer Rate and Why

Sierra Leone has the lowest reported cancer rate in the world, with an age-standardized incidence of 35.9 cases per 100,000 people. That’s nearly 13 times lower than Australia, which tops the list at 462.5 per 100,000. But this ranking comes with a major caveat: the countries that appear to have the fewest cancers are also the countries least equipped to detect and record them.

Countries With the Lowest Reported Rates

According to the World Cancer Research Fund, which draws on data from the International Agency for Research on Cancer (IARC), the countries clustered at the bottom of global cancer rankings are predominantly in sub-Saharan Africa and parts of South Asia. Sierra Leone sits at the very bottom with an age-standardized rate of 35.9 per 100,000. The Gambia, another West African nation, reports a rate of 79.2 per 100,000, also far below the global average.

For comparison, most high-income countries report rates between 250 and 460 per 100,000. Australia leads the world largely because of its high melanoma rates, strong screening programs, and near-complete cancer tracking. The gap between Australia and Sierra Leone, a difference of over 426 cases per 100,000 people, is striking. But much of that gap reflects differences in data quality rather than differences in actual cancer burden.

Why These Numbers Are Misleading

Cancer registries are the systems countries use to track every new diagnosis. Only about 21% of the world’s population is covered by population-based cancer registries, and most of the roughly 700 high-quality registries are in high-income countries. The United States and parts of northern Europe have nearly total population coverage. In Africa, that figure drops to 2%. More than 20 countries in sub-Saharan Africa lack any meaningful cancer surveillance capacity at all.

When a country has no reliable way to record cancer cases, its reported rate will naturally appear low. People develop cancer but never receive a diagnosis. They may die of what gets recorded as a fever, an infection, or simply unknown causes. Without pathology labs, imaging equipment, or trained specialists, many cancers go entirely undetected. So a rate like Sierra Leone’s 35.9 per 100,000 doesn’t mean cancer is rare there. It means the country’s health system captures only a fraction of the cancers that actually occur.

Factors That Genuinely Lower Cancer Rates

That said, some real biological and demographic factors do contribute to lower cancer incidence in certain regions. The most significant is age. Cancer risk rises sharply after age 50, and many low-income countries have younger populations with shorter average life expectancies. In countries where fewer people live into their 60s and 70s, fewer people reach the ages when cancer is most common.

Lifestyle factors also play a role. Populations with lower rates of obesity, less alcohol consumption, and less exposure to processed food tend to develop fewer of the cancers tied to those risk factors, particularly colorectal, breast, and prostate cancers. Some countries also have lower rates of tobacco use, which reduces lung cancer incidence.

However, lower-income countries face their own distinct cancer risks. Infections cause an estimated 12% of new cancer cases worldwide, but in many low-income countries across Africa and Asia, infections are responsible for more than a quarter of all cancers. The five most important cancer-causing infectious agents are the bacterium that causes stomach ulcers (responsible for roughly 850,000 cases globally), HPV (730,000 cases), hepatitis B (380,000), Epstein-Barr virus (200,000), and hepatitis C (170,000). Together, these infections drive over 90% of all infection-related cancers. So while these countries may have fewer lifestyle-driven cancers, they carry a heavier burden of infection-driven ones, many of which go undiagnosed.

What Higher Rates Actually Reflect

It might seem paradoxical, but a high national cancer rate is partly a sign of a well-functioning health system. Countries like Australia, Denmark, and the United States report high numbers because they screen aggressively, diagnose early, and record every case. Widespread mammography, colonoscopy, PSA testing, and skin checks catch cancers that would go unnoticed in countries without those programs.

This creates a statistical illusion. Screening can temporarily inflate a country’s cancer rate by detecting tumors that might never have caused symptoms. Meanwhile, countries without screening appear healthier on paper while people die of undiagnosed disease. Australia’s top ranking, for instance, reflects both a genuine melanoma problem and an exceptionally thorough cancer registry, not simply that Australians are uniquely unhealthy.

How to Interpret Global Cancer Rankings

If you’re looking at a list of countries ranked by cancer rate, the most reliable comparisons are between nations with similarly robust health systems. Comparing Denmark to Canada tells you something real. Comparing Australia to Sierra Leone tells you mostly about infrastructure gaps.

The countries with genuinely low cancer rates relative to their peers, after adjusting for data quality, tend to share a few traits: younger populations, lower obesity rates, less tobacco and alcohol use, and in some cases, dietary patterns linked to lower cancer risk. But no country is immune to cancer, and no population has a rate anywhere close to zero once adequate detection is in place.

The honest answer to “which country has the lowest cancer rate” is that we don’t fully know. The countries at the bottom of published rankings are almost all places where cancer goes largely uncounted. Until registry coverage improves in Africa and parts of Asia, those numbers will continue to reflect what gets measured, not what actually exists.
