Most bone spurs cause no sensation at all. Many people discover them by accident on an X-ray taken for something else entirely. But when a bone spur does cause symptoms, what you feel depends almost entirely on where it is and what it’s pressing against. The sensations range from a dull ache and stiffness to sharp, radiating pain, tingling, or a grinding feeling inside a joint.
Many Bone Spurs Feel Like Nothing
Despite the name, bone spurs are actually small, smooth bumps of extra bone that grow along the edges of joints. They develop gradually, often as part of the body’s response to wear and tear from osteoarthritis. The word “spur” makes them sound sharp and painful, but the growth itself isn’t jagged or pointy.
People often learn they have a bone spur only after getting an X-ray for an unrelated issue. That’s because many bone spurs never press on anything sensitive. A spur only becomes a problem when it pushes against a nerve, rubs on a tendon, or physically blocks a joint’s normal range of motion. So if you suspect you have one, the absence of symptoms doesn’t mean it isn’t there, and the presence of a spur on imaging doesn’t automatically explain your pain.
Pain That Gets Worse With Movement
When a bone spur does hurt, the most common sensation is a dull ache or stiffness around the affected joint. The pain tends to be activity-related: it flares when you use the joint and settles when you rest. In the knee, for example, bone spurs can make it painful to fully extend or bend the leg. In the hip, the pain shows up when you walk or rotate the joint. The soreness often comes not from the bone itself but from the inflammation it triggers in surrounding tendons, ligaments, or the fluid-filled sacs (bursae) that cushion your joints.
Swelling around the area is common, and you may notice the joint feels warm or puffy after use. Over time, you might find yourself unconsciously avoiding certain movements because they consistently provoke discomfort.
Tingling, Numbness, and Nerve Pain
Some of the most intense symptoms happen when a bone spur presses on a nerve. This produces a distinctly different sensation from joint pain. Instead of a localized ache, you feel sharp or burning pain that radiates outward, often traveling down an arm or leg far from where the spur actually sits. You may also experience tingling, pins and needles, or numbness in the area the nerve supplies. In some cases, the affected hand or foot feels like it has “fallen asleep.”
Spinal bone spurs are particularly likely to cause these symptoms. As extra bone grows on the vertebrae, it can narrow the channel where the spinal cord and nerve roots travel. When those structures get compressed, the result is weakness or numbness in the arms or legs, depending on which part of the spine is affected. A large spur in the lower back, for instance, can press into surrounding muscle tissue or compress nerve roots, causing radiating pain through the hip and down the leg.
If you notice progressive weakness, loss of coordination, or difficulty controlling your bladder or bowels, that suggests significant nerve compression that needs prompt medical attention.
Heel Spurs and First-Step Pain
Heel bone spurs produce one of the most recognizable pain patterns. The classic experience is a stabbing pain on the bottom of your foot with your very first steps in the morning. After you’ve walked around for a few minutes, the pain often dulls to a manageable ache, only to return after long periods of standing or when you stand up after sitting for a while.
This pattern happens because the band of tissue along the bottom of your foot (the plantar fascia) tightens while you sleep. When you step down, that tissue stretches suddenly over and around the spur, and repeated stress can cause tiny tears that become inflamed. The pain is typically worst at the heel and may feel like stepping on a pebble or a nail. It tends to improve with gentle activity but worsens again after prolonged weight-bearing.
Shoulder Spurs and Nighttime Pain
Bone spurs on the bony roof of the shoulder (the acromion) create a very specific problem. As the spur grows, it reduces the space available for the rotator cuff tendons to slide through when you lift your arm. The result is pain that strikes when you reach overhead, behind your back, or out to the side. It typically feels like it’s coming from the front of the shoulder.
What catches many people off guard is how much worse shoulder spurs feel at night. Lying on the affected side compresses the already-tight space, and many people find the pain wakes them up or makes it impossible to sleep in their usual position. This nighttime worsening is one of the hallmarks of shoulder impingement caused by bone spurs.
Grinding and Catching Sensations
In joints like the knee, bone spurs can produce a grinding or grating sensation during movement, sometimes called crepitus. It feels like sandpaper or gravel inside the joint and is often audible as a continuous scratching noise when you bend or straighten the leg. This happens because bone spurs, along with the cartilage loss that typically accompanies them, create an uneven joint surface. Instead of gliding smoothly, the joint surfaces scrape against each other.
Research using MRI imaging has confirmed that crepitus in the knee is strongly associated with the presence of bone spurs at both the inner and outer compartments of the joint, as well as under the kneecap. You might also feel the joint catch or lock briefly during certain movements, as if something is mechanically blocking it from completing its normal arc. This is different from the ache of inflammation. It’s a physical, almost mechanical sensation that can be startling the first time it happens.
What You Can Feel Through the Skin
In some locations, particularly the fingers and tops of the feet, bone spurs can grow large enough that you can feel them as firm, immovable bumps under the skin. The bony knobs that develop on finger joints in people with osteoarthritis are a common example. These bumps are hard to the touch, don’t move when you press them (because they’re part of the bone), and may or may not be tender. In the feet, a spur on the top of the foot can rub against the inside of a shoe, creating irritation and soreness at the skin surface even if the joint itself isn’t particularly painful.
How Bone Spurs Are Confirmed
Because the symptoms of bone spurs overlap heavily with tendinitis, arthritis flares, pinched nerves from disc problems, and other conditions, you can’t diagnose a bone spur by feel alone. Standard X-rays are the primary tool used to confirm whether a spur is present and are considered the gold standard for identifying them. An MRI may be ordered if your doctor suspects the spur is compressing a nerve or damaging soft tissue, since X-rays only show bone. The key diagnostic question isn’t just whether a spur exists, but whether it’s actually responsible for the symptoms you’re experiencing, since so many spurs are incidental findings that aren’t causing any trouble at all.
