Breasts look noticeably deflated immediately after implant removal, but this initial appearance is temporary. Over the following months, the skin gradually retracts and breast tissue settles into a more natural shape. The final result depends heavily on how large your implants were, how long they were in place, and how elastic your skin is.
The First Few Weeks
Right after surgery, expect your breasts to look flat, loose, and somewhat empty. The skin that had stretched to accommodate the implant is now draped over a smaller volume of natural tissue, creating visible laxity and wrinkling. Swelling from the procedure itself can mask some of this deflation initially, but as swelling subsides over the first week or two, the “deflated” look often becomes more pronounced before it gets better.
Between two and three weeks post-surgery, breasts typically begin to regain some volume as your natural tissue responds. This early phase can feel discouraging if you’re comparing what you see in the mirror to what you expected, but the breast is still in the very beginning of a long reshaping process.
How Breasts Change Over 3 to 12 Months
Skin retraction is the biggest factor in how your breasts ultimately look, and it happens in stages. The most noticeable tightening occurs within the first six weeks. After that, retraction continues more gradually through three to six months, which is when most women notice their breasts taking on a more natural, settled shape.
Final results typically become visible around the one-year mark, once the skin has fully retracted and breast tissue has settled into its new position. Some women continue to see subtle changes for up to 12 to 18 months. The process requires patience: what your breasts look like at week four is not what they’ll look like at month eight.
What Determines Your Final Result
Three factors matter most. First, implant size: larger implants stretch the skin more, and that stretched skin has a harder time bouncing back after removal. A woman who had 250cc implants will generally see better retraction than someone who had 500cc or larger. Second, duration: implants that were in place for 15 years created more sustained stretching than those in place for five. Third, skin elasticity, which is influenced by age, genetics, sun exposure, and smoking history. Younger skin with good collagen simply retracts better.
Your natural breast tissue also plays a role. Some women had very little tissue before augmentation, and implants don’t create new breast tissue. If you started with an A cup and had large implants for a decade, you may end up smaller than your original size because the tissue can thin over time under the pressure of the implant. Women who had more natural tissue before augmentation tend to retain a fuller shape afterward.
Sagging and Skin Laxity
Some degree of sagging is extremely common after explant surgery. Implants stretch the breast skin and the ligaments that provide structural support. Once the implant is removed, gravity acts on skin that no longer has the elasticity it once did. This sagging, combined with natural aging and any weight changes that occurred while implants were in place, means most women will notice their breasts sitting lower on the chest than they did before augmentation.
The nipple position often shifts as well. With implants, the nipple may have pointed forward or slightly upward. After removal, the lower pole of the breast can deflate while excess skin above the nipple creates a “waterfall” effect, where the breast tissue hangs below a downward-pointing nipple. This is one of the more noticeable cosmetic changes and a primary reason some women choose to combine removal with additional procedures.
Options if You’re Unhappy With the Result
In a study of 946 implant revision cases, over 60% of cosmetic patients who had implants permanently removed also opted for a breast lift to reshape the remaining tissue and reposition the nipple. A lift removes excess skin, tightens the breast envelope, and creates a more projected shape. It does add scars, typically around the areola and sometimes extending vertically or along the breast fold.
Fat transfer is another option that’s gaining popularity. A surgeon harvests fat from another area of your body (typically the abdomen, flanks, or thighs) and injects it into the breast to restore volume. The result tends to look and feel softer and more natural than implants. Surgeons typically inject a volume equal to or slightly larger than the removed implant, anticipating that some of the transferred fat won’t survive. The final volume stabilizes around four to six months. In comparative studies, both plastic surgeons and members of the general public rated fat transfer results higher than implant results for attractiveness, naturalness, symmetry, and femininity.
Some women choose removal alone with no additional procedures. Only about 6% of revision surgeries involve implant removal without any reconstruction, but for women whose primary goal is simply getting the implants out, many are satisfied with the result. In a study of 248 patients who had implants removed, 90.4% reported satisfaction at their first follow-up visit. That number gradually decreased over subsequent visits, settling around 77% by the fourth follow-up, suggesting that some women become less satisfied with the cosmetic result over time even as they feel better physically.
What to Realistically Expect
Your breasts after explant surgery will not look like they did before you got implants. They also won’t look as deflated at six months as they do at six days. The most realistic expectation is a breast that’s softer, smaller, and lower on the chest than it was with implants, with a shape that gradually improves over the course of a year. Women with smaller implants, shorter implant duration, and naturally elastic skin tend to get the best cosmetic outcomes from removal alone. Women with larger or older implants, or those over 50, are more likely to benefit from combining removal with a lift or fat transfer to achieve a shape they’re comfortable with.
