What Do Cavities Look Like? From White Spots to Holes

Cavities don’t always look like the dark holes you might picture. In their earliest stage, they appear as chalky white spots on the enamel, and many people walk past a mirror every day without noticing them. As decay progresses, those spots darken to yellow, brown, and eventually black, and the tooth surface may visibly break down. But the appearance varies quite a bit depending on where the cavity is, how deep it’s gone, and whether it’s still actively growing.

The Earliest Sign: White Spots

Before a cavity becomes a cavity, it starts as a white spot lesion. This is a patch of enamel that has lost minerals but hasn’t yet developed a hole. It looks opaque and chalky compared to the glassy sheen of healthy enamel around it. The white appearance comes from tiny pores that form inside the enamel as minerals dissolve. These pores scatter light differently, making the area look flat and dull instead of smooth and shiny.

Here’s the tricky part: the very earliest white spots are only visible when the tooth surface is dry. If saliva is covering the tooth, the spot can look completely normal. This is one reason your dentist blows air on your teeth during an exam. At this stage, the enamel surface is still intact, and the damage can actually reverse with fluoride and good oral hygiene. Once a white spot becomes visible even on a wet tooth, the demineralization is more advanced, though still potentially reversible.

How Color Changes as Decay Deepens

As a cavity progresses beyond the white spot stage, it picks up color. The pattern generally moves from white to brown to black, though not every cavity follows a neat sequence.

  • Light brown or yellow: Active decay that has moved past the initial white stage but hasn’t yet broken through the enamel surface. The discoloration comes from bacterial byproducts and mineral breakdown staining the porous enamel.
  • Dark brown: Decay has penetrated deeper, often reaching the layer beneath the enamel called dentin. You may notice a shadowy discoloration visible through the remaining enamel, almost like a bruise beneath the surface.
  • Black: This can mean one of two things. It may indicate extensive, long-standing decay. Or it can actually be a sign of arrested (inactive) decay, where the lesion has stopped progressing and hardened. Dark or black discoloration alone doesn’t tell you whether a cavity is active or not.

Once the enamel surface breaks down, you’ll see an actual hole or pit in the tooth. A shallow cavity might look like a small rough divot, while advanced decay can destroy half the tooth or more, exposing soft, discolored dentin underneath. At the most severe stage, the tooth structure is visibly crumbling.

Dark Shadows Under the Enamel

Some cavities don’t show an obvious hole at all. Instead, you might notice a grayish or dark shadow showing through otherwise intact-looking enamel. This happens when decay has reached the dentin layer below and is spreading there while the enamel shell above remains mostly whole. It looks like a dark area visible beneath the tooth’s surface, similar to how a bruise looks under skin.

These shadowed cavities are surprisingly hard to catch, even for professionals. In one study of teeth with dark shadows from underlying dentin decay, about two-thirds showed no abnormality on X-rays, and roughly 82% had some enamel breakdown that was easy to miss on casual inspection. This is a good reminder that a tooth can look mostly fine from the outside while decay is advancing underneath.

Cavities Between Teeth

The cavities you’re least likely to spot yourself are the ones that form between teeth, on the surfaces where neighboring teeth press together. These interproximal cavities are essentially hidden from view. You might eventually notice a dark shadow at the edge of a tooth, a rough spot you can feel with your tongue, or a place where floss consistently catches or shreds. But in many cases, the only way to detect them is with dental X-rays.

Research comparing visual exams to X-rays shows the two methods catch different things. In one study of teeth with existing fillings, about 16% of cavities found on X-rays had no visible signs at all during a visual exam. The takeaway: not all cavities are something you can see by looking in the mirror, especially between teeth.

Cavities Along the Gum Line and on Roots

If your gums have receded and exposed the root surfaces of your teeth, decay in those areas looks and feels distinctly different from cavities on the chewing surfaces. Root cavities tend to appear as yellowish or light brown patches along the gum line. The texture is the real giveaway: active root decay feels soft or leathery rather than hard and smooth.

Root surfaces don’t have the protective enamel layer that covers the crown of the tooth, so they’re more vulnerable and decay can spread faster there. An inactive root cavity that has stopped progressing typically turns dark brown or black and feels hard and smooth to the touch. If you notice a soft, discolored area near your gum line, that’s worth getting checked promptly.

Staining vs. Actual Decay

One of the most common sources of confusion is telling the difference between harmless staining and a real cavity, particularly in the grooves on your back teeth. Coffee, tea, and certain foods can stain the natural pits and fissures on molars, turning them dark brown or black. This looks alarming but doesn’t necessarily mean decay.

The distinction matters: noncariogenic staining is just color deposited on a sound, hard surface, while true decay involves mineral loss and bacterial damage creating a porous, weakened area. In a study of 62 teeth with suspicious-looking discolorations in their grooves, about 19% turned out to be harmless staining with no actual decay underneath. The rest had real enamel or dentin cavities. Visually, the two can look nearly identical, which is why dentists use tools like X-rays, probes, or fluorescence imaging rather than relying on color alone.

A few practical clues can help you gauge what you’re seeing at home. Staining tends to sit in narrow grooves and feels smooth and hard if you run your tongue over it. Active decay is more likely to feel rough, sticky, or soft. Staining doesn’t usually cause sensitivity, while a cavity that’s reached the dentin layer often does, particularly with cold, sweet, or hot foods. But none of these self-checks are reliable enough to replace an actual dental exam, especially since many cavities hide in places you simply can’t see or feel.
