Cavities change in appearance as they progress, starting as faint white spots on the enamel and eventually becoming visible holes with dark brown or black discoloration. Catching them early means knowing what each stage actually looks like on your teeth.
The Earliest Stage: White Spots
Before a cavity becomes a hole, it starts as a chalky white patch on the tooth surface. This happens because minerals are leaching out of the enamel, creating microscopic pores that scatter light differently than healthy tooth structure. The spot looks opaque and dull compared to the natural gloss of surrounding enamel. In many cases, these white spot lesions are only visible after the tooth surface has been dried, which is one reason dentists blow air on your teeth during exams.
At this point, the surface of the enamel is still intact. There’s no hole, no roughness you can catch with your tongue. The damage is happening beneath a thin mineralized outer layer. This is the one stage where the process can actually be reversed with fluoride and improved oral hygiene, re-depositing minerals back into the weakened enamel before it breaks down further.
Brown and Dark Spots
If demineralization continues, that white spot darkens to a yellowish brown, then deeper brown or gray. The color change reflects both the structural breakdown of enamel and the absorption of pigments from food, drinks, and bacteria. At this stage, the surface may start to feel slightly rough or sticky when you run your tongue over it.
Once decay pushes through the enamel and reaches the softer layer underneath (dentin), the discoloration often appears as a dark shadow visible through the remaining enamel. You might notice a grayish or brownish area that looks like it’s coming from inside the tooth rather than sitting on the surface. This shadow effect is especially common on the chewing surfaces of back teeth, where decay can enter through the natural grooves and pits and then spread wider in the dentin below.
Visible Holes and Breakdown
At more advanced stages, the enamel collapses and a physical hole forms. This can range from a small pit you can feel with your tongue to a large cavity exposing the inner tooth structure. The edges of the hole are often dark brown or black. The decayed material inside may feel soft or mushy if you press on it. Some cavities on the chewing surface look like a dark line running along a groove, while others on the smooth sides of teeth appear as crescent-shaped or round dark spots.
In severe cases, more than half the tooth surface can be destroyed. The remaining tooth structure may chip or fracture during normal chewing because it’s been hollowed out from the inside. By this point, pain is usually constant rather than intermittent, and the tooth is sensitive to hot, cold, and sweet foods.
Cavities Between Teeth
Some of the hardest cavities to spot are the ones that form on the surfaces where two teeth touch. You can’t see these directly in a mirror, which is why they often go undetected until they’re fairly advanced. The clues are subtle: a faint shadow or dark discoloration visible at the edges where the teeth meet, or a grayish translucency you notice while flossing. In later stages, you might feel a small indentation or notch on the side of a tooth with your tongue or fingernail, which means the decay has already reached the dentin.
Floss can also give you indirect information. If floss consistently shreds or catches in the same spot, the enamel surface there may be rough from early decay.
How Cavities Look in Children
Cavities in baby teeth follow a recognizable pattern, especially in toddlers who fall asleep with a bottle or sippy cup. The upper front teeth are hit first and hardest because they erupt early and sit in direct contact with pooling liquid during feeding. The lower front teeth are typically spared because the tongue shields them and saliva from nearby glands washes over them.
The earliest sign is a white chalky band running along the gum line of the upper front teeth. Parents often miss this because it blends with the naturally pale color of baby teeth. Without treatment, those bands darken into brown or black rings that encircle the necks of the teeth. In severe cases, the visible crowns of the upper front teeth can be completely destroyed, leaving only root stumps at the gum line while the lower front teeth remain healthy.
Cavity or Just a Stain?
Dark spots on teeth aren’t always cavities, and telling the difference can save you unnecessary worry. A few reliable distinctions help:
- Location pattern: Stains tend to affect broad areas or multiple teeth uniformly, often from coffee, tea, or tobacco. Cavities are typically isolated spots, concentrated in grooves, between teeth, or along the gum line.
- Texture: Stains sit on a smooth, intact surface. Cavities feel rough, sticky, or soft. If you can catch a spot with your fingernail or tongue, it’s more likely decay.
- Persistence: Surface stains can lighten or shift after brushing or a professional cleaning. A cavity won’t improve on its own and will only get darker or larger over time.
- Accompanying symptoms: Stains don’t cause pain or sensitivity. If a dark spot comes with new sensitivity to cold drinks, hot food, or sweets, decay is the more likely cause.
- Holes: This one is straightforward. Stains never create holes in teeth. Any visible pit, crater, or break in the tooth surface is a cavity.
Brown staining in the grooves of molars is especially common and frequently mistaken for decay. If the surface is hard and smooth when probed, it’s likely just pigment trapped in the natural anatomy of the tooth. If it’s soft or catches an instrument, it’s active decay. Your dentist can distinguish between the two in seconds with a dental explorer or by taking a bitewing X-ray to check for hidden damage beneath the surface.
