A high triglyceride reading on a blood test means your body has more fat circulating in your bloodstream than is considered healthy. Triglycerides are the most common type of fat in your body, and levels at or above 200 mg/dL are classified as high. While triglycerides themselves are a normal and necessary energy source, elevated levels signal increased risk for heart disease and, at very high concentrations, a painful condition called acute pancreatitis.
What Triglycerides Actually Are
Triglycerides are molecules made of glycerol bonded to three fatty acids. They are your body’s primary form of long-term energy storage, yielding more than twice the energy per unit of mass compared to carbohydrates or protein. When you eat more calories than your body needs right away, the excess is converted into triglycerides and stored in fat cells. Between meals or during periods of low food intake, your body breaks those triglycerides back down into fatty acids and glycerol, which are then burned for fuel inside your cells’ mitochondria.
A triglyceride blood test captures how much of this fat is traveling through your bloodstream, typically packaged inside particles called lipoproteins. The test is usually part of a standard lipid panel alongside LDL cholesterol, HDL cholesterol, and total cholesterol.
Understanding Your Numbers
Triglyceride levels are measured in milligrams per deciliter (mg/dL) and fall into four categories:
- Normal: below 150 mg/dL
- Borderline high: 150 to 199 mg/dL
- High: 200 to 499 mg/dL
- Very high: 500 mg/dL or above
Your doctor may have asked you to fast for 9 to 12 hours before the blood draw, since eating a meal can temporarily spike triglyceride levels. If you weren’t fasting, your result may read higher than your true baseline, and your provider might ask you to retest.
Why High Triglycerides Are a Problem
Triglycerides on their own don’t directly clog your arteries the way LDL cholesterol does. But when your triglyceride levels are elevated, your blood carries more of the fat-rich particles that transport them. As those particles are broken down, they leave behind smaller, cholesterol-packed remnants that can penetrate artery walls and trigger the same kind of plaque buildup that LDL causes. These remnant particles promote inflammation in blood vessel walls, increase the stickiness of immune cells that accumulate in plaques, and interfere with your blood vessels’ ability to relax and dilate normally.
High triglycerides also reduce the protective function of HDL, the so-called “good” cholesterol. Normally, HDL helps pull excess cholesterol out of artery walls. When triglyceride-rich particles are abundant, they impair HDL’s ability to do that job. The combined effect is a meaningful increase in cardiovascular risk, even when your LDL cholesterol looks acceptable.
Pancreatitis Risk at Very High Levels
When triglycerides climb above 1,000 mg/dL, the risk of acute pancreatitis, a sudden and severe inflammation of the pancreas, is roughly 5%. At levels above 2,000 mg/dL, that risk jumps to 10 to 20%. Pancreatitis causes intense abdominal pain, often requires hospitalization, and can be life-threatening. This is why very high readings are treated more urgently than borderline ones.
Common Causes of Elevated Triglycerides
Diet is the most direct lever. Sugary foods and starchy carbohydrates like white bread, pasta, rice, and cereal are broken down into sugars that your liver converts into triglycerides when consumed in large quantities. Alcohol has a particularly strong effect because it stimulates the liver to produce more triglycerides while also adding extra calories that contribute to the problem. Even limiting fat intake too aggressively can backfire: people who cut fat drastically often compensate by eating more carbohydrates, which raises triglycerides.
Several medical conditions also drive levels up. Uncontrolled type 1 or type 2 diabetes is one of the most common secondary causes, especially when blood sugar management is poor. Obesity raises triglycerides by reducing the efficiency of enzymes that clear fat from your blood and by stimulating your liver to produce more fat-carrying particles. Hypothyroidism and kidney disease can both cause elevated triglyceride readings, sometimes as the first sign of those conditions. If your levels are high and you eat reasonably well, your doctor may check for one of these underlying causes.
Genetics play a role too. Some people inherit conditions that make their bodies overproduce triglycerides or clear them slowly, leading to persistently high levels regardless of diet. These familial forms often require medication.
How High Triglycerides Are Managed
For borderline or moderately high levels, lifestyle changes are the first line of treatment. Reducing refined carbohydrates and added sugars, cutting back on alcohol, losing even a modest amount of weight, and increasing physical activity can each lower triglycerides meaningfully on their own. Combined, the effect is significant enough that many people can bring their levels below 150 mg/dL without medication.
When levels stay elevated despite those changes, or when cardiovascular risk is already high, medication enters the picture. Your doctor may recommend a statin, which primarily targets LDL cholesterol but also improves the overall lipid profile. For very high triglycerides (above 500 mg/dL), prescription omega-3 fatty acids at a dose of 4 grams per day can lower triglycerides by 20% to 30%. These are distinct from over-the-counter fish oil supplements, which are not FDA-reviewed for effectiveness and are not recommended as a substitute. The AHA has specifically cautioned against trying to self-treat high triglycerides with store-bought supplements.
What to Do With Your Results
A single high reading doesn’t necessarily define your health, but it does warrant attention. If your test was non-fasting, a repeat fasting test gives a clearer picture. If the number is confirmed above 150 mg/dL, think first about what you eat and drink regularly: sugary beverages, alcohol, and large portions of starchy foods are the most impactful targets to change. If your levels are above 500 mg/dL, that reading is urgent enough to discuss with your doctor promptly because of the pancreatitis risk.
Triglyceride levels respond relatively quickly to changes in diet and activity compared to other blood markers. Many people see noticeable improvement within a few weeks of sustained changes, making it one of the more responsive numbers on a lipid panel.
