Licensed practical nurses (LPNs) provide hands-on patient care under the direction of registered nurses or physicians. How LPNs Differ From RNs
The biggest distinction between an LPN and a registered nurse is clinical judgment. RNs are responsible for tasks that require independent nursing assessment and decision-making: performing initial admission assessments, creating care plans, evaluating those plans, and teaching patients about managing their conditions. LPNs cannot be delegated any activity that requires clinical reasoning or critical decision-making in the nursing sense.
That doesn’t mean LPNs work without thinking. They use significant skill and training in their daily tasks. But legally, they operate under the direction of an RN, physician, nurse practitioner, physician assistant, or other qualified practitioner. The supervising professional is ultimately accountable for the care that’s delegated. In practice, this means an RN or doctor sets the care plan, and the LPN carries out the specific interventions and reports back on how the patient is responding.
Where LPNs Work
Nursing homes and long-term care facilities employ a large share of LPNs. In these settings, LPNs manage medication distribution for residents, monitor chronic conditions like diabetes and heart failure, assist with wound care, and ensure residents follow their prescribed treatment plans. The complexity of caring for aging populations with overlapping health issues keeps demand for LPNs consistently high in these facilities.
LPNs also work in hospitals, physician offices, outpatient clinics, and home health care. Home health is a growing area: for patients recovering from surgery or managing a chronic illness at home, an LPN visits regularly to handle medication management, wound care, and ongoing health monitoring. The work environment shapes the daily routine significantly. An LPN in a busy hospital may focus on post-surgical vitals and IV monitoring, while one in a pediatric clinic might spend most of the day giving immunizations and recording growth measurements.
State-by-State Scope Differences
What an LPN is allowed to do varies meaningfully by state. Each state’s Nurse Practice Act defines the legal scope of practice, and individual employers can add further restrictions on top of that. IV therapy is one of the clearest examples of how these rules differ.
In Wyoming, for instance, LPNs with a basic IV therapy certification can initiate peripheral IV fluids, calculate and maintain flow rates, and administer certain IV medications like antibiotics and electrolyte solutions. But they’re prohibited from administering IV push medications, programming pain pumps, giving blood products, or working with central venous lines. An advanced IV certification opens up additional tasks like mixing and labeling IV medications. In other states, LPNs may not be permitted to start IVs at all. Education and Licensing

If you don’t pass on the first attempt, you can retake the exam after 45 days.

Compared to the two to four years required for an RN degree, a one-year program offers a faster entry point into nursing.
