What Do Probiotics Do for You? Benefits & Side Effects

Probiotics are live microorganisms that support your health primarily by strengthening your gut, where they help digest food, train your immune system, and crowd out harmful bacteria. Most people associate them with digestive benefits, but their influence extends to immune function, skin health, and possibly even mood. Here’s what the evidence actually shows.

How Probiotics Work in Your Gut

Your digestive tract houses trillions of microorganisms that collectively help you break down food, produce vitamins, and synthesize essential amino acids your body can’t make on its own. Probiotics reinforce this existing community in several ways. They compete with harmful bacteria for space and resources along your intestinal lining, making it harder for pathogens to gain a foothold. They also produce short-chain fatty acids by breaking down fiber and other complex carbohydrates that your stomach and small intestine can’t process. These fatty acids lower the pH inside your colon, creating an environment that favors beneficial bacteria and discourages harmful ones.

Probiotics also help maintain the physical barrier of your intestinal wall. When that barrier weakens, partially digested food particles and toxins can slip through and trigger inflammation. By supporting the tight junctions between intestinal cells, probiotics help keep the contents of your gut where they belong.

Digestive Relief, Especially for IBS

The strongest evidence for probiotics centers on digestive conditions. For people with irritable bowel syndrome, specific strains have been shown to meaningfully reduce abdominal pain. A systematic review in The Lancet’s eClinicalMedicine identified four probiotic strains that significantly improved pain relief in IBS patients, with some strains nearly five times more likely to provide relief compared to placebo. The key word there is “specific strains.” Not all probiotics work the same way, and the benefits are tied to particular organisms at particular doses, not to probiotics as a general category.

Beyond IBS, probiotics are well supported for preventing diarrhea caused by antibiotics. Antibiotics kill bacteria indiscriminately, wiping out beneficial gut microbes alongside the ones causing your infection. A meta-analysis of 17 studies covering over 3,600 patients found that taking a probiotic during antibiotic treatment reduced the risk of antibiotic-associated diarrhea by about 51%. The two most effective strains were Lactobacillus rhamnosus GG, which cut the risk by roughly 71%, and Saccharomyces boulardii, which reduced it by about 59%. Starting your probiotic at the same time you begin antibiotics appears to be important for this benefit.

Immune System Support

About 70% of your immune activity is concentrated in and around your gut, so it makes sense that the bacteria living there influence how your immune system behaves. Probiotics strengthen one of your body’s first lines of defense by boosting production of a protective antibody called secretory IgA, which coats the intestinal lining and helps neutralize pathogens before they can cause infection.

Probiotics also help regulate the balance between inflammatory and anti-inflammatory signals in your body. Your immune system needs inflammation to fight infections, but too much of it causes tissue damage and chronic disease. Probiotic bacteria help calibrate this balance by influencing the chemical messengers (cytokines) that your immune cells produce. This gut-stabilizing effect is one reason probiotics are studied for conditions far beyond the digestive tract.

Skin Health and Atopic Dermatitis

The connection between gut bacteria and skin is well enough established that researchers now talk about a “gut-skin axis.” The most convincing evidence involves atopic dermatitis (eczema). A systematic review in the Journal of the American Academy of Dermatology examined 15 studies covering 874 patients who applied probiotic-containing solutions to their skin for periods ranging from four days to 16 months. Every study found improvements in symptom severity, quality of life, and frequency of flare-ups. Oral probiotics have also shown promise for eczema, particularly in children, though the topical evidence is currently stronger and more consistent.

Mood and the Gut-Brain Connection

Your gut and brain communicate constantly through the vagus nerve, hormones, and immune signals. Chronic stress can disrupt this communication by driving overproduction of cortisol, your primary stress hormone, which in turn promotes inflammation and further dysregulates the stress response. Probiotics that target this pathway are sometimes called “psychobiotics.”

The honest picture here is mixed. A large systematic review found that out of 32 clinical trials examining probiotics for depressive symptoms, only seven reported significant antidepressant effects. Some strains showed real benefits, but results varied widely depending on the specific organism used. The current evidence isn’t strong enough to recommend probiotics as a standalone treatment for depression or anxiety, but it does suggest that certain strains may play a supporting role, particularly for people whose mood issues coincide with digestive problems.

Food Sources vs. Supplements

You can get probiotics from fermented foods like yogurt, kefir, kimchi, sauerkraut, and some cheeses. The fermentation microbes in these foods are typically still alive when you eat them, meaning they deliver live bacteria to your gut along with nutrients. However, fermented foods are not automatically probiotic. Some products are pasteurized, baked, or filtered after fermentation, which kills the microbes. Sourdough bread and most commercially bottled kombucha, for example, contain few or no live organisms by the time they reach you.

Supplements offer more precision. They contain defined strains at measured doses, which matters when you’re targeting a specific condition like antibiotic-associated diarrhea or IBS. Fermented foods, on the other hand, often contain complex, uncharacterized mixtures of microbes that may provide broad benefits but aren’t matched to clinical evidence the way individual supplement strains are. The most important factor isn’t food versus pill. It’s whether you’re getting an effective strain at an effective dose.

Dosage: How Much Do You Need?

Probiotics are measured in colony-forming units, or CFUs, which indicate the number of viable bacteria per dose. Most supplements contain 1 to 10 billion CFU, though some pack 50 billion or more. Higher numbers don’t necessarily mean better results. The optimal dose depends entirely on the strain and the condition you’re trying to address.

For preventing antibiotic-associated diarrhea, the evidence points to roughly 10 to 20 billion CFU per day of Lactobacillus rhamnosus GG or Saccharomyces boulardii. A European pediatric health group recommends at least 5 billion CFU per day for children on antibiotics. For general digestive maintenance, doses in the 1 to 10 billion range are typical in studies showing benefit. The NIH’s Office of Dietary Supplements recommends choosing products that match the specific strains and doses tested in clinical research rather than chasing the highest CFU number on the shelf.

When and How to Take Them

Stomach acid destroys most probiotic bacteria before they reach the lower intestine, where they do their work. Taking your probiotic with a meal that contains carbohydrates, fat, and protein gives the bacteria the best chance of surviving the trip. Food raises the pH in your stomach, creating a less acidic environment. Milk and yogurt are particularly good pairings because they contain all three macronutrients.

Avoid taking probiotics alongside acidic foods and drinks like coffee, orange juice, or tomato sauce, which add extra acid to your stomach and reduce bacterial survival. Morning, paired with breakfast, tends to be the most practical and effective time for most people.

Side Effects and Safety

For most healthy adults, probiotics cause nothing worse than temporary gas and bloating as your gut adjusts to the new bacterial population. These symptoms typically resolve within a few days. The more significant safety concern applies to people with weakened immune systems, including those on immunosuppressant medications, people with critical illnesses, and premature infants. In these groups, there’s a small risk that a probiotic product could introduce harmful microbes alongside beneficial ones, potentially causing a serious infection. If you fall into one of these categories, the risk-benefit calculation is different than it is for a generally healthy person.
