Registered nurses are the largest group of healthcare professionals in the United States, and their core job is managing patient care from start to finish. That includes everything from taking vital signs and administering medications to coordinating treatment plans across an entire medical team. The median annual wage for RNs was $93,600 in May 2024, and employment is projected to grow 5 percent from 2024 to 2034.
How RNs Approach Patient Care
Every decision an RN makes follows a structured five-step process: assessment, diagnosis, planning, implementation, and evaluation. This isn’t a one-time checklist. It’s a continuous cycle that repeats throughout a patient’s stay or course of treatment.
Assessment comes first. The nurse collects both subjective data (what the patient describes, like pain level or nausea) and objective data (measurable information like blood pressure, weight, temperature, and fluid intake). From there, the nurse forms a nursing diagnosis, which is a clinical judgment about the patient’s response to a health problem. This is distinct from a medical diagnosis. A doctor might diagnose pneumonia, while the nurse’s diagnosis addresses the patient’s difficulty breathing or risk of dehydration related to that pneumonia.
Next comes planning. The nurse sets specific, measurable goals tailored to the individual patient and builds a care plan around them. During implementation, the nurse carries out the plan: administering medications, applying monitoring equipment, performing treatments, and directing tasks to licensed practical nurses or nursing assistants when appropriate. Finally, evaluation closes the loop. The nurse reassesses the patient to determine whether the goals are being met and adjusts the plan if they’re not. In a fast-changing situation, like a post-surgical patient whose vitals shift, this reassessment can happen many times within a single shift.
Day-to-Day Clinical Tasks
On a practical level, RNs spend their shifts doing a wide range of hands-on work. Medication administration is one of the most time-intensive responsibilities. Before giving any medication, nurses verify specific safety checks, often called the “rights” of medication administration, confirming the right patient, drug, dose, route, and time. A single nurse in a hospital unit may administer dozens of medications across multiple patients in one shift.
Beyond medications, RNs monitor patients continuously. They track vital signs, manage IV lines, change wound dressings, assist with mobility, educate patients and families about their conditions, and document everything in the medical record. Documentation is a significant part of the job. It creates the legal record of care and ensures that every provider on the team has accurate, up-to-date information.
The Coordinator Role
One of the less visible but most critical things RNs do is coordinate care across multiple specialists and departments. In complex cases like cancer treatment, the nurse acts as the anchor that guarantees continuity throughout the entire healthcare process, including follow-up. That means scheduling referrals so patients don’t face unnecessary delays, linking patients who need multiple types of treatment with physicians from different departments, and adapting the schedule at every step based on the patient’s evolving needs.
Nurses also activate support services based on what each patient requires: nutritional assessments, swallowing and speech evaluations, dental care, psychosocial support, rehabilitation, and palliative care. When financial barriers arise, the nurse connects the patient with a social worker who can help access resources. This case management role, systematically coordinating care for patients and families navigating a complex illness, is a defining feature of what RNs do that often goes unrecognized.
How RNs Differ From LPNs
Licensed practical nurses (LPNs) provide direct patient care, but their scope is narrower. LPNs typically work under the direction of an RN or physician and are expected to consult with an RN when a patient becomes unstable or requires more complex care. RNs can function as independent practitioners or team members in all clinical settings, while LPNs generally work in a team member role only. RNs also carry responsibility for developing and modifying care plans, making higher-level clinical judgments, and supervising the work delegated to LPNs and nursing assistants.
Where RNs Work
Hospitals are the most common workplace, but RNs practice in a wide range of settings: outpatient clinics, physician offices, home health agencies, nursing facilities, schools, public health departments, and insurance companies. The setting shapes the work considerably. A hospital nurse on a medical-surgical floor juggles multiple acutely ill patients, while a clinic nurse may focus on chronic disease management, patient education, and preventive screenings. Home health nurses assess patients in their own environments and often work with more independence.
Most hospital nurses work 12-hour shifts. One large study of over 22,000 nurses found that 65 percent worked shifts of 12 to 13 hours, while about 26 percent worked 8 to 9 hour shifts. The 12-hour model means many full-time hospital nurses work three days per week, though the physical and mental demands of those shifts are considerable.
Why Staffing Levels Matter
The number of RNs on a hospital unit has a direct, measurable effect on whether patients survive. A three-year study at a Swiss university hospital analyzed data from nearly 80,000 patients and found that shifts with higher registered nurse staffing had 8.7 percent lower odds of patient mortality. Shifts with low staffing, conversely, were associated with 10 percent higher odds of death. These aren’t abstract numbers. They reflect the reality that when nurses are stretched too thin, assessments get delayed, subtle changes in a patient’s condition go unnoticed, and errors become more likely.
Specialization Options
After gaining experience, many RNs pursue advanced practice roles that require a master’s or doctoral degree. Family nurse practitioners provide primary care across all age groups and, depending on the state, practice with authority similar to a primary care physician. Psychiatric nurse practitioners assess and diagnose mental health conditions and may prescribe medication. Women’s health nurse practitioners manage reproductive care, fertility concerns, and gynecological conditions. Acute care nurse practitioners treat patients with critical, chronic, or complex conditions, often in hospital intensive care units or specialty departments.
Even without an advanced degree, RNs can specialize at the bedside. Emergency nurses, labor and delivery nurses, oncology nurses, pediatric nurses, and operating room nurses all develop deep expertise in their areas. These specializations typically involve certification exams and ongoing education but don’t require additional degrees.
How to Become an RN
There are two main educational paths. An associate degree in nursing (ADN) takes about two years, while a bachelor of science in nursing (BSN) takes four. Both qualify graduates to sit for the NCLEX-RN, the national licensing exam required in every state. Passing the NCLEX-RN is what grants the legal authority to practice as a registered nurse. Many hospitals now prefer or require a BSN, and nurses who start with an associate degree often complete a bridge program to earn their bachelor’s while working.
Graduates from international nursing programs can also pursue U.S. licensure, provided their education is verified by an independent credentials review agency and they pass the NCLEX-RN. Licensing requirements vary slightly by state, but the exam itself is standardized nationwide.
