What Do Teeth Cavities Look Like at Every Stage?

Cavities don’t always look like obvious holes in your teeth. In their earliest stage, they appear as chalky white spots on the enamel surface. As decay progresses, those spots darken through a predictable sequence of colors, from light brown to yellow-gray to dark brown or black, eventually forming a visible pit or hole.

The Earliest Sign: White Spots

Before a cavity becomes a hole, it starts as a flat, opaque white patch on the tooth surface. This happens because acids from bacteria dissolve minerals out of your enamel, making it more porous. The porous enamel scatters light differently than healthy tooth structure, creating a chalky, matte appearance that lacks the normal glossy sheen of your teeth.

These white spot lesions can be tricky to notice. They’re often only visible after the tooth surface has been dried, meaning you might spot them after wiping your teeth with a tissue but not when your mouth is wet with saliva. At this stage, no actual hole has formed. The enamel surface is still intact, just weakened. This is the one point where decay can sometimes be reversed with fluoride and improved oral hygiene, because the mineral structure hasn’t been physically broken yet.

How the Color Changes as Decay Grows

Once a white spot progresses, the color shifts follow a fairly reliable pattern. Light brown spots appear as the weakened enamel starts to break down further and trap staining from food and bacteria. This is still early decay, but it’s moving in the wrong direction.

As the outer enamel layer erodes enough to expose the softer layer underneath (called dentin), the spot may take on a yellow or grayish tone. Dentin is naturally darker than enamel, so what you’re seeing is the inner tooth showing through a thinning outer shell. At this point, you might also notice a shadowy appearance around the area, almost like a bruise beneath the surface, even if the outer enamel hasn’t fully collapsed yet.

In more advanced decay, the color deepens to dark brown or black. This is bacteria and acids working through the dentin, enlarging the cavity from the inside. By this stage, you can often see an actual hole or pit in the tooth, and the dark discoloration tends to be concentrated in one spot rather than spread across the whole tooth surface.

Where Cavities Form Changes How They Look

Cavities on chewing surfaces tend to be the easiest to spot yourself. Your back molars have natural grooves and pits on top, and food gets trapped in these crevices easily. Decay here often looks like a dark line or dot sitting in one of those grooves. You might mistake it for a stain at first, but if the discoloration stays put and doesn’t change with brushing, it’s worth having checked.

Cavities between teeth are a completely different story. These are largely invisible to the naked eye, even when they’re significant. A study comparing visual examination to X-rays found that X-rays detected 71 percent more cavities on the surfaces between teeth than a dentist could find just by looking. This is the main reason dental offices take periodic X-rays. The decay hides in the contact point where two teeth press together, and by the time you can see it from the outside, it’s often already well into the dentin.

Cavities along the gumline form on exposed root surfaces, which are softer than enamel and decay faster. These tend to appear as dark spots or shallow pits right where the tooth meets the gum tissue. They can be hard to distinguish from tartar buildup, which also creates a yellowish or brownish deposit in the same area.

Cavities vs. Stains

Not every dark spot on a tooth is a cavity, and telling the difference matters. A few visual clues can help you sort one from the other before your next dental visit.

  • Location pattern: Stains tend to affect broad areas or multiple teeth at once, especially along the gumline or across the front surfaces. A cavity is typically a single, localized spot on one tooth.
  • Persistence: Stains from coffee, tea, or tobacco can lighten or shift with brushing or professional cleaning. A cavity doesn’t go away. If a dark spot stays in the same place week after week, that’s more concerning.
  • Surface texture: Run your tongue over the spot. Stains sit on a smooth tooth surface. A cavity may feel rough, sticky, or like a small catch or dip in the enamel.
  • Holes: Any visible pit or hole is a cavity. Stains don’t create physical openings in the tooth.

What You Can Feel, Not Just See

Cavities often announce themselves through sensation before they’re clearly visible. New sensitivity to hot, cold, or sweet foods and drinks is one of the most common early signs, caused by enamel breaking down enough to expose the more sensitive layers beneath. You might notice a sharp twinge when drinking ice water or biting into something sugary that you didn’t have before.

Pain patterns also change over time. Early on, discomfort comes and goes, sometimes disappearing for days. As the cavity deepens, the pain becomes more frequent and eventually constant. If a tooth that was occasionally sensitive starts hurting without any trigger, the decay has likely reached the inner pulp of the tooth, which contains nerves and blood vessels.

Bad breath or a persistent unpleasant taste in one area of your mouth can also signal decay, since the bacteria breaking down your tooth structure produce sulfur compounds as a byproduct.

Why Some Cavities Are Impossible to See Yourself

Dentists use a standardized scale that rates decay from 0 (healthy) to 6 (extensive destruction), and several of those middle stages involve damage that’s genuinely invisible without professional tools. At the mildest level, the white spot only appears on a dried tooth surface. One step up, a color change is visible on a wet tooth but there’s no hole yet. At the next stage, decay has reached the dentin layer but only shows as a dark shadow beneath intact enamel, something a dentist might catch with a bright light or X-ray but you’d never notice in your bathroom mirror.

This is especially true for the surfaces between your teeth, where the vast majority of hidden cavities develop. The agreement between what dentists can see with their eyes and what X-rays reveal is statistically poor, with a study in caries-active adults showing only slight overlap between the two methods. In practical terms, this means even trained professionals miss cavities without imaging. You at home, with no dental light or mirror angles, will miss even more. Regular X-rays aren’t just routine protocol. They catch decay in locations where visual inspection simply can’t.
