Hives can be triggered by a surprisingly long list of things, from foods and medications to temperature changes, stress, and even exercise. The raised, itchy welts appear when certain immune cells in your skin release a flood of inflammatory chemicals, most notably histamine. What sets off that release varies widely from person to person, and sometimes the cause is never identified. Here’s a breakdown of the most common culprits.

How Hives Form in Your Skin

Your skin contains immune cells called mast cells. When something activates them, they burst open and dump histamine and other inflammatory chemicals into the surrounding tissue. Histamine makes tiny blood vessels leak fluid into the skin, which creates the characteristic raised, red, itchy welts. This activation can happen through a true allergic reaction (where your immune system produces antibodies against a specific trigger) or through non-allergic pathways where irritants activate mast cells directly. That’s why the list of possible triggers is so broad.

Foods

Food allergies are one of the most recognizable causes of hives, and symptoms typically appear within two hours of eating. The nine most common food allergens are peanuts, tree nuts (almonds, walnuts, cashews, pistachios, pecans, hazelnuts), milk, eggs, fish, shellfish, soy, wheat, and sesame. In some people, the reaction is mild and limited to skin welts. In others, it can progress to more serious symptoms involving the throat, breathing, or blood pressure.

Not all food-related hives involve a true allergy, though. Some foods contain compounds that stimulate mast cells directly. Strawberries, tomatoes, and fermented or aged foods (like aged cheese and wine) are common examples. This type of reaction tends to be dose-dependent, meaning a small amount might be fine while a large serving causes problems.

Medications

Antibiotics and anti-inflammatory painkillers (NSAIDs like ibuprofen and diclofenac) are the two drug classes most frequently linked to hives. Among antibiotics, certain types of penicillin-family and cephalosporin drugs are the most common offenders, with amoxicillin, ceftriaxone, and cephalexin topping the list. Aspirin can also trigger hives in susceptible people. Drug-induced hives can appear minutes to hours after taking a dose, or even after you’ve taken the same medication without problems before.

Things That Touch Your Skin

Some substances cause hives right where they make contact with your skin. Natural rubber latex is one of the best-known triggers, particularly for healthcare workers, dental workers, and anyone who wears latex gloves regularly. Nickel, found in jewelry, belt buckles, and phone cases, is another. Raw meat, fish, and certain vegetables can cause contact hives in food workers and home cooks.

Cosmetics and skin products are a less obvious source. Preservatives like benzoic acid and sorbic acid, fragrances containing cinnamon-derived compounds, and ingredients like parabens and polyethylene glycol can all activate mast cells on contact. The welts usually stay localized to the area of skin that was exposed, though widespread reactions are possible with repeated or heavy exposure.

Physical and Environmental Triggers

Your body’s physical environment can trigger hives even when no allergen is involved. These are sometimes called “physical urticarias,” and they include:

  • Heat and sweating: A rise in core body temperature from exercise, a hot bath, fever, or even emotional stress can produce a distinctive pattern of small (2 to 4 mm), intensely itchy welts surrounded by red flares. This is called cholinergic urticaria, and exercising in warm, humid conditions makes it more likely.
  • Cold: Exposure to cold air, cold water, or cold objects can cause hives on exposed skin within minutes.
  • Pressure: Tight clothing, a heavy bag strap, or prolonged sitting can produce welts along the line of pressure, sometimes hours after the pressure is removed.
  • Sunlight: In rare cases, UV exposure triggers hives on sun-exposed skin within minutes.
  • Skin friction: Firmly stroking or scratching the skin produces a raised welt that traces the exact path of contact. This is called dermatographism, and it affects roughly 2 to 5 percent of the population.

Exercise

Exercise-related hives deserve their own mention because the mechanism is more complex than simple overheating. In one survey, 54% of people with exercise-induced reactions identified food eaten within three to four hours before a workout as a contributing factor. In these cases, the food alone doesn’t cause a reaction, and exercise alone doesn’t either, but the combination does. Alcohol, aspirin, NSAIDs, antibiotics, and even cold remedies taken before exercise have also been linked to reactions. If you notice hives mainly during or after workouts, the timing of meals and medications beforehand is worth paying attention to.

Infections

A recent viral infection is one of the most common triggers for a sudden, unexplained case of hives, especially in children. Upper respiratory infections, stomach bugs, and other common viral illnesses can activate the immune system in ways that lead to widespread hives days or even a couple of weeks after the infection started. These episodes are usually short-lived and resolve on their own as the body clears the virus. Bacterial infections, including urinary tract infections and sinus infections, have also been associated with hive outbreaks.

Stress and Emotional Triggers

Stress doesn’t just make existing hives worse. It can set off new outbreaks. The connection runs through your body’s stress-hormone system. Under short-term stress, your body ramps up cortisol production, which normally keeps inflammation in check. But under chronic, sustained stress, that system fatigues. Cortisol levels drop below normal, and inflammatory markers rise. Research on people with chronic hives has found significantly elevated inflammation markers and stress scores alongside abnormally low cortisol, creating a pattern where stress fuels hives and hives fuel more stress in a self-reinforcing cycle.

Autoimmune Conditions

About 1 in 5 people with chronic hives also have an autoimmune disease. The conditions most commonly linked include thyroid disease (the most frequent), lupus, rheumatoid arthritis, celiac disease, type 1 diabetes, and vitiligo. In these cases, the immune system produces antibodies that mistakenly activate mast cells, causing hives without any external allergen. If hives persist for more than six weeks, your doctor may check for thyroid antibodies and other autoimmune markers as part of the workup.

When No Cause Is Found

Chronic hives, defined as episodes lasting longer than six weeks, affect roughly 0.8% of the U.S. population. In many of these cases, no clear external trigger is ever identified, which is why the condition is often called chronic spontaneous urticaria. Current dermatology guidelines recommend a limited, targeted workup rather than extensive panels of allergy tests, because broad screening rarely identifies a cause and can produce misleading results. Treatment focuses on controlling symptoms by blocking histamine’s effects, and most people with chronic hives eventually see their condition resolve, though it can take months to years.
