Levothyroxine is a synthetic version of thyroxine (T4), the primary hormone your thyroid gland produces. It is prescribed mainly to treat hypothyroidism, a condition where the thyroid doesn’t make enough hormone on its own. It’s also used after thyroid cancer surgery to suppress the growth of any remaining cancer cells. These are its two FDA-approved uses, though the details of when and why doctors prescribe it go well beyond those two categories.

Hypothyroidism: The Most Common Reason

The vast majority of people taking levothyroxine have hypothyroidism. This means their thyroid gland is underactive, leaving the body short on the hormones that regulate metabolism, energy, body temperature, heart rate, and dozens of other functions. Hypothyroidism can be caused by autoimmune disease (most commonly Hashimoto’s thyroiditis), surgical removal of part or all of the thyroid, radiation treatment, certain medications, or problems with the pituitary gland or hypothalamus that control the thyroid.

The FDA approves levothyroxine for all forms of hypothyroidism: primary (originating in the thyroid itself), secondary (caused by a pituitary problem), and tertiary (caused by a hypothalamic problem). It’s approved for adults, children, and newborns. In congenital hypothyroidism, where a baby is born without adequate thyroid function, early treatment is critical for normal brain development.

How It Works in the Body

Your thyroid primarily releases T4, which is relatively inactive on its own. To do its job, T4 must be converted into a more active form called T3. This conversion happens throughout the body, including in the brain. Levothyroxine is a pharmaceutical copy of T4. After you swallow a tablet, it’s absorbed into the bloodstream and then converted to T3 by your cells, mimicking the natural process. For most people, this works well enough to restore normal thyroid function.

Thyroid Cancer: Suppressing Regrowth

After surgery to remove a thyroid cancer, levothyroxine serves a dual purpose. It replaces the thyroid hormone your body can no longer produce (since the gland has been removed), and it’s dosed high enough to suppress TSH, the pituitary hormone that stimulates thyroid tissue to grow. By keeping TSH very low, sometimes near undetectable levels, levothyroxine helps prevent any remaining cancer cells from being stimulated to multiply. This is standard practice for well-differentiated thyroid cancers, the most common type.

Subclinical Hypothyroidism: A Gray Area

Some people have mildly elevated TSH levels but no obvious symptoms. This is called subclinical hypothyroidism, and whether to treat it with levothyroxine depends on the numbers and the person’s circumstances. The strongest case for treatment exists when TSH is above 10 mIU/L. At levels between 5 and 10, treatment may be appropriate if you have symptoms like fatigue, weight gain, or brain fog, or if you have high cholesterol or cardiovascular risk factors. When TSH sits between 3 and 5, levothyroxine generally isn’t recommended because this range overlaps with normal function for many people.

The progression risk matters here too. About 70% of people with a TSH above 8 mIU/L will see it climb past 10 within four years, at which point treatment becomes more clearly justified.

Pregnancy and Fertility

Thyroid hormone, particularly T4, is essential for fetal brain development. The placenta allows T4 to cross to the baby but blocks T3, which is why levothyroxine (synthetic T4) is the preferred treatment during pregnancy over other thyroid preparations.

If you already take levothyroxine and become pregnant, your dose will likely need to increase. The standard recommendation is to bump your dose by about 20 to 30 percent as soon as pregnancy is confirmed, often by simply adding two extra doses per week. Thyroid levels should be checked at least every four weeks throughout pregnancy, with a target TSH below 2.5 mIU/L.

For women not yet on treatment, levothyroxine is recommended if TSH is above 2.5 mIU/L and thyroid antibodies are present, because untreated hypothyroidism during pregnancy raises the risk of miscarriage. Women with a history of Graves’ disease who take levothyroxine need additional monitoring for antibodies that can cross the placenta and affect the baby’s thyroid.

Getting the Dose Right

Levothyroxine dosing is weight-based. The typical full replacement dose for adults is about 1.6 micrograms per kilogram of body weight per day, which works out to roughly 100 to 125 mcg daily for someone weighing around 155 pounds. In practice, doctors start lower and adjust upward in small increments of 12.5 to 25 mcg every four to six weeks, checking blood levels each time, until TSH falls into the target range of 0.4 to 4.0 mIU/L and symptoms improve.

Dose adjustments are common and don’t mean anything is wrong. Weight changes, aging, pregnancy, and other medications can all shift how much you need.

Why Timing and Absorption Matter

Levothyroxine is absorbed in the small intestine, and food significantly reduces how much gets into your bloodstream. On an empty stomach, absorption runs between 60 and 80 percent. With food, it drops considerably. The standard recommendation is to take it at least 30 minutes before eating or drinking anything other than water.

Coffee and tea are common culprits that people overlook. Both interfere with absorption and should be delayed until after that 30-minute window. Calcium supplements, iron supplements, and proton pump inhibitors (used for acid reflux) also reduce absorption and should be spaced several hours apart from your levothyroxine dose. Most people find that taking it first thing in the morning, before getting out of bed or starting their routine, is the easiest way to stay consistent.

Risks of Taking Too Much

Because levothyroxine is replacing a natural hormone, the goal is to land in a precise range. Too high a dose pushes you into a hyperthyroid state, with symptoms that mirror an overactive thyroid: anxiety, rapid or irregular heartbeat, weight loss, trouble sleeping, and chest pain. Over the long term, excess levothyroxine increases the risk of bone loss (potentially leading to osteoporosis) and atrial fibrillation, an abnormal heart rhythm that raises stroke risk. This is why regular blood work matters, especially in the first year of treatment and after any dose change.

Brand vs. Generic Formulations

Levothyroxine is available under several brand names and as a generic. The American Thyroid Association has historically cautioned against switching between different levothyroxine products because small differences in potency could shift your thyroid levels. However, a large FDA-sponsored study of more than 15,000 patients found that people who switched among generic products maintained the same average thyroid function as those who stayed on a single product. The practical takeaway: if your pharmacy switches your manufacturer, it’s reasonable to continue, but get your TSH checked at your next scheduled blood draw to confirm your levels are stable.
