At 10 centimeters dilated, the cervix has opened wide enough to let a baby’s head pass through. That’s roughly the diameter of a large bagel, a cantaloupe slice, or the opening of a mason jar. For most of pregnancy, the cervix is closed to about the width of a fingertip, so reaching 10 centimeters represents a dramatic transformation that happens gradually over hours (sometimes days) of labor.
What 10 Centimeters Actually Looks Like
Ten centimeters is about 4 inches across. To picture it, hold both hands in front of you and make a circle with your fingers roughly the size of a small grapefruit. That’s the opening your cervix creates. At this point, the cervix has essentially disappeared. It started out as a firm, tubular structure (think of the neck of a balloon), but by full dilation it has thinned, softened, and pulled back so completely that a care provider doing an internal exam can no longer feel it around the baby’s head.
For comparison, earlier stages of dilation look very different. At 1 centimeter, the cervix barely fits a single finger. At 3 centimeters, two fingers fit snugly inside. At 6 centimeters, the opening is about the width of a small cookie. The jump from 6 to 10 is where labor intensifies the most.
How the Cervix Gets There
Two things happen simultaneously during labor: dilation and effacement. Dilation is the cervix opening wider. Effacement is the cervix thinning out and shortening. Your cervix needs to reach both 100% effaced and 10 centimeters dilated before it’s safe to push.
Think of the cervix like the neck of a balloon. As pressure builds behind it (from contractions pushing the baby downward), the neck draws up, gets shorter, and eventually becomes part of the balloon wall itself. That’s effacement. Meanwhile, the opening at the bottom stretches wider and wider. When the baby’s head is pressing directly against the cervix, that downward force accelerates both processes. By the time you reach 10 centimeters, the cervix has gone from a thick, rigid tube (roughly 3 to 4 centimeters long) to tissue so thin and stretched that it blends into the lower part of the uterus.
How Providers Measure Dilation
There’s no ultrasound or device involved. Dilation is measured by hand during a vaginal exam. A provider inserts two gloved fingers and gently feels the cervical opening, estimating how many centimeters apart their fingers can spread. At 1 centimeter, one finger fits tightly. At 4 centimeters, two fingers fit loosely. Beyond 5 centimeters, the provider is gauging how much further the opening extends past the width of two fingers.
At 10 centimeters, the provider reaches in and feels no cervix at all around the baby’s head. That’s the clearest sign of full dilation: the cervix is completely out of the way. At this point, pushing can begin without risk of tearing the cervix.
What Full Dilation Feels Like
The stretch from about 7 or 8 centimeters to 10 is called transition, and it’s widely considered the most intense phase of labor. Contractions come very close together, often lasting 60 to 90 seconds with only a minute or two between them. Many people experience nausea, shaking, intense pressure in the pelvis, and a feeling of being overwhelmed. The good news is that transition is also the shortest phase, typically lasting 30 minutes to 2 hours.
Once you hit 10 centimeters, many people feel an unmistakable urge to push, similar to the pressure of a bowel movement but much more intense and lower in the pelvis. Some describe a brief window of relief between transition and active pushing, where contractions space out slightly and the body gets a moment to rest. Others feel the urge to push almost immediately.
What Happens After 10 Centimeters
Full dilation marks the end of the first stage of labor and the start of the second: pushing the baby out. How long this takes varies widely. For a first baby, pushing can last anywhere from a few minutes to a few hours. People who have given birth before tend to push for a shorter time. Having an epidural also tends to lengthen the pushing stage because it reduces the sensation that guides your effort.
As you push, the baby moves further down through the pelvis. Care providers track this using “fetal station,” a scale that describes where the baby’s head sits relative to your pelvic bones. At station 0, the head is level with the middle of the pelvis. By station +4 or +5, the baby’s head is visible at the vaginal opening, which is the moment just before birth.
It’s worth knowing that reaching 10 centimeters doesn’t always mean instant pushing. Sometimes the baby’s head still needs to descend further into the pelvis, and your provider may suggest “laboring down,” which means waiting for contractions to move the baby lower before you start actively pushing. This is especially common with an epidural, where you may not feel a strong urge to push right away.
How Long It Takes to Reach 10 Centimeters
Early labor (from 0 to about 6 centimeters) is the longest stretch and the hardest to predict. It can take hours or even a day or more, especially with a first baby. Many people spend this phase at home, timing contractions and waiting for them to become regular. Progress during early labor is often slow and uneven, with long plateaus where the cervix seems stuck at the same number.
Active labor (roughly 6 to 10 centimeters) moves faster and more predictably, typically lasting four to eight hours. The rate of dilation picks up noticeably once active labor begins, with many people progressing about a centimeter per hour, though this varies. Second or subsequent labors often progress faster across every stage.
