What Does a Bone Bruise Look Like on the Outside?

A bone bruise often shows little or nothing on the skin’s surface. You may see some discoloration or swelling over the area, but many bone bruises produce no visible marks at all, even when the pain is significant. That disconnect between what you see and what you feel is one of the most frustrating things about this injury.

What You Might See on the Skin

Skin bruising or discoloration can accompany a bone bruise, but it’s not guaranteed. When surface discoloration does appear, it typically looks like a standard bruise: reddish at first, shifting to purple or blue, then fading through green and yellow as it heals. The key difference is that this surface bruise often seems disproportionately mild compared to the deep, aching pain underneath.

In many cases, though, you won’t see any visible changes at all. Because the injury sits deep inside the bone, in the spongy tissue beneath the hard outer shell, the bleeding and swelling happen where your eyes can’t reach. You might notice some puffiness or warmth over the area, particularly around joints like the knee or ankle, but the skin itself can look completely normal. This is why bone bruises are so commonly missed or dismissed as “just a knock” in the early days after an injury.

Why the Pain Doesn’t Match What You See

A regular bruise happens when small blood vessels in your skin and soft tissue break, spilling blood into the layers you can see. A bone bruise is a different injury entirely. It involves tiny fractures in the spongy, inner mesh of bone tissue, along with fluid buildup (called bone marrow edema) inside the bone itself. All of that damage is sealed beneath the bone’s hard outer surface, so there’s no visible leak of blood toward the skin in many cases.

This mismatch is what brings most people to a search engine. The area hurts more than a muscle bruise, the pain is sharp and deep when you press on it or bear weight, and yet the outside of your leg or arm looks fine. That pattern, deep pain with minimal visible evidence, is actually one of the hallmarks of a bone bruise.

How It Differs From a Regular Bruise

With a standard soft tissue bruise, you typically see a clear patch of discoloration within hours, and the tenderness is spread across the surface. The pain fades noticeably within a week or two. A bone bruise behaves differently in almost every way:

  • Pain location: The tenderness is pinpoint and deep. Pressing directly over the bone reproduces the pain, while pressing the surrounding muscle may feel fine.
  • Duration: Soft tissue bruises usually resolve within two to three weeks. Bone bruises commonly take weeks to months, and some take over a year to fully heal.
  • Swelling pattern: Swelling from a bone bruise tends to sit right around a joint and may feel firm rather than squishy. A muscle bruise swells more diffusely across the soft tissue.
  • Skin color: A regular bruise almost always produces visible discoloration. A bone bruise may produce none.

Where Bone Bruises Happen Most

Bone bruises most commonly show up around the knee, ankle, heel, shin, and wrist, areas where bone sits close to the skin surface and absorbs direct impact. The knee is especially common after sports injuries or falls, where the tibia and femur can slam together. Because these joints have relatively thin layers of soft tissue over the bone, you’re slightly more likely to see some surface swelling or faint discoloration at these sites, but it’s still inconsistent.

The heel is another frequent location, particularly in runners. A bone bruise on the calcaneus (heel bone) may show no visible change at all, yet standing and walking can produce sharp, deep pain that doesn’t respond to the usual ice and rest you’d use for a soft tissue injury.

How Bone Bruises Are Diagnosed

Because the outside of your body gives so few clues, diagnosis almost always requires imaging. Standard X-rays won’t show a bone bruise. Neither will CT scans. The only reliable way to see a bone bruise is an MRI, which can detect the fluid buildup inside the bone marrow. On an MRI, a bone bruise appears as a bright white area within the normally dark bone, signaling inflammation and swelling deep inside.

This is why many bone bruises go undiagnosed for weeks. A person gets hurt, sees minimal bruising, gets an X-ray that looks clean, and assumes everything is fine. But the deep pain persists, and only an MRI eventually reveals the bone contusion hiding underneath.

What Recovery Looks Like

Bone bruises heal slowly compared to soft tissue injuries. Most take anywhere from several weeks to several months, and larger or more severe bruises can linger for a year or more. During that time, any surface discoloration you had will fade long before the bone injury actually resolves, which can create a false sense that you’re fully healed.

Recovery generally involves reducing stress on the injured bone. That might mean limiting weight-bearing activities, using a brace or crutches, and cutting back on exercise. The bone goes through similar healing stages as other injuries, laying down new tissue and gradually reabsorbing the fluid, but it all happens internally where you can’t track it visually. Pain level during activity is your best external gauge of how healing is progressing.

When a Bone Bruise Signals Something More Serious

Most bone bruises heal without complications, but a severe one can disrupt blood flow to the surrounding bone tissue. When blood supply to a section of bone gets cut off, that bone can gradually break down, a condition called avascular necrosis. The timeline from early symptoms to joint damage ranges from a few weeks to over a year. If the affected bone sits within a joint, it can eventually collapse and lead to arthritis.

The warning signs worth paying attention to are persistent deep pain that doesn’t improve over several weeks, increasing stiffness in a nearby joint, or pain that worsens with activity rather than gradually improving. A bone bruise that still hurts significantly after two to three months deserves a follow-up, even if the surface of your skin has looked normal the entire time.
