What Does a Bone Spur Feel Like? Symptoms by Location

Most bone spurs cause no sensation at all. These small bony growths form along the edges of joints, often as a response to long-term wear and tear, and the majority are discovered accidentally on X-rays taken for other reasons. When bone spurs do cause symptoms, what you feel depends entirely on where the spur is and what surrounding tissue it presses against. The sensations range from a dull ache and stiffness to sharp, stabbing pain, tingling, or a grinding feeling during movement.

Why Most Bone Spurs Feel Like Nothing

Bone spurs in the lumbar spine (lower back) are a good example of how common and how quiet these growths can be. Research on lumbar spondylosis shows that people with osteophytes experience symptoms at the same rate as people without them. The spur itself has no nerve endings. It only produces pain or other sensations when it’s large enough or positioned in a way that irritates nearby soft tissue, presses on a nerve, or physically blocks a joint from moving through its full range.

This means you could have bone spurs right now and never know it. It also means that if you’re feeling joint pain, a bone spur visible on imaging isn’t necessarily the cause. The location and what it’s pressing on matter far more than the spur’s existence.

When a Bone Spur Presses on a Nerve

The most intense sensations from bone spurs happen when a spur compresses or pinches a nerve, a condition called radiculopathy. This is most common in the spine, particularly the neck (cervical spine) and lower back. Instead of localized joint pain, nerve compression creates symptoms that travel. You might feel tingling, numbness, or a pins-and-needles sensation radiating down an arm or leg, depending on which nerve is affected.

The pain from nerve compression is often described as sharp, electric, or burning, and it can come with muscle weakness. In the lower body, this sometimes means difficulty controlling your legs, a feeling of heaviness, or even trouble with bladder or bowel control in severe cases. These symptoms can be confusing because they show up far from where the spur actually is. A bone spur in your lower back, for instance, can create pain or numbness in your foot.

Heel Spurs and Foot Pain

Heel spurs are among the most commonly searched types, and here the sensation is distinctive. If a heel spur becomes symptomatic, which many don’t, the pain typically mimics plantar fasciitis: an intense stabbing feeling in the bottom of the heel, worst with your first steps in the morning or after sitting for a long period. The pain often eases once you’ve been walking for a few minutes as the tissue warms up, then returns when you stand again after resting.

For heel spurs specifically, research published in the International Journal of Surgery found that the size of the spur does correlate with pain severity and reduced foot function. Longer spurs were associated with higher pain scores and greater disability, and the correlation held even after accounting for age and body weight. This is one of the few locations where spur size has been clearly linked to how much it hurts.

Shoulder Spur Sensations

Bone spurs on the top of the shoulder blade (the acromion) create a specific pattern of symptoms. As the spur grows, it narrows the space where the rotator cuff tendons pass through. This causes a catching or pinching sensation when you raise your arm overhead, reach behind your back, or lift something away from your body. The pain usually feels like it originates from the front of the shoulder.

You might also hear or feel a cracking or popping sound when moving the shoulder through certain positions. Lying on the affected side at night often makes the pain worse, which is a hallmark that distinguishes shoulder impingement from other types of shoulder pain. Over time, the irritation from the spur can weaken the rotator cuff, making the shoulder feel progressively less stable.

Hip and Knee Spurs

Bone spurs in the hip can be tricky to identify by feel alone because the pain often refers to other locations. A hip spur might create a deep ache in the groin, the front of the thigh, or even the knee, making you think the problem is somewhere else entirely. The most telling sign is reduced range of motion: difficulty rotating the hip, crossing your legs, or bending to tie your shoes, combined with stiffness that’s worse after sitting still.

In the knee, bone spurs typically make extending and bending the leg painful. You might feel a locking or catching sensation, as though something is physically blocking the joint from moving smoothly. Some people describe a grinding feeling during movement. The pain tends to be localized to the joint line and worsens with weight-bearing activities like climbing stairs or walking on uneven ground.

Bone Spurs in the Fingers

Bone spurs at the finger joints, called Heberden’s nodes when they appear at the joint closest to the fingertip, have a very different feel from spurs in larger joints. These present as small, firm, pea-sized bumps you can see and touch. The joints become swollen, stiff, and sometimes visibly enlarged. Pain tends to be a combination of aching and throbbing, and it gets worse when you use your hands for fine motor tasks like buttoning a shirt or opening a jar.

Over time, the fingers may lose range of motion and become permanently enlarged. Heberden’s nodes are a sign of advanced osteoarthritis, so by the time you can see and feel them, the underlying joint has already experienced significant cartilage loss.

How to Tell if Your Pain Might Be a Bone Spur

Bone spur pain shares features with many other joint and nerve conditions, so sensation alone isn’t enough for a definitive answer. That said, a few patterns are worth paying attention to. Bone spur symptoms tend to develop gradually over weeks or months rather than appearing suddenly. The pain is usually tied to specific movements or positions rather than being constant. And stiffness that’s worst after periods of inactivity, then loosens up with gentle movement, is a common thread across nearly every location.

If your symptoms include tingling, numbness, or muscle weakness, a nerve is likely involved, and these warrant prompt evaluation. Joint pain combined with a visible bump (as in the fingers) or a mechanical sensation like catching or locking (as in the knee or shoulder) also fits the bone spur pattern more closely than muscle strain or inflammation alone.

Imaging is the only way to confirm a bone spur. Plain X-rays show them clearly. But the presence of a spur on an image doesn’t automatically explain your symptoms, so a careful match between where the spur is, what it’s pressing on, and what you’re feeling is what ultimately determines whether the spur is the problem.
