What Does a Broken Metatarsal Feel Like?: Symptoms Explained

A broken metatarsal typically causes sharp, intense pain localized to one spot on the top or outer edge of your foot, along with swelling that develops rapidly in the first few hours. Unlike a sprain, where pain spreads diffusely around a joint, a metatarsal fracture produces a very specific point of tenderness directly over the broken bone. The experience differs depending on whether the break happens suddenly from an injury or develops gradually as a stress fracture.

Pain Location and Quality

The metatarsals are the five long bones that run from the middle of your foot to the base of your toes. When one breaks, most people can pinpoint exactly where it hurts. If you press gently along the top of your foot and one spot produces sharp, intense pain while the surrounding area feels comparatively fine, that’s a hallmark sign of a fracture rather than a soft tissue injury. Sprain pain, by contrast, tends to be spread out around a joint.

With an acute fracture, the pain is immediate and often severe enough that you instinctively stop putting weight on that foot. Some people describe a snapping sensation at the moment of injury, though the metatarsals have very limited movement at the fracture site, so you’re unlikely to feel grinding or a gap in the bone. The pain usually worsens with any attempt to push off the ground while walking, since the metatarsals bear a significant portion of your body weight during that phase of your step.

Fifth metatarsal fractures (the bone along the outer edge of your foot) are the most common type. Turning your foot inward, even slightly, tends to spike the pain because that motion puts maximum strain on the fracture site.

What a Stress Fracture Feels Like

Not all metatarsal fractures happen in a single moment. Stress fractures develop slowly from repetitive motion, and their symptoms follow a different pattern that can be confusing. In the early phase, you may only notice a dull ache during activity, like running or long periods on your feet. The pain stops when you rest, so it’s easy to dismiss as soreness or fatigue.

Over days or weeks, the pain gets worse during activity and takes longer to fade afterward. Eventually, you may feel it even when you’re sitting or lying down. This progression from “only hurts when I’m active” to “hurts all the time” is the signature pattern of a stress fracture. Patients with stress fractures often report that the symptoms have been present for several weeks before they seek help, because the early stages mimic overuse soreness. The key difference is that normal muscle soreness improves with rest over a few days, while stress fracture pain keeps returning and gradually escalates.

Swelling, Bruising, and Visible Changes

Swelling is one of the earliest and most noticeable signs. After an acute fracture, the top of your foot can puff up significantly within the first hour, sometimes enough to make your shoe feel tight or impossible to put on. Bruising (a blue or purple discoloration) often follows, though it may take 12 to 24 hours to become visible and can spread across a wider area than you’d expect.

Stress fractures produce more subtle swelling. You might notice a slight puffiness on the top of your foot that comes and goes with activity, without the dramatic bruising of an acute break.

How It Affects Walking

One of the clearest signals of a broken metatarsal is what happens when you try to walk. With a full fracture, putting weight on the foot is frequently impossible. Complete inability to stand on the injured foot, especially right after the injury, strongly suggests bone damage rather than a sprain. Even with severe sprains, most people can at least hobble or bear partial weight.

That said, walking does not rule out a fracture. Some non-displaced fractures (where the bone cracks but stays in alignment) and stress fractures allow limited weight-bearing, especially in the first minutes after injury when adrenaline masks the pain. If you could walk on it initially but the pain worsened significantly over the following hours, a fracture is still possible.

Research tracking real-world walking patterns after a fifth metatarsal fracture found that the injury affects your gait for a surprisingly long time. Walking speed and stride length drop immediately and take roughly 16 weeks to return to normal. Your ability to push off the ground with the injured foot decreases dramatically, from a normal angle of about 77 degrees down to 54 degrees. This reduced push-off force even changes how your uninjured foot lands, because the two feet work as a coordinated system. In practical terms, this means months of walking more slowly and with shorter steps, even after the initial pain has improved.

Broken Metatarsal vs. Sprained Foot

The overlap in symptoms is real, and even experienced clinicians use a specific checklist (called the Ottawa Foot Rules) to decide whether an X-ray is needed. But a few features help distinguish the two:

  • Point tenderness over bone vs. joint pain: A fracture hurts sharply when you press directly on a specific bone. A sprain produces more diffuse pain centered around a joint or along ligaments.
  • Weight-bearing ability: Sprains are painful but you can usually put at least some weight on the foot. Fractures more often make weight-bearing impossible.
  • Pain with specific movements: A fifth metatarsal fracture flares when the foot turns inward. Sprains tend to hurt with a broader range of movements.

The clinical guideline for determining whether you need an X-ray is straightforward: if you have midfoot pain plus bone tenderness at the base of the fifth metatarsal (the bony bump on the outer edge of your foot), tenderness at the navicular bone (the inner midfoot), or you can’t bear weight both immediately after the injury and when attempting four steps, imaging is warranted.

Healing Timeline and What to Expect

How long a broken metatarsal takes to heal depends heavily on where the fracture is and what type it is. For the most common acute fractures that stay in alignment, conservative treatment with a stiff-soled shoe or walking boot leads to healing in 6 to 8 weeks. Pain and discomfort typically begin to subside enough to start gradually increasing weight on the foot within 3 to 6 weeks.

Jones fractures, which occur in a specific zone of the fifth metatarsal with poor blood supply, are more stubborn. These often require 6 to 8 weeks of keeping all weight off the foot in a cast. Stress fractures in the shaft of the metatarsal can be the slowest to heal, sometimes requiring immobilization for up to 20 weeks before X-rays show the bone has knit together. Even then, some of these fractures fail to heal completely.

Beyond bone healing, the period of immobilization itself causes muscle weakness and joint stiffness in the ankle. This is part of why real-world mobility takes about 15 to 16 weeks to return to pre-injury levels, even for fractures where the bone heals within 6 to 8 weeks.
