What Does a Cavity Look Like? From Spots to Holes

A cavity’s appearance depends entirely on how far the decay has progressed. In its earliest stage, a cavity looks like a small white spot on the tooth surface, chalky and dull compared to the surrounding enamel. As it advances, that spot darkens to brown or black, and eventually a visible hole forms in the tooth. Knowing what each stage looks like can help you catch decay before it becomes painful or expensive to treat.

The Earliest Sign: White Spots

Before a cavity becomes a hole, it starts as a patch of demineralization. Acids from bacteria dissolve minerals out of the enamel, creating tiny pores in the tooth’s surface. These pores scatter light differently than healthy enamel, producing an opaque white spot that looks chalky or matte. The spot loses the natural shine of the surrounding tooth and may only be visible after the tooth surface has been dried, which is one reason dentists blow air on your teeth during an exam.

At this stage, the surface of the tooth is still intact. There’s no hole, no pain, and no sensitivity. Many people never notice these white spots on their own, especially when they appear on back teeth or between teeth. The good news is that white spot lesions can sometimes be reversed with fluoride and improved oral hygiene before they progress into an actual cavity.

What a Cavity Looks Like as It Gets Worse

Once decay breaks through the enamel surface, you’re dealing with a true cavity. Here’s how the appearance changes at each stage:

In early enamel decay, the white spot darkens to a light brown. You may notice a small pit or rough area you can feel with your tongue. The tooth surface has broken down, but the damage is still shallow.

When decay reaches the dentin, the layer beneath enamel, things accelerate. Dentin is softer and more porous, so the cavity grows faster. Visually, the spot becomes a deeper brown or dark yellow. The affected dentin often looks wet or moist and feels soft. You may start feeling sensitivity to hot, cold, or sweet foods because dentin contains tiny tubes that connect to the tooth’s nerve.

In advanced decay, the cavity appears as a dark brown or black hole. The opening may be small, but the damage underneath can be extensive because decay spreads outward once it hits dentin. Sometimes you’ll see a dark shadow visible through otherwise intact-looking enamel, a sign that significant decay is hiding beneath the surface. At this point, pain can become constant rather than triggered by food or drinks.

If decay reaches the pulp (the innermost part containing nerves and blood vessels), swelling and infection can develop. A dental abscess may form at the root, sometimes visible as a small pimple-like bump on the gum near the affected tooth.

How Cavities Look in Different Locations

Not all cavities look the same, partly because they form on different surfaces of the tooth.

On the chewing surfaces of molars, cavities typically start in the grooves and pits. You might see a dark line or stained fissure. When the decay is only in the enamel of the fissure, it may just look like a brown or black line with no surrounding changes. But if you notice a white, pearly halo around a stained groove, that opalescent ring signals that decay has spread deeper into the dentin below, even if the enamel surface still looks smooth and shiny.

Along the gumline, cavities appear as chalky white bands or brown crescents hugging the base of the tooth. These are common in older adults as gums recede and expose the softer root surface.

Between teeth, cavities are the hardest to spot visually. They develop on the flat contact surfaces where two teeth press together, hidden from view. You might notice a grayish shadow when looking at the tooth from a certain angle, or the floss may catch or shred in that area. These interproximal cavities are the main reason dentists take bitewing X-rays, since the early mineral loss on these surfaces is nearly impossible to see with the naked eye.

Cavity vs. Stain: How to Tell the Difference

Dark spots on teeth aren’t always cavities. Coffee, tea, red wine, and tobacco can leave brown or black stains that look alarming but sit only on the surface. Here are practical differences to look for:

  • Texture: A stain feels smooth when you run your tongue over it. An active cavity often feels rough, sticky, or soft. You may feel a catch or a small ledge.
  • Color pattern: Stains tend to appear on multiple teeth in similar locations, especially along the gumline or in grooves. A cavity is usually isolated to one spot on one tooth.
  • Appearance when wet vs. dry: The earliest cavities (white spots) disappear when the tooth is wet and reappear when dry. Stains look the same whether the tooth is wet or dry.
  • Sensitivity: Stains don’t cause pain or sensitivity. If a dark spot is accompanied by a zing when you drink cold water, decay is more likely.

That said, some stains sit in fissures that also have early decay, making it hard to tell them apart without a dental exam. A dark fissure without any surrounding white or opalescent halo and no visible changes on X-ray is more likely just a stain.

What Cavities Look Like in Children

Cavities in baby teeth follow a distinctive pattern that’s worth recognizing. In young children, decay most commonly hits the four upper front teeth first. It begins as a white band of demineralization running along the gumline, which parents often miss because it blends in with the tooth color. Over time, those white bands darken into brown or black rings that encircle the necks of the teeth like a collar.

In severe cases, the crowns of the upper front teeth can be completely destroyed, leaving only root stumps. One telltale feature of this pattern: the lower front teeth are usually spared or only mildly affected, because the tongue and saliva flow from the salivary glands underneath protect them. If you notice chalky white lines near the gums on your child’s upper front teeth, that’s the earliest visual warning sign.

Cavities You Can’t See

Some cavities never become visible to you at all. Decay between back teeth can grow extensively before any external sign appears, because the outer enamel can remain largely intact while the dentin underneath is hollowed out. The first clue might be sensitivity or pain rather than a visible change.

This is why X-rays remain essential for detecting decay. On a bitewing X-ray, a cavity appears as a dark shadow within the normally bright white outline of the tooth. Dentists can spot mineral loss on these images before a cavity becomes large enough to see or feel. AI-assisted analysis of dental images is increasingly used alongside traditional methods, with detection accuracy ranging from about 73% to 99% depending on the system and the type of image analyzed.

If you’re trying to assess your own teeth at home, use good lighting and a small mirror. Look for any white chalky patches, brown or black spots, visible holes, or areas where the tooth surface looks uneven. But keep in mind that the most consequential cavities are often the ones you can’t see, developing silently between teeth or under old fillings where only imaging can reveal them.
